1.Clinical observation on eyelid herpes zoster with external use ganciclovir ophthalmic gel

AlM: To observe the treatment efficacy of eyelid herpeszoster with external use ganciclovir ophthalmic gel.METHODS:A total of 77 patients were randomly divided into treatment group and control group. ln the treatment group, ganciclovir ophthalmic gel was topical coated on the basis of antiviral drugs, while the control group was coated with conventional acyclovir ointment and antiviral treatment.
RESULTS: The cure rate was 95% in the treatment group, and the control group was 83% after 10d treatment. The difference was significant.
CONCLUSlON: The efficacy of ganciclovir ophthalmic gel treated eyelid herpes zoster is obvious.
2.The Value of Preserving Intercostobrachial Nerve during Modified Radical Mastectomy for Breast Neoplasm under Transverse Incision

Objective: To discuss the value of preserving ICBN during modified radical mastectomy for breast neoplasms. Methods: Sixty-three cases of breast carcinoma who underwent modified radical mastecto-my were studied. Of them, ICBN was completely preserved in 33 cases and was dissected in 30 cases. Sen-sory function of the medial upper arm of all patients was followed up after surgery. Results: No local recur-rence or metastasis was found among the 63 patients during the first, sixth, and twelveth month of follow-up period. Among the 33 cases with ICBN reserved, 29 cases (87.88%) had normal sensation of the skin in up-per medial arm and axilla, while 4 cases (12.12%) had paresthesia. Conclsion: Preserving ICBN can en-hance the quality of life of breast cancer patients after surgery.
3.Significance of lateralized focus of intra-carotid arterial diazepam injection for intractable epilepsy with bilateral synchronous epileptiform discharges

Objective To explore the significance of intra carotid arterial diazepam injection in lateralization of intractable epilepsy with bilateral synchronous epileptiform discharges. Methods Intra carotid arterial diazepam injection test was performed in 20 patients suffering from intractable epilepsy with bilateral synchronous epileptiform discharges under EEG monitoring. Side of the epileptogenic focus was determined by observation of the inhibition of epileptiform discharges after injection. Results After diazepam injection, disappearance of bilateral synchronous epileptiform discharges was found in 16 patients (80%), so the injection side was decided as the epilepogenic focus. Disappearance of the epileptiform discharges was found at the injection side, but disappearance of the bilateral epileptiform discharges was found after injection at the opposite side in 4 cases (20%). The accuracy of lateralization was proved by surgery, electrocorticography (EcoG), and depth electroencephalography (DEEG) in 12 out of the 20 cases. During the follow up for 1-6 5 years after surgery and gamma knife treatment, disappearance of clinical seizure and epileptiform discharges of EEG and improvement were found in 15 cases (75%) and 5 cases (25%), respectively. Conclusion Intra carotid arterial diazepam injection is of important clinical value in lateralization of the epileptogenic focus before surgery for patients with intractable epilepsy.
5.Comparison of equivalent dose between intensity modulated radiotherapy and conventional radiother-apy for patients with nasopharyngeal carcinoma

Objective To estimate and compare the equivalent dose between the intensity modula-ted radiotherapy (IMRT) and conventional radiotherapy (CR) for patients with nasopharyngeal carcinoma (NPC). Methods The peripheral equivalent dose of NPC patients was measured by TLD during IMRT and CR. The whole-body equivalent dose was calculated. Results When prescribed tumor dose of 70 Gy was given, the mean number of accelerator output units were 25235 MU and 8575 MU, and the whole-body e-quivalent dose were 73.65 mSy and 15.28 mSy for IMRT and CR, respectively. Conclusion The equiva-lent dose in IMRT is 4.8 times higher than that in CR for patients with nasopharyngeal carcinoma.
6.Risk factors of postoperative acute lung injury of liver transplantation

Objective To study the risk factor of postoperative acute lung injury(ALI)after liver transplantation.Methods The clinical data of I00 patients with end-stage liver diseases who re- ceived liver transplantations were retrospectively reviewed.The risk factors of postoperative ALI after liver transplantation were analyzed by using single variance analysis and multiple variance regression analysis.Results Thirteen patients(13 %,13/11t0)altogether were diagnosed as ALI after liver transplantation.Binary logistic analysis revealed that massive transfusion during operation(more than 5000 ml)and severity of reperfusion injury(ALT above 600 U/L)were two independent risk factors of postoperative ALI following liver transplantation.Massive transfusion significantly increased the risk of ALI by 12.7 times,whereas the severe reperfusion significantly increased the risk of ALI by 7.0 times.Conclusions ALl is a serious multifactoral complication after liver transplantation with high mortality and fatality.Massive transfusion and the severe reperfusion injury are two independent risk factors with high morbidity and mortality.
7.Study on membrane extraction method for alkaloids of Ephedra Herba and Rhizoma Coptidis

Objective: Extracting ephedrine and berberine from the water extract of Ephedra Herba and Rhizoma Coptidis. Methods: Modified PVA was used as separating membrane and separated alkaloids. Results: A colorless ephedrine aqueous solution was obtained when extraction time was less than 8 hours; while a three constitution solution was obtained when time of extraction is over 8 hours. So is Rhizoma Coptidis. The membrane is hardly fouled. Conclusion: Purity of alkaloids from Herba Ephedra and Rhizoma Coptidis by membrane separation is Superior to that by alcoholic sediment but amount of alkaloids is reverse.
8.Chromatography-efficacy relation study between HPLC fingerprints and allelopathic effect of Salvia miltiorrhiza aqueous solution on radish.

To explore the effective components represented by fingerprint contributed to allelopathic effect of different Salvia miltiorrhiza aqueous concentration on seeds and seedlings of radish, grey relational analysis was used to establish the chromatography-efficacy relation. The results show that 15 peaks devote high allelopathic contribution to radish seeds and seedlings. The study will provide a new concept for allelochemicals screening and study.

10.Protective effects of different modes of ventilation on lungs on operated side during one-lung ventilation in patients undergoing thoracic surgery

Objective To evaluate the protective effects of different modes of ventilation on the lungs on the operated side during one-lung ventilation (OLV ) in patients undergoing thoracic surgery .Methods Forty-five ASA physical status Ⅰ or Ⅱ patients of both sexes ,aged 45-64 yr ,weighing 65-80 kg ,were randomly divided into 3 groups (n=15 each) using a random number table :group A ,group B and group C .After induction of anesthesia ,the patients were intubated with double-lumen tube and OLV was performed .During OLV ,the lung on the operated side was collapsed naturally in group A ,positive pressure ventilation (FGF 2 L/min) was applied in the lung on the operated side in group B ,and high-frequency jet ventilation (frequency 100 beats/min ,driving pressure 0.5 kg/cm2 ) was used in the lung on the operated side in group C .Immediately after intubation (T0 ) , and at 1.5 h (T1 ) and 2 h (T2 ) of OLV ,blood samples were taken from the central vein and radial artery for determination of the serum interleukin-6 (IL-6 ) and IL-8 concentrations .The net release of IL-6 and IL-8 was calculated .Blood samples were taken from the radial artery at T0-2 for blood gas analysis and for determination of surfactant protein A (SP-A) concentration in the serum .Respiratory index (RI) was calculated .The non-cancer tissues 1.0 cm × 1.0 cm × 1.0 cm which were extracted from the lung cancer specimens were used for microscopic examination of the pathological changes of lungs which were scored .Results Compared with group A ,the net release of IL-6 and IL-8 ,serum SP-A concentration ,RI and pathological scores were significantly decreased at T1 ,2 in B and C groups ( P<0.05) .Compared with group B ,the serum SP-A concentration and RI were significantly decreased at T1 ,2 , and the net release of IL-6 was increased at T2 in group C ( P< 0.05 ) .Conclusion Continuous positive ventilation and high-frequency jet ventilation both can effectively protect the lungs on the operated side during OLV in patients undergoing thoracic surgery ,and the efficacy of high-frequency jet ventilation is better .
