1.Strain and Suicide:an Analysis of 155 Suicide Cases

Objective:To explore the strain experiences by suicides and to test the strain theory of suicide.Methods:A consecutive sample of 155 suicides(91 males and 64 females)aged from 15 to 34 years was studied with the psychological autopsy method.Informants consisted of family members and close friends of the suicides.Suicide stories were composed based on the background information of the suicides and what happened at the suicide site and then content-analyzed and rated by independent judges.Results:All of the victims had experienced at least one type of strain before suicide took place.The sequence of the 4 types of strains beginning with the most frequent one was coping strain,100%;aspiration strain,81%,deprivation strain,72% and value strain,45%.Generally,males experienced more coping strains,especially loss of money or investment than females [(8.85±3.64)vs.(7.72±3.77),(0.35±0.92)vs.(0.09±0.52);Ps<0.05],except for the traditionalism and modernity for status of women,religion and atheism [(0.08±0.37)vs.(0.53±0.97),(0.00±0.00)vs.(0.06±0.30);Ps<0.05].As age increased,value strain tended to increase(r= 0.16,P=0.048),whereas aspiration strain tended to decrease(r= -0.30,P<0.001).Conclusion:With the prevalence of dissonant stresses among suicide victims,it is suggested that crisis intervention and suicide prevention should begin with strain reduction through psychological counseling and therapy.
2.Study on preparation of matrine double-sensitive colon-specific pellets and in vitro release.

OBJECTIVETo prepare matrine double-sensitive colon-specific pellets and study the factors affecting its quality and evaluateing the colon-specific effects of preparation.
METHODMatrine enzyme-sensitive pellets core were prepared by carboxymethyl konjac glucomannan as the main carrier material, and coated the core by acrylic resin II and III to prepare matrine double-sensitive colon-specific pellets. The prescription and technology of the matrine colon-specific pellets were studied by the single factor investigation, through the in vitro release test and coating rate determination.
RESULTThe optimized process conditions: FeCl3 concentration is 4.0 g x L(-1), chitosan concentration is 3.0 g x L(-1), carboxymethyl konjac glucomannan concentration is 20 g x L(-1), mixed gel solution pH value is 3. The release of matrine is less than 30% in the simulation of the upper gastrointestinal medium. The release of matrine is close to 100% in simulated full gastrointestinal medium, the coating weight is 7%.
CONCLUSIONThe prepared pellets have good colon positioning effect in vitro.

3.Effect of single lung ventilation for pneumoresection on blood gas

Purpose:To study the effect of single lung ventilation for pneumoresection on blood gas. Methods:20 patients with lung cancer whose lung function were normal or reduced slightly before the operation were divided into 2 groups (A and B),of 10 cases each.The patients in group A underwent single lung ventilation with an endobronchial tube during the operation ,and two lung ventilation with an endotracheal in group B.Blood gas analysis was measured in every patient before, during and after the operation.Results:The values of blood gas analysis during the operation in all patients were normal and no significant difference was found between the two groups during the same time. Conclusions:The patients with lung cancer whose lung function were normal or reduced slightly before the operation can receive single lung ventilation safely for pneumoresection.[

5.Comparison between ultrasound and nerve stimulator guidance for interscalene brachial plexus block

Objective To compare the clinical effect of ultrasound and nerve stimulator guidance for interscalene brachial plexus block. Methods Sixty patients scheduled for upper extremity surgery were randomly allocated into two groups to receive either nerve stimulator guidance (group N,30 cases) or ultrasound guidance (group U, 30 cases) for interscalene brachial plexus block. The time for completing the procedure, the onset time of sensory block and motor block, as well as success rate of the procedure were recorded. The requirement for fentanyl during surgery, pain related with procedure and incidence of other complications were also recorded. Results The success rate of the procedure was 100% in both groups.Longer time was needed for completing procedure in group U [(16±7)min] than that in group N [(11±5)min](P<0.05). The onset time of sensory block was significantly shorter in group U [(13±9)min] than that in group N [(18±7)min] (P < 0.05 ), while the onset time of motor block was similar between the two groups. Twelve patients in group N and 5 patients in group U complained pain related with procedure, which were significantly different (P<0.01 ). Conclusion Interscalene brachial plexus block with ultrasound guidance has similar clinical effect and success rate compare with nerve stimulator guidance.

7.Diclofenac sodium extended-release capsule for cancer pain in senile patients

Purpose:To study analgesia effect and adverse reaction of diclofenac sodium extended release capsule for cancer pain in senile patients. Methods:20 senile patients with light or moderate cancer pain were selected.The analgesia effect and adverse reaction were observed after diclofenac sodium extended release capsule 100 mg qn po.Results:The treatment of cancer pain with diclofenac sodium extended release capsule in senile patients had obvious analgesia effect without obvious adverse reaction. Conclusions:Diclofenac sodium extended release capsule 100 mg qn po is a safe,effective and simple way to relieve cancer pain in senile patients with light or moderate cancer pain.
8.Experimental study on H2-Ab1 gene expression in the nasal mucosa of mice with allergic rhinitis.

OBJECTIVE:
To investigate the level of H2-Ab1 in the nasal mucosa of mice with allergic rhinitis.
METHOD:
Twenty-four female 129/sv mice were divided into 2 groups: ovalbumin (OVA) group and control. The allergic rhinitis models were induced by classical method with OVA. After the last challenge, the pathological differences between the two groups were surveyed. The levels of H2-AB1 were measured by ELISA and quantitative real time PCR.
RESULT:
The expression of H2-Ab1 is higher in subjects with AR than that in controls (P < 0.05).
CONCLUSION
The levels of H2-Ab1 were highly increased in allergic rhinitis group, which might be associated with the pathogenesis of allergic rhinitis.

9.Dual-drug sustained-release carrier:Preparation and performance

BACKGROUND: During conventional treatment for bone tuberculosis, there is a low effective concentration of anti-tuberculosis drugs, and the therapeutic effect is poor. OBJECTIVE:To develop a new biomaterial as a slow-release artificial carrier that can be directly implanted into the surrounding tissue of bone tuberculosis, maintain a certain anti-tuberculosis drug concentration for a long time, thereby playing an effective therapeutic action. METHODS:Rifampicin/polylactic acid/glycolic acid microspheres and isoniazid/polylactic acid/glycolic acid microspheres were prepared using the emulsion-solvent evaporation method. Usingα-cyanoacrylate, a biological adhesive, two kinds of microspheres were processed into a long-term slow-release bicomponent drug carrier. Then, in vitro release characteristics of the dual-drug sustained-release carrier were observed. After that, the dual-drug sustained-release carrier was implanted into rabbit intertrochanteric femur bone defects for observing drug release concentrations, histocompatibility and bone defect healing at different time points after drug delivery carrier implantation. RESULTS AND CONCLUSION:For rifampicin/polylactic acid/glycolic acid microspheres, the mean particle size was (240±13)μm, and the drug loading load rate was (26±1.5)%. For isoniazid/polylactic acid/glycolic acid microspheres, the mean particle size was (250±10)μm, and drug loading rate was (28±1.8)%. The in vitro cumulative release rate could reach 80%for rifampicin and 90%for isoniazid at day 90. The in vivo released concentration of rifampicin and isoniazid within 90 days was (0.5±0.4) and (0.6±0.3)μg/g, respectively. There were a smal amount of infiltrated neutrophils between the fascia and muscle fibers after the drug delivery carrier was implanted, and the amount of neutrophils in the muscle were reduced significantly at day 59. X-ray plain film showed that bone defects decreased obviously in size. These findings indicate that this dual-drug sustained-release carrier can maintain a certain anti-tuberculosis drug concentration in the surrounding tissues of bone tuberculosis, which is expected to provide a new type of dual-drug delivery carrier in the surgical treatment of bone tuberculosis.
10.Clinical features and management of combined thoracoabdominal injury

Objective To review the clinical features and early management of combined thorcoabdominal injury(CTI),to decrease the mortality rate.Methods A retrospective study was carried out in 63 cases of CTI treated surgically from May 1990 to May 2007.There were 34 cases with blunt injuries and 29 cases with penetrating injuries.Forty-seven patients had rapture of left diaphragm,9 cases had rupthure of right diaphragm,7 cases had rupture of both diaphragms,24 cases were combined with traumatic diaphragmatic hernia,63 cases were combined with hemopneumothorax,and 60 cases were combined with shock.Forty-one cases were diagnosed correctly before surgery.Sixty-three cases were treated by surgery,among them,surgery was performed through left thoracotomy in 18 cases,through a right thoracotomy in 2 cases,through bilateral thoracotomy in 5 cases,through thoraco-abdominal incision in 2 cases,and through the abdominal route in 32 cases.Results Fifty-six cases were cured,and 7 cases(11.1 %) died.The main cause of death was hemorrhagic shock.Conclusions CTI has high mortality rate,and the most frequent cause of mortality is hemorrhagic shock.The operative approach is based on the individual injury condition.Early diagnosis and timely surgical intervention is the key to improving the cure rate and reducing mortality in severe CTI.
