1.Cultivation strategy and path analysis on big brand Chinese medicine for small and medium-sized enterprises.

Small and medium-sized enterprises (SMEs) are important components in Chinese medicine industry. However, the lack of big brand is becoming an urgent problem which is critical to the survival of SMEs. This article discusses the concept and traits of Chinese medicine of big brand, from clinical, scientific and market value three aspects. Guided by market value, highlighting clinical value, aiming at the scientific value improvement of big brand cultivation, we put forward the key points in cultivation, aiming at obtaining branded Chinese medicine with widely recognized efficacy, good quality control system and mechanism well explained and meanwhile which can bring innovation improvement to theory of Chinese medicine. According to the characters of SMEs, we hold a view that to build multidisciplinary research union could be considered as basic path, and then, from top-level design, skill upgrading and application three stages to probe the implementation strategy.

2.Application of native pelvis and ureter in kidney transplantation

Objective To study the application of native pelvis and ureter in kidney transplantation. Methods Between 1977 and 2001, 1 364 kidney transplantations were performed in our institution. 18 cases of ureteral complication after kidney transplantation were treated with native pelvis and ureter. The mean age of the patients was 42 years. In 10 patients with urine leakage, 3 cases underwent pyeloureterostomy using native ureter, 2 cases ureteroureterostomy using native ureter and 5 cases pyelopyelostomy using native pelvis. Five cases due to short ureter underwent ureteroureterostomy using native ureter. Three cases of ureteral stenosis were subjected to ureteroureterostomy using native ureter.Results Two grafts were rejected. Sixteen patients developed a normal function of kidney postoperatively (follow up: 3 months to 8 years). Conclusion Application of native pelvis and ureter might be a therapy of choice when secondary reconstruction by re-ureteroneocystostomy is not possible.

6.A Case of Cyanide Poisoning Caused by Acrylonitrile Inhalation.

Acrylonitrile is most commonly used aliphatic nitrile compounds characterized by the structural formula R-C=N and used mossy to make acrylic fibers, plastics, synthetic rubber, and wall coverings. In recent, because of its extensive usage and the rapid expansion of the chemical industry, many poisonings have been reported and many studies on its health effects have been performed. Acute toxicity resembles cyanide poisoning and results mainly in effects on the nervous system. High exposure also can cause temporary damage to red blood cells and the liver and can cause lead to death. Because long-term occupational exposure to the acrylonitrile has been with cancer in humans, the U.S EPA classifies acrylonitrile as probable carcinogen. For this reason, The federal government has developed regulations and advisories to protect individuals firm the potential health effects of acylonitrile in the environment, but there are few studies, case reports and regulations of the government in our country. We experienced acute poisoning caused by acrylonitrile inhalation that occurred in an industrial accident. So, we report this case with literature reveiw.

7.Application of argon beam coagulator under electronic bronchoscope in the management of pediatric glottic and subglottic diseases.

OBJECTIVE:
To summarize clinical experience of application of argon knife under electronic bronchoscope in the management of pediatric glottic and subglottic diseases.
METHOD:
Reports in the literature were reviewed. Outcome and prognosis of 4 cases with pediatric glottic and subglottic diseases managed with argon beam coagulator technique in Shanghai children's Medical Center since 2011 were retrospectively analyzed.
RESULT:
Granulation tissue hyperplasia in 3 cases with subglottic lesions did not recur after surgery. Scarring or stenosis were not found in the airway. Intubation of the trachea and tube of tracheotomy was pulled out successfully in all cases, restoring normal breathing. No lesion recurrence was found in 1 case with squamous cell moderate atypical hyperplasia one month after the surgery.
CONCLUSION
Electronic bronchoscope has advantages of clear image, fine and soft lens body, optional bending, angle conversion, accurate positioning. And the argon knife with small head diameter can pass the pediatric supraglottic. It can be manipulated precisely with short operation time and no damage to surrounding normal tissues. Its worth in popularization and application of management for with glottic and subglottie disease in pediatric patients.

9.Diagnostic Value of CT in the Hip Joint Dislocation

Objective To discuss the diagnostic value of CT in the hip joint dislocation.Methods We analyzed and compared 17 cases of the hip dislocation examined by X-ray and CT.Results X-ray plain film displayed hip joint dislocation in 15 cases,fractures in 10 cases,soft tissue swelling in 9 cases,widened hip joint space in 6 cases and bone fragment in 3 cases after restoration.CT showed hip joint dislocation and fractures in 17 cases,soft tissue swelling in 15 cases,widened hip joint space in 11 cases and bone fragment in 10 cases after restoration.Conclusion For the direction and the degree of hip joint dislocation,acetabulum fracture,bone fragment in articular space and soft tissue swelling,CT has distinct advantage.We find that CT may be one of a routine method in hip joint dislocation,especially after restoration.
10.SARS-CoV S protein induces chemokine IP-10 expression in airway epithelial cells

AIM: To investigate the molecular mechanism by which the SARS-CoV S protein induces chemokine IP-10 in airway epithelial cells.METHODS: cDNA microarrays were used to screen the gene expression profiles of human bronchial epithelial cells(16HBEs) stimulated by SARS-CoV S protein.In addition,RT-PCR,EMSA,and Western blotting were performed to analyze the phosphorylation of JAK/STAT signal pathway.The changes of IRF-1 and IP10 gene expression and the influence by the corresponding inhibitors were analyzed.RESULTS: IRF-1,a key transcription factor of the JAK/STAT signal pathway,was activated in human bronchial epithelial cells after stimulation by the S protein of SARS-CoV.The IP-10 gene expression was detected 2 h following the phosphorylation of STAT1 after 15 min,which was blocked by STAT1or JAK2 inhibitors.Electrophoretic mobility shift assay(EMSA) demonstrated that the nuclear proteins bound to ISRE and GAS but not NF-?B DNA motif.CONCLUSION: The SARS-CoV S protein induces IP-10 gene expression in human bronchial epithelial cells through activation of the JAK/STAT signal pathway,suggesting that the JAK/STAT signal pathway activated by virus plays key roles in virus infection related acute lung injury.
