AIM: To observe the effects of valsartan on the cardiac hypertrophy and the expression of PTEN in spontaneously hypertensive rats (SHR). METHODS: Twenty 12-week-old male SHR were randomly divided into 2 groups:SHR positive control group and valsartan treating group ( 30 mg?kg -1?d -1). 10 homogenous Wistar-kyoto (WKY) rats were served as normal control group. Blood pressure and the ratio of left ventricular weight to body weight(LVW/BW)of rats were monitored periodically during the 8-weeks studies. Expression of PTEN in cardiac myocytes was determined by immunohistochemistry. RESULTS: Blood pressure and LVW/BW increased in the valsartan group more than those in the SHR control group, and the expression of PTEN in cardiac myocytes in valsartan group increased more than that in the SHR control group, but the indexes were lower than those in the WKY group. CONCLUSION: Valsartan can not only inhibit the progression of cardiac hypertrophy in spontaneously hypertensive rats, but also increase the expression of PTEN. PTEN may play a role in cardiac hypertrophy.

Acute paraplegia is a rare presentation for a spinal cord ependymoma. Subarachnoid hemorrhage
(SAH) due to spinal ependymoma is also very rare. We report a 32-year-old woman who presented
with acute paraplegia and typical clinical signs of SAH with normal cerebral angiography, and further
diagnostic work-up revealed an spinal cord ependymoma as the source of the hemorrhage. There
is evidence that some spinal cord ependymomas have intratumoral hemorrhage, but most of these
bleedings occur without symptoms. We discuss the clinical and neuroradiological findings of this rare
case and review the literature related to this unusual presentation.431

This study aims to explore and analyze the condition of concurrent diseases and medicine use of traditional Chinese medicine (TCM) and western medicine among the patients with insomnia. One thousand and sxity seven cases of data from 20 national hospitals' hospital information system (HIS) databases were collected. The frequent concurrent diseases included hypertension (26.9%), brain blood supply insufficiency (24.93%), cerebral infarction (19.49%), blood lipoprotein disturbance (15.28%), coronary heart disease (14.15%), headache (10.68%), chronic gastritis (8.81%), type 2 diabetes mellitus (7.87%), depressive disorder (7.4%) and anxiety disorder (6.65%). The 10 most frequently-used western drugs included alprazolam (35.99%), aspirin (25.4%), olanzapine (24.18%), cinepazide (23.06%), flupentixol & melitracen (18.74%), zolpidem (18.37%), oxiracetam (15.65%), estazolam (15%), aniracetam (13.4%) and piracetam (13.31%). The 10 most frequently-used TCM included Shuxuening injection (16.4%), Shuxuetong injection (15.18%), extract of ginkgo biloba leaf (14.71%), gastrodin (12.46%), Dengzanxixin injection (11.34%), Xueshuantong (8.53%), Danhong injection (6.37%), compound liquorice tablet (5.81%), Sanqi Tongshu capsule (5.72%) and sowthistle-leaf ixeridium injection (5.34%). Among all combined uses, the most frequent western drug use was alprazolam and olanzapine, while combined use of hypnotic drug and Huoxuehuayu formula is the most frequent. This study concludes that the concurrent diseases mainly include cardio-cerebrovascular diseases, metabolic disorders and anxiety-depression disorders, with increasing tendency of diseases types by ages, especially for cardio-cerebrovascular diseases. The most frequently-used hypnotic is alprazolam in the insomnia patients, and it is worth being concerned about the off-label use of olanzapine as an antipsychotic for the treatment of insomnia However, due to the fact that all cases data are from the inpatients, these findings have some limitations.

OBJECTIVETo observe the clinical effect of anatomic shape bracket splints designed by 3D-MAX software.
METHODSHealthy adult volunteers' forearms and wrist data were measured and processed by 3D-MAX software. Then we designed and made anatomic shape bracket splints. Totally 60 distal radial fracture patients were equally assigned to the test group and the control group. After manual reduction, patients in the test group used anatomic shape bracket splints, while those in the control group used common splints. The palmar dip angle, ulnar fleet angle, and radius height of standard X-ray in normal and lateral positions were measured after treatment, at 1, 3, and 6 week after treatment, respectively. Meanwhile, the incidence of complications was recorded during the whole treatment process.
RESULTSCompared with the control group, the palmar dip angle and ulnar fleet angle were larger in the test group after one week of treatment (P < 0.05). The radius height of the treatment group was higher after one, three, and six weeks of treatment (P < 0.05). Compared with the control group, the incidence of complications was obviously less in the test group (P < 0.05).
CONCLUSIONSAnatomic shape bracket splints designed by 3D-MAX software fit local anatomic features and need no more shaping. Its easier use with lesser complications could maintain the stability of bone fracture better.

OBJECTIVETo evaluate biomechanical properties in different methods of internal fixation combined with distal tibiofibular syndesmosis injury, in order to provide a theoretical basis for clinical choice.
METHODSSix lower limbs specimens were collected and divided into 5 groups, including normal group, distal tibiofibular syndesmosis injury (injury group), 3 cortexes group, 4 cortexes group and hook-plate fixation group. Neutral position, plantar flexion position (30°), dorsiflexion (20°) supination external rotation position of foot movement were simulated on universal materials tester. Strength, stiffness and stability of ankle joint in 4 kinds of motion conditions were measured.
RESULTSThere was significant differences in strength and stiffness of ankle joint between injury group and normal group in 4 different kinds of motion conditions (P<0.05). Strength and stiffness of ankle joint in 3 cortexes group, 4 cortexes group and hook-plate fixation group were improved obviously in 4 different kinds of motion conditions, and biomechanical indexes were recovered normally or better than normal group. Stiffness of the three fixation groups were better than normal group, but there was no significant differences among three groups (P > 0.05), while stiffness of hook-plate fixation group was closed to normal group.
CONCLUSIONFor distal tibiofibular syndesmosis injury, 1 screw with 3 cortexes, 4 cortexes and hook-plate had a positive impact on strength, stiffness and stability of distal tibiofibular syndesmosis injury stress, and could restore the normal stabllity of ankle joint after reconstruction. While fixing by screw fixation would limit the rototary motion of ankle joint, ankle mortise could not adapt to changes of talus bone, thus induces screw breakage and traumatic arthritis. Hook-plate fixation is more suitable than 3 cortexes or 4 cortexes fixation for bilmechanical properties, and its' stress is more balance and can reduce postoperative complcations.

Objective To observe the expression of Bcl-2,Bax and proliferating cell nuclear antigen(PCNA) in liver of rats with hepatic fibrosis and the effects of transforming growth factor (TGF)-?1 vaccine on them. Methods Thirty healthy male Sprague-Dawley rats were assigned into 3 groups, named healthy control group(n=10),hepatic fibrosis group(n=10) and TGF-?1 vaccine treated group(n=10).The animal model with hepatic fibrosis was established by injecting solution dimethylnitrosamine (DMN) into abdominal cavity with concentration as 0.5% and dose as 0.2 mL/ 100 g.In TGF-?1 vaccine treated group, every rat was not only injected with DMN but also 150?g TGF-?1 vaccine protein. On the 42nd day,all rats were sacrificed. Then the blood and the liver tis- sues were collected. The expression levels of Bcl-2,Bax and PCNA in liver tissues were detected by S -P immunohistochemistry and observed by routine pathological evaluation.Alanine aminotransferase (ALT),aspartate aminotransferase(AST) and albumin(Alb) were determined by auto biochemical analytical tool.Serum levels of hyaluronic acid (HA),laminin(LN) were detected by radioimmunoas- say (RIA).Results The expression of Bax,which promoted apoptosis, directly correlated with pathological grade in liver of rats, while the expression of Bcl-2 and Bcl-2/Bax,which protected a gainst apoptosis, inversely correlated with pathological grade in liver of rats. The expression levels of TGF-?1 and Bax in healthy control group were significantly lower than those of fibrosis group, how ever, the expression levels of Bcl-2 were comparable between these two groups. As compared with fi- brosis group, the expression of TGF-?1 was significantly lower while the expression of Bcl-2 was sig nificantly higher in TGF-?1 vaccine treated group. However, the expression of Bax was comparable between these two groups. The expression level of PCNA of fibrosis group was significantly higher than that of healthy control group but dramatically lower than that of TGF-?1 vaccine treated group (Both P
