Objective To explore the feasibility of minimally invasive video-assisted thyroidectomy (M IVAT). Methods Clinical data of 8 consecutive patients with ben ign thyroid nodules treated by MIVAT in this hospital from September 2002 to Sep tember 2004 were retrospectively reviewed. Results All the pro cedures were successfully completed. The operative time was 90~150 min (mean, 12 0 min), and the hemorrhage volume, 10~30 ml (mean, 20 ml). No complications happ ened and no conversions to open surgery were needed. All the patients were compl etely recovered and discharged from hospital 3~4 days after the surgery. Follow- up checkups for 6~12 months (mean, 9 months) in the 8 patients found normally he aled wounds and no paraesthesia. Conclusions MIVAT is safe and effective, with excellent cosmetic outcomes.

Diabetic retinopathy ( DR ) is one of the main blinding eye diseases for people over the age of 50, and diabetic macular edema ( DME) is the leading cause of vision loss is DR patients. The early diagnosis and early treatment is important. As OCT and FFA, mfERG, especially the retinal thickness, volume, retinal edema index quantitative indicators such as objective evaluation of macular edema, embodies the new progress of retinal imaging technology in recent years. OCT is a non -contact clinical application in recent years, noninvasive, high resolution of ophthalmic imaging examination, can do it on retinal ultrastructure observation and quantitative analysis, and the technology is relatively mature, become a routine inspection diagnosis of macular edema. Laser photocoagulation, intravitreous injection with Ranibizumab and vitrectomy is nowadays the important means for the treatment of intractable macular edema.

The effects of magnolol (Mag) on hyperglycemia and hyperlipemia, hepatic oxidative stress and cytochrome P4502E1 (CYP2E1) activity of diabetic rats induced by high-fat diet (HFD) and streptozotocin (STZ) were studied. After oral administration of Mag (25, 50 and 100 mg x kg(-1) x d(-1)) for continuous 10 weeks, the blood glucose and lipids (TC, TG and LDL-C) levels, as well as the hepatic CYP2E1 activity and MDA content of diabetic rats, decreased significantly (P < 0.05 or P < 0.01), whereas the oral glucose tolerance and hepatic antioxidant enzymatic activities (CAT and GSH-Px) of diabetic rats, increased significantly (P < 0.05 or P < 0.01). The results indicated that Mag was effective against the hepatic oxidative damage, hyperglycemia and hyperlipemia of diabetic rats induced by HFD and STZ, and the inhibition of Mag on hepatic CYP2E1 activity could be an important mechanism of Mag against hepatic insulin resistance and oxidative damage.

Objective To investigate the effects of neoadjuvant chemotherapy on the hepatic function and histology of liver in patients with hepatic metastasis of colorectal cancer.Methods The clinical data of 65 patients with hepatic metastasis of colorectal cancer who received hepatectomy at the First People's Hospital of Foshan from January 2010 to September 2012 were retrospectively analyzed.Thirty-nine patients were in the experimental group,and they received partial hepatectomy 1 month after receiving a 6-month continous FOLFOX7 chemotherapy (5-fluorouracil + calcium leucovorin + oxaloplatin) ; 39 patients were in the control group,and they received partial hepatectomy without adjuvant chemotherapy.The perioperative condition,pre-and postoperative hepatic function,postoperative complications and the histopathological changes of the hepatic tissues of the 2 groups were compared by t test or chi-square test,data repeated measured were analyzed using the repeated measure analysis of variance.Results The operation time of the experimental group and the control group were (195 ± 37)minutes and (190 ±41) minutes,respectively,with no significant difference (t =0.1,P＞0.05).The operative blood loss of the experimental group was (410 ± 75)ml,which was significantly greater than (348 ± 44) ml of the control group (t =6.3,P ＜ 0.05).The levels of aspartate aminotransferase (AST) and alanine aminotransferase (ALT) at postoperative day 3 were (328 ± 121)U/L and (330 ± 120)U/L in the experimental group,which were significantly higher than (160 ±22) U/L and (168 ±26) U/L of the control group (t =13.4,12.8,P ＜ 0.05).There were no significant differences in the levels of prothrombin time,AST,ALT,total bilirubin and albumin between the experimental group and the control group (t =1.0,0.0,1.4,1.3,0.4,P ＞ 0.05).The levels of AST and ALT at postoperative day 7 were (243 ± 132) U/L and (253 ± 147)U/L in the experimental group,which were significantly higher than (90 ± 17)U/L and (99 ± 16)U/L of the control group (t=12.5,12.0,P＜0.05).The incidence of congestion and edema of the liver was 71.8％ (28/39) in the experimental group,which was significantly higher than 0 (0/26) of the control group (x2 =90.0,P ＜ 0.05).The incidence of hepatic sinusoid expansion with liver cellular altrophy and necrosis was 48.7％ (19/39),which was significantly higher than 15.4％ (4/26) of the control group (x2=89.2,P ＜ 0.05).Conclusion There is no effect of long-time FOLFOX7 chemotherapy on the hepatic function of the patients with hepatic metastasis of colorectal cancer who received hepatectomy 1 month later,but the postoperative hepatic function is affected.The hepatic parenchyma is changed after adjuvant chemotherapy,which might have adverse effect on hepatectomy.

Purpose:To evaluate the effect of oxaliplation (L-OHP)+flurouracil/calcium folinate(5-FU/CF) in combination with concurrent radiotherapy for advanced rectal cancer and to find a more effective way of giving them.Meth- ods:Chemotherapy:L-OHP 130 mg/m~2,iv infusion for 2 hours D_1 combined with calcium folinate 200 mg/m~2 for 2 h ours followed by 5-fluorouracil 300 mg/m~2 for 4h D_1～D_5,repeated every 3 weeks.Radiotherapy:The whole pelvic irradiation DT 50～60Gy/25～30/F(5～6 weeks).Results:30 advanced rectal cancer patients entered the study.One patient reached complete response(CR),15 partial response,13 stable disease,and 1 with progression after treatment,The overall re- sponse rate(CR+PR) was 53.3%,the 1-year survival rate was 70% (21/30).The main adverse acute effects were gas- trointestinitis,anemia,neuro-sensory toxicity and irradiation rectitis,bone marrow suppression was mild.Conclusions: This approach of therapy could improve the therapeutic effect on advanced rectal cancer.

ObjectiveTo investigate the effects of sevoflurane on short-term memory impairment and the relative synaptic mechanism.MethodsAt 12 h,24 h and 72 h after the mice were exposed to 1.5％ sevoflurane for 1h,the spontaneous alternation and locomotor activity was assessed by Y maze,the short term potentiation (STP) were measured with extracellular recording technique in hippocampal slices.ResultsAt 12h after administration with sevoflurane in vivo,the spontaneous alternation ( (54.7 ± 1.7) ％,P＜0.01 ) and locomotor activity (16.4 ± 1.3,P ＜ 0.01 ) decreased significantly compared with that of control group,and the population spike amplitude after induction of STP ( ( 122.3 ± 13.9) ％,P ＜ 0.05 ) decreased significantly in hippocampal slices,but there was no different at 24h and 72h.After administration with sevoflurane in vitro,the basic ( ( 83.5 ± 8.9) ％,P ＜ 0.01 ) or titanic ( ( 116.5 ± 14.9) ％,P ＜ 0.01 ) population spike amplitude decreased significantly in hippocampal slices,but the amplitude could recovery after wash-out.ConclusionSevoflurane can impair the shortterm memory by suppressing synaptic transmission in the near future but not the long future.

Objective　To investigate the experience of diagnonsis and treatment of post-orthotopic liver transplantation (OLT) complications. Methods　The clinical data of diagnosis and treatment of post-OLT complications in 7 cases were analysed retropectively. Results　Complications following OLT including intracranial hemorrhage (1/7), renal failure (1/7), intrabdominal hemorrhage (2/7), pulmonary infection and/or, pleurorrhea (5/7), adult respiratory distress syndrome (1/7), billirubinemia (5/7). Five patients survived while two died. Conclusions　Proper prevention and management can effectively reduce post-OLT complications, Timely diagnosis and suitable therapy would improve the result of liver transplantation.

Objective To investigate the enhancement characteristics of the mass of liver and pancreas on Mn-DPDP enhanced MRI. Methods Mn-DPDP enhanced MRI were carried out in 25 cases, of which 15 cases had examined by Gd-DTPA-enhanced MRI. Conventional plain with SE T 1WI、FSE T 2WI were done in all patient. SE T 1WI and FSPGR T 1WI were performed during 40 minutes～4 hours after the administration of Mn-DPDP, with a dose of 0.5 ml/kg.Parts of the patients were repeat examined 24 hours after the administration of Mn-DPDP. Results Among 10 cases of hepatocelluler carcinoma, slightly heterogeneous enhancement were found in 8 cases.hemi-ring enhancement were found in 2 cases. 2 cases of liver cirrhosis had apparently nodular homogenous enhancement and their signal intensity were higher than normal liver tissue. No enhancement were found in 2 cases of hemangioma, 2 cases of liver metastases and 1 case of hepatic cyst. Also no enhancement were found in 8 cases of pancreatic carcinoma, but the tissue of pancreas around the tumor were enhanced remarkably. Conclusion The Mn- DPDP can show remarkably enhancement of normal liver and pancreas.The medicine's peak value keeps on time grows, can provide plentiful scanning time windows.

BACKGROUND:A variety of embryonic stem cells induction and differentiation systems have been established so far, while the research that promotes embryonic stem cells to differentiate into hematopoietic stem cells is stil at an initial stage, and the induction efficiency needs to be improved.
OBJECTIVE:To active the Wnt/β-catenin signal pathway in mouse embryonic stem cells with exogenous win3a as an inducer, and then to observe whether the activation of this pathway wil promote the directional differentiation of embryonic stem cells into hematopoietic progenitor cells.
METHODS:The ES-E14TG2a mouse stem cells were cultured with the exogenous wnt3a (100 μg/L) for 21 days, the content ofβ-catenin was tested by cellimmunofluorescence and western blot, and expression of Wnt downstream target gene was detected by quantitative reverse transcription PCR to determine the activation of Wnt/β-catenin signal pathway. Single-layer adherent culture method was used to induce the directional differentiate of above-mentioned cells into hematopoietic stem cells, and detection of hematopoietic development associated surface marker CD34+/Sca-1+was achieved by flow cytometry;meanwhile, the expression of hematopoietic associated gene was measured by quantitative reverse transcription PCR.
RESULTS AND CONCLUSION:We found thatβ-catenin accumulated in ES-E14TG2a mouse stem cells after cultured with wnt3a (100 μg/L) for 21 days;the expressions of Wnt downstream target genes such as Pitx2, Frizzled, Sox17 and Oct4 showed the different degrees in increase, meaning the activation of Wnt/β-catenin signal pathway. Furthermore, during the time that we used single-layer adherent culture method to induce hematopoietic stem cells, the CD34+/Sca-1+cells accounted for 20.2%of total cells at day 14, and control cells only accounted for 11.9%. Again, expression quantity of hematopoietic associated gene BMP4, FLK2 and CD34 increased while Smad5 was suppressed significantly. Our data suggest that sustaining action by wnt3a wil active Wnt/β-catenin signal pathway, and also promote the directional differentiation of ES-E14TG2a mouse stem cells into hematopoietic progenitor cells.
