1.Effects of filter reuse on solute clearance and safety in on-line hemodiafiltration

Objective To investigate the effects of filter reuse on solute clearances, safety and oxidative stress parameters in on-line hemodiafiltration(HDF) . Methods 8 stable maintained uremic patients were treated by post-dilution on-line HDF with first-use or reuse F60 polysulfone filter, respectively. Both blood-side and dialysate-side solute clearances at 20 minute of HDF or during the whole session were measured. Whole blood interleukin-1?(IL-1?) production was monitored before HDF, after HDF, and at 20 minutes of HDF at both venous and arterial lines. Plasma before and after treatment and dialysate were collected for measuring total ascorbic acid, dehydroascorbic acid and total vitamin E. Restults No difference of small molecular substance clearances was observed in blood and dialysate side, while filter reuse group had a significantly higher dialysate side clearance and a significantly lower absorption clearance for ?2-microglobulin. In contrast with stable vitamin E concentration during dialysis, total ascorbic acid level decreased after treatment, with reuse group further inducing a reduced ration of dehydroascorbic acid to total ascorbic acid. No significant changes were found in whole blood IL-1? production within and between each group, so did intra-dialysis symptoms and temperature curves. Reuse filter also caused more albumin loss through high-flux membrane than that of first-use filter. Conclusions Although reuse filter can maintain both small and large molecular weight substance clearance, it increases albumin loss through high-flux membrane. Reuse filter does not stimulate white blood cell to produce more cytokine than the first-use filter, but it increases oxidative stress, and may harm uremic patients in a long run.
2.Variation and significance of histamine receptors before and after the protection to the mucous membrane of urinary bladder of interstitial cystitis rat model

Objective To investigate the changes of histamine receptors in bladder before and after the treatment by sodium hyaluronate in rats of interstial cystitis (IC). Methods Twenty IC model rats were randomly divided into experimental group and control group, with 10 for each group.The bladders in experimental group were filled with sodium hyaluronate, while the rats in control group were executed at once. The bladders were dyed with HE staining, special staining and immunohistochemistry staining to count the number of mononuclear cells and mast cells and observe the changes of histamine receptors. Results In experimental group,the counts of mononuclear cells and mast cells were 12.20±2.48 and 2.90±0.87 respectively;the numbers of average optical of histamine receptor H1, H2, H3, H4 were 0.015±0.007, 0.006±0.001, 0.007±0.004, 0.061±0.026 , respectively. In control group, the counts of mononuclear cells and mast cells were 23.90±3.07 and 7.08±1.23;the numbers of average optical of histamine receptor H1, H2, H3, H4 were 0.055±0.033, 0.031±0.023, 0.033±0.017, 0.091±0.059, respectively. The differences of mononuclear cells and mast cells were significant between the 2 groups(P<0.01). The differences of average optical of histamine receptor H1, H2, H3 between the 2 group were significant (P<0.05), while the difference of histamine receptor H4 was not significant(P>0.05). Conclusion Histamine receptor H1, H2, H3 take part in the development of IC, the antagons may be used for the treament of IC.
3.Practicality and repeatability of B-mode ultrasonography on diagnosis and score of carotid atherosclerosis

Objective To study the practicality and repeatability of B-mode ultrasonography on diagnosis and score of carotid atherosclerosis. Methods Ninety-nine B-mode ultrasonography pictures of carotid atherosclerosis were obtained from field investigation. According to the established standard(normal scored as 0, other scored as 1 - 7 by the severity), the 99 pictures were diagnosed and scored once together by three trained inexperienced members of the research team. Ten days later, these pictures were diagnosed and scored respectively, by the three members.The diagnostic results were analyzed with statistical methods to test the feasibility and repeatability of this set of diagnostic and scoring system. Results The joint diagnostic results by the three group members were regard as standard with scores 0 in 27 cases, 1 in 27 cases, 2 in 10 cases, 3 in 4 cases, 4 in 13 cases, 5 in 8 cases, 6 in 6 cases, and 7 in 4 cases. Independent diagnostic results by member A was 0 in 29 cases, 1 in 24 cases, 2 in 11 cases, 3 in 2 cases, 4 in 12 cases, 5 in 11 cases, 6 in 6 cases, and 7 in 4 cases. Independent diagnostic results by member B was 0 in 29 cases, 1 in 22 cases, 2 in 14 cases, 3 in 4 cases, 4 in 13 cases, 5 in 5 cases, 6 in 8 cases, and 7 in 4 cases. Independent diagnostic results by member C was 0 in 28 cases, 1 in 25 cases, 2 in 8 cases, 3 in 5 cases, 4 in 14 cases, 5 in 10 cases, 6 in 4 cases, and 7 in 5 cases. Comparison of the 4 diagnostic scores, the difference was not statistically significant(F = 0.019, P > 0.05). Joint diagnostic results of the three members were compared with the standard and correlation coefficient were 0.977,0.987,0.932, respectively(all P < 0.01 ). The correlation coefficient of diagnostic results of each member were 0.969,0.935,0.928, respectively (all P <0.01 ). Diagnostic results of each member were consistent with the standard and the compliance rate were 88.9% (88/99) ,90.9%(90/99) and 86.9%(86/99), respectively. Conclusions B-mode ultrasonography is a non-injure method for diagnosis of carotid atherosclerosis in epidemiology investigation. The method is easy to grasp and has a good repeatability.
4.Combination of protamine sulfate and potassium chloride in establishing the interstitial cystitis animal model

5.Effect of soman intoxication under hypoxia on the expression of M_2R mRNA of rat myocardium

Methods The expressions of M 2 receptor mRNA under hypoxia, soman intoxication and soman intoxication under hypoxia were detected by semi quantitative RT PCR, respectively. Results The expression of M 2 receptor mRNA increased in the high altitude hypoxia group. Both simple soman intoxication and combined soman intoxication and hypoxia decreased the expression of M 2 receptor rapidly. But under hypoxia, the expression increased significantly at 12 h and 24 h. Conclusion M 2 receptor was sensitive to nerve agents. Compared to simple soman intoxication group, the expression of M 2 receptor increased in combined soman intoxication and hypoxia group. This may be one of the major factors leading to aggravation of the injury of heart function by nerve agents in high altitude area.
6.Changes of histamine receptors in bladder before and after the establishment of interstitial cystitis in animal model

Objective To investigate the changes of 4 histamine receptors (H1R, H2R, H2R and H4 R)in interstitial cystitis on animal experimental models. Methods Thirty female SD rats (250-300 g) were randomly divided into 2 groups as follows: 20 in experimental group and 10 in control group. The experimental group was filled with protamine sulfate+ potassium chloride to create interstitial cystitis model, the control group was sacrificed directly. At the end of the experiment, the bladders of all these SD rats were studied by the immunohistochemistry staining and the value of their mean absorbance (-A) was calculated by IPP4.5 image analysis software. The SPSS 11.5 was used to analyze the differences between the groups. Results Four kinds of histamine receptors mainly expressed in the bladder epithelium. In the experimental group, the (-A) of H1 R was 0. 054±0.031, the of H2R was 0.032±0.021, the (-A) of H3R was 0.047±0.033 and the (-A) of H4R was 0. 149±0. 191,respectively. In the control group, the A of H1R was 0. 017±0. 011, the (-A) of H2R was 0. 018±0. 015, the (-A) of H3R was 0. 014±0. 011, the (-A) of H4R was 0. 060±0.039, respectively. In contrast,the A of H1 R, H2 R and H3R in experimental group was increased significantly(P<0.05); there was no significant change in (-A) of H4R expression(P>0.05). Conclusions H1, H2 and H3 receptors in rat model interstitial cystitis bladder epithelium have increased and it indicates H1 R, H2R and H3R may be related to interstitial cystitis. H3 R may be a new treatment target of interstitial cystitis.
7.Effect of Foreign Plasmid DNA on Material and Energy Metabolism of Spleen in Mice

To study the influence of foreign plasmid DNA on spleen metabolism in immune-stimulated mice via the gastrointestinal tract. Mice were oral administered by pipette with 200?g of plasmid pcDNA3 and spleen were isolated at 4h and 18h after oral administration. Total RNA were extracted from spleen and spleen gene expression profile of Balb/c mice was analyzed by using Affymetrix oligonucleotide genechip after oral plasmid pcDNA3 administration. Functional cluster analysis was conducted by Genmapp and MAPPFinder software. By functional cluster analyzing the genes which were up-regulated more than twofolds, Genmapp results showed that plenty of metabolic pathway were induced in spleen after oral administration of plasmid pcDNA3. These metabolic process included purine metabolism, pyrimidine metabolism, protein synthesis, cholesterol synthesis, fatty acid synthesis, Glycolysis, TCA cycle and mitochondria oxidative phosphorylation pathway. The similar results also took place at 18h after oral administration. The result indicated that foreign plasmid DNA can modulate metabolism process in spleen of mice via the gastrointestinal tract, and may help understand the mechanism of action of foreign plasmid DNA uptaked via the gastrointestinal tract.
8.Imaging characteristics of intraparenchymal schwannoma and the related pathology

Objective To Analyze the imaging characteristics of intraparenchymal schwannoma and the related pathology,in order to improve the accuracy of diagnosis and be in favor of the clinics and the prognosis.Methods Four cases were confirmed to be intraparenchymal schwannoma by pathological and immunohistochemistry examination.One case was examined with precontrast and enhanced CT scanning,one with unenhanced MRI scanning,two with unenhanced and enhanced CT and MRI scanning.Their images were retrospectively analyzed.Results Of the four cases,three patients were less than 30 years old,with tumors located supratentorially.Cysts were found in all cases,with nodules on the wall in 3 cases.The nodules were enhanced markedly in two cases and moderately in one ease.In addition,calcification was detected in one case and prominent peritumoral edema existed in 1 case.The picture of the pathology demonstrated Antoni type A and Antoni type B.Immunostaining showed intense immunoreactivity for S-100 protein and Vim and negative immunoreactivity for GFAP and EMA.Conclusions Intraparenchymal schwannoma mostly occurred in juvenile,which located supratentorially in most cases.The presence of a cyst and peritumoral edema together with the tumor appears to be characteristic of intraparenchymal schwannoma.Calcification or the enhanced nodule is the helpful sign for the diagnosis.Combining the imaging findings with the pathology and immunohistochemistry results can gain the accurate diagnosis.
9.Diagnostic and Therapeutic Value of Single-balloon Enteroscopy in Suspected Small Intestinal Diseases

Background:Single-balloon enteroscopy(SBE)is a new method for the examination of small intestine,its clinical value in suspected small intestinal diseases need to be further studied. Aims:To evaluate the diagnostic and therapeutic value of SBE in suspected small intestinal diseases. Methods:A total of 73 suspected small intestinal diseases patients who had undergone 81 SBE examinations from July 2011 to October 2013 at Chengdu Military General Hospital were retrospectively examined,indications,diagnostic and therapeutic value of SBE in suspected small intestinal diseases were analyzed. Results:Of all the 81 examinations,33(40. 7% )were obscure gastrointestinal bleeding,29(35. 8% ) incomplete intestinal obstruction,and 19(23. 5% )chronic abdominal pain or diarrhea. The intubation depth was 230 cm for the oral approach,and 100 cm for the anal approach. The diagnostic yield of SBE was 67. 9% ,the main lesions were small intestinal ulcer,small intestinal inflammation,small intestinal tumor,small intestinal polyp. A total of 8 patients underwent endoscopic therapy,of whom 5 underwent endoscopic hemostatic therapy and 3 underwent resection of polyp. No serious complications were found. Conclusions:SBE is a safe and reliable diagnostic and therapeutic method for suspected small intestinal diseases,and its main indications are obscure gastrointestinal bleeding and incomplete intestinal obstruction.
10.Electro-acupuncture combined with surface gastric pacing in the treatment of gastrointestinal dysfunction after spinal cord injury

Objective To explore the effects of electro-acupuncture combined with surface gastric pacing on gastrointestinal dysfunction for patients with spinal cord injury (SCI).Methods Seventy-five SCI patients with gastrointestinal dysfunction were divided randomly into an electro-acupuncture group (group A),a gastric pacing treatment group (group B) and a group for which electro-acupuncture was combined with gastric pacing (group C).There were 25 cases in each group.Members of group A were given 30 minutes of electro-acupuncture per day ; those of group B had 30 minutes of gastric pacing treatment twice per day ; group C was provided with both 30 minutes of electro-acupuncture and one 30-minute gastric pacing treatment per day.All of the interventions lasted ten days.Before and after the treatment,all patients were assessed using a clinical symptoms score and with electrogastrograms,and any adverse effects were observed.Results There was a significant improvement in upper abdominal pain and epigastric discomfort,bloating and early satiety,nausea,vomiting,as well as belching and loss of appetite in each group after treatment.Before treatment there was no significant difference in the average clinical symptom scores of the three groups.After treatment the values had decreased,and those of group C were significantly lower than group A or B.The average scores of groups B and A were not significantly different.The electrogastrogram results showed than after treatment there was significant improvement in N% and T% in groups A and B,and also significant improvement in N%,B% and T% in group C compared to before treatment.Group C's improvements were all significantly greater than those observed in groups A and B.Conclusion Electro-acupuncture or gastric pacing alone can give some improvement in gastrointestinal function and its electrophysiological indicators,but their combined effects are better than when either is used alone.
