1.Effects of xianggui pill on cytokine in endometriosis model rat.

OBJECTIVETo study the adjustment of Xianggui pill on the cytokine of endometriosis model rat, and investigate the mechanism of Xianggui pill on the treatment of endometriosis.
METHODTo set up endometriosis model by rat self-endometria transplantation, drench sodium chloride, Xianggui pill elixation or Danazol after grouping, and to detect the contents of interleukin-8 (IL-8) and tumor necrosis factor-alpha (TNF-alpha) by ELISA.
RESULTThe contents of IL-8, TNF-alpha in the peripheral blood and peritoneal fluid of model group were higher than that of the blank group; The quality of allotopia growth intima tissue, the quantity of macrophage in peritoneal fluid and the contents of IL-8, TNF-alpha in the Xianggui pill group and Danazol group were all lower than those of the model group; but there was no significant difference of each target between the Xianggui pill group and Danazol group.
CONCLUSIONXianggui pill can restrain significantly the growth of allotopia intima tissue, and has apparently adjustment to the cytokine.

2.Effect of Chinese herbal medicine for nourishing yin, supplementing qi, and activating blood on the Th1/Th2 immune balance in peripheral blood in patients with primary Sjogren's syndrome.

OBJECTIVETo observe the effect of Chinese herbal medicine for nourishing yin, supplementing qi, and activating blood on the expression of interferon-γ (IFN-γ)/interleukin-4 (IL-4) in peripheral blood and disease activity in primary Sjogren's syndrome (pSS) patients, and to study the relationship between the immune balance of Th1/Th2 and the disease activity.
METHODSA total of 66 pSS patients were randomized with tossing coins method into two groups: the integrative therapy group (34 cases) and the control group (32 cases); and 28 healthy subjects were taken as the normal group. The integrative therapy group was treated by Chinese herbal medicines for nourishing yin, supplementing qi, and activating blood combined with hydroxychloroquine sulfate tablets and the control group was treated with hydroxychloroquine sulfate tablets. The treatment course was 3 months for both groups. The levels of serum immunoglobulin G (IgG), erythrocyte sedimentation rate (ESR), IFN-γ and IL-4 in peripheral blood were measured before and after treatment.
RESULTSCompared with the normal group, the levels of IgG, ESR, IFN-γ and IL-4 were significantly increased in pSS patients (P<0.05). Remarkably, after 3 months of treatment, these levels were dramatically decreased in both the integrative therapy group and the control group, although still higher than the normal group. The levels of IgG, ESR, IFN-γ and IL-4 in the integrative therapy group were lower than the control group and the same group before treatment (P<0.05). The ratio of IFN-γ/IL-4 also significantly decreased after treatment. Moreover, the level of IFN-γ and the ratio of IFN-γ/IL-4 in the integrative therapy group were significantly lower than the control group (P<0.05). For all patients the ratio of IFN-γ/IL-4 before and after treatment was positive correlated with the levels of IgG and ESR.
CONCLUSIONChinese herbal medicine for nourishing yin, supplementing qi, and activating blood can alleviate the disease activity of pSS by regulating the immune balance of Th1/Th2.

3.Baseline study on a stroke-related cohort in the rural community of Shanghai.

OBJECTIVETo present data from a baseline investigation on stroke-related cohort population in rural area of Shanghai.
METHODSA cross-sectional study was carried out in a cluster sampling population aged 40 years and over. General information and data on common risk factors in the population were gathered and cerebral vascular hemodynamic indexes were checked. Hemodynamic score was estimated according to single indexes by unified methods. 5335 persons who had met the inclusion criteria were enlisted in the study. Exposure level of risk factors, prevalence of stroke, and hemodynamic indexes were analyzed and distributional characteristics were described.
RESULTSExposure rate of hypertension, heart disease, diabetes, family history of hypertension, overweight or obesity in males were 31.74%, 6.09%, 1.16%, 3.22%, 17.64%, 29.68% and were 32.76%, 9.22%, 1.55%, 3.84%, 19.22%, 29.44% in female respectively. Standard prevalence of stroke was 1167.3/100000, which in male was significantly higher than that in female (P < 0.05). The change of cerebral vascular hemodynamic indexes was significantly associated with age. Hemodynamic score in 21.3% of the subjects was below 75 points.
CONCLUSIONSAmong population of 40 years old and over in rural areas, hypertension was the principal risk factor regarding the rate of stroke. Prevalence of stroke in males was significantly higher than that in females. Abnormal rate of hemodynamic score was about 20% in this population.

5.Impact on the PDD of MV X-ray for the glass mirror in linac.

OBJECTIVETo explore the effect on the iso-center and OAR and PDD of the MV class X-ray coming from the linac when we use the glass mirror to replace an imported one.
METHODData collected by using a film and 3D water phantom OAR and PDD measuring methods based on the different conditions. Comparison and analysis of the effect levels for different thickness and material mirrors are made.
RESULTThe PDD max error of the 4 mm general glass mirror is 6%.When the 8MV X-ray 720 Gy irradiation is given, the light field appears a dual cross lines and has darker brightness. The PDD max error of the 1.5 mm quartz glass mirror is less than 1%. When it accepts the equal radiation dose, the light field iso-center and OAR and field brightness are agreed well with clinical demands.
CONCLUSIONOur practice shows that the 4 mm general glass mirror can't suffer the MV class X-ray irradiation but the thin quartz glass mirror can. So the thin quartz glass mirror can be used for the linac.

6.Association between interleukin-18 gene promoter polymorphisms and Alzheimer's disease

Objective To investigate the role of interleukin-18 (IL-18) gene promoter polymorphisms in the susceptibility of sporadic Alzheimer's disease (SAD). Methods A total of 109 patients with SAD and 109 healthy controls matched for age and gender were enrolled in our study with case-control method. Polymerase chain reaction with sequence specific primer was used to detect the single nucleotide polymorphisms (C/A) of gene promoter of IL-18 at position -607. Results Significant differences in the C/A genotype and allele distributions of IL-18 promoter at position -607 were noted between patients with SAD and controls (P=0.041 and P=0.021, respectively). The CC genotype frequency of gene promoter of IL-18 at position -607 in patients with SAD was significantly higher than that in healthy controls (x2=4.109, P=0.043); and the people with this genotype had a 1.90-fold increased risk as compared with the people with other genotypes (OR=1.90, 95%CI: 1.017-3.550).Conclusion The C/A polymorphisms of IL-18 gene promoter at position -607 are correlated with SAD in Han Chinese population. People with -607CC in IL-18 gene promoter have an increased risk of SAD.
7.Expression of a human single-chain Fv antibody against HBsAg in Pichia pastoris.

To express and secrete native HBscFv (anti-HBsAg single-chain Fv) in P. pastoris, HBscFv was amplified from plasmid pGEM-HBscFv, and then sub-cloned into expression vector pPICZalphaA. The resulting plasmid pPIC-HBscFv was linearized and transformed into P. pastoris GS115. The recombinant Pichia strains, identified by direct PCR and Zeocin-resistant screening of Pichia transformants, were cultured and induced with methanol. It was found that recombinant HBscFv, lead by alpha-factor, could be secreted into the culture supernatant to a level of 80mg/L. The bioactivity of Pichia produced HBscFv was confirmed by indirect ELISA, which also suggested that the bioactivity of HBscFv in the culture supernatant reached its peak in 72h and decreased in the late-stage of the induction. PAS staining suggests that HBscFv produced by yeast is poorly glycosylated or none-glycosylated protein.

8.Layout design of operating area in Model 920 hospital ship

Objective To research the location,composition,zoning,flow and combination form of medical system of Model 920 hospital ship, and to provide theoretical support for the design of the hospital ship. Methods The layout of medical system of the hospital ship was constructed based on the theories of ship engineering design,hospital architecture design and naval health service as well as the requirements for height and internal environment of medical system.Results The mode combining multi corridor and single column was used to design 8 operating rooms and accessories in the midship of No.01 deck.Conclusion The multi-corridor single-column combination operating area occupies less ship space resources and the surgical treatment of wounded and sick patients is efficient, which is suitable for the platform of the ship and is worthy of reference for the design of the medical system of the large-scale rescue platform on the sea. 9.The application of extracorporeal membrane oxygenation for surgical treatment of coronary artery disease.

OBJECTIVETo summarize the results and experiences on extracorporeal membrane oxygenation (ECMO) for post-cardiac surgery of coronary artery disease.
METHODSFrom June 2004 to November 2006, sixteen patients with the mean age of (58 +/- 11) years old undergoing cardiac surgical procedures were placed on ECMO using a heparin-bonded circuit. Fourteen patients were male and two patients were female. Thirteen patients underwent on pump coronary artery bypass surgery (CABG) and three patients underwent off-pump coronary artery bypass grafting. The duration of ECMO support, stay of intensive care unit (ICU stay), complications and turnovers were recorded.
RESULTSThe mean duration of ECMO support was 51 hours, and the mean duration of ICU stay was 5 days. Thirteen patients (81.3%) were successfully weaned form ECMO, ten patients (62.5%) were discharged from hospital. The main complications were bleeding, infection, renal failure and ischemia of the lower limbs with the incidence of 18.8%, 37.5%, 25% and 18.8% respectively.
CONCLUSIONECMO is an acceptable technique for shortterm treatment of refractory low cardiac output after cardiac surgery of coronary artery disease.

10.A cohort study on the relationship between cerebrovascular hemodynamic changing and risk of stroke.

OBJECTIVETo study the role of cerebrovascular hemodynamic indexes (CVHI) changing in stroke and to provide reference for stroke prevention and risk factor study.
METHODSFrom 2003 to 2004, participants aged 40 years and above in two communities in Fengxian district were recruited by cluster sampling. Risk factors of stroke and CVHI were investigated and checked during baseline investigation. A total of 10 565 individuals completed the survey and met the inclusion criterion. After baseline investigation, the cohort was followed up for stroke occurrence. Relative risk (RR) of CVHI and common risk factors were estimated by cohort study design.
RESULTSAge of the cohort was (56.2 ± 11.4) years. 4444 (42.1%) were males and 6121 (57.9%) were females. Total follow-up duration was 67 885.7 person-years. A total of 195 stroke cases occurred and incidence density of stroke was 287.2 per 100 000 person-years. Stroke incidence in exposure groups of hypertension, heart disease and alcohol drinking was 3.47% (108/3118), 2.96% (21/710) and 2.50% (47/1882), respectively. The incidence in corresponding non-exposure group was 1.17% (87/7448), 1.77% (174/9855) and 1.70% (148/8683) respectively. There was significant difference between 2 groups (χ(2) value was 62.72, 4.56 and 4.94, respectively, P < 0.05). Stroke incidence in CVHI score < 25, 25 - 49, 50 - 74 and ≥ 75 groups was 9.12% (59/647), 5.68% (44/775), 2.52% (39/1545) and 0.72% (53/7403)(χ(2)trend = 273.57, P < 0.05), respectively. Incidence of stroke in 40 - 49, 50 - 59, 60 - 69, ≥ 70 years age group was 0.22% (8/3565), 1.28% (43/3357), 2.71% (50/1848) and 5.88% (94/1600) (χ(2)trend = 181.48, P < 0.05), respectively. Multiple Cox regression analysis indicated that RR (95%CI) value of hypertension and cigarette smoking was 1.40(1.02 - 1.92) and 1.59(1.19 - 2.12), respectively when comparing with non-exposure group. RR (95%CI) value in CVHI score < 25, 25 - 49 and 50 - 74 points group were 6.15 (4.08 - 9.26), 4.55 (2.98 - 6.96) and 2.68 (1.75 - 4.09), respectively when comparing with the score ≥ 75 points group. RR (95%CI) value in age 50 - 59, 60 - 69 and ≥ 70 years group was 4.61 (2.16 - 9.82), 7.81 (3.67 - 16.60) and 13.49(6.44 - 28.24), respectively when comparing with below 40 years group.
CONCLUSIONCVHI score is the strong independent predictive factor and hypertension, cigarette smoking and age are the independent risk factors of stroke.
