1.Application of Quality Metrics in Food Inspection Laboratory Management

Because of the particularity and the complexity of integrated management of food, government regulators must perform the whole process of quality control and trace to evaluate and judge the merits of food instead of only depending on product standards.Quality metrics is a new concept for the food industry, and can be regarded as the comprehensive evaluation and measurement for product quality and the standard for judging the quality of food products.Request for Quality Metrics Guidance for Industry was issued, which not only established a new model for the quality control of pharmaceutical industry, but also provided a new way of quality management for the inspection organization.By introducing the contents of Request for Quality Metrics Guidance for Industry issued by the US FDA, on the basis of summarization and thought, the quality management and evaluation of pharmaceutical industry was compared, and the feasibility and applicability of quality metrics management in inspection and testing field, especially in food testing field was evaluated.In this paper, the concept of quality metrics was extended to the level of food inspection laboratory management with hope to provide ideas for the future development of food quality management model.
2.Application experience of postoperative gastrointestinal decompression tube in 15 gastric cancer cases of basic-level hospitals

Objective To sum up the experiences of the application of gastric cancer postoperative gastroin-testinal decompression tube.Methods Retrospectively analyzed the implementation of postoperative gastrointestinal decompression tube undergoing elective surgery.The clinical data of 15 cases of gastric cancer,preoperative education communication,preoperative preparation,intraoperative and postoperative treatment were analyzed.Results 14 cases recovered,1 case was not completely correct anastomotic fistula by malnutrition.The average for the first time the anus exhaust time was (48.7 ±16.4)h,for the first time defecation time was (65.1 ±5.7)h,postoperative length of hospi-tal stay was (12.4 ±3.1)d,postoperative nausea and vomiting occurred in 2 cases,diarrhea in 2 cases,sore throat discomfort in 3 cases.Conclusion Postoperative gastric cancer patients with gastrointestinal decompression tube can reduce stress damage,improve postoperative experience,accelerate the postoperative rehabilitation,does not increase the incidence of postoperative complications,it is safe and feasible.
3.Radiographic and MRI features of the hemangiomas and vascular malformation in the deep soft-tissue of the extremities

Objective To explore radiographic and MRI features of the hemangiomas and vascular malformation in the deep soft-tissue of the extremities.Methods 89 cases confirmed by surgical pathology and angiography were analyzed retrospectively with clinical data and radiological findings.Imaging exams included plain X-ray performed in all patients,and MRI scan in 33 patients.Results On X-ray plain film,there was normal bone and soft tissue in 54 cases(60.7%),soft tissue abnormalities in 14 cases(1 5.7%),and phleboliths in 30 cases(33.7%).Also,plain X-ray film studies demonstrated bone changes adjacent to the deep soft tissue in 32 cases(36.0%), including periosteal reactions(13 cases),cortical erosion (1 6 cases),involvement of the bone marrow (10 cases)and 7 diffuse lesions with all above changes.On MRI,lesions were similar to honeycomb or sponge,and T1 WI showed isohypointense signal in 25 cases (75.8%),hypointense in 5 cases (1 5.2%),inhomogeneous slightly hyperintense in 3 cases (9.0%).On T2 WI,all lesions were well defined and showed hyperintense signals with hypointense septation,of which there were 9 cases with nodular hypointense areas and vascular flow effect.Hypointense phleboliths were showed in 10 cases(30.3%).Following injection of the contrast medium,all lesions had heterogeneous enhancement patterns.Of the 33 patients studies with MRI,lesions of 18 cases(54.5%)had bone changes which were adjacent to or partially or fully wrapped by neighbouring soft tissue lesions,in which lesions of 12 cases showed abnormal signal within bone marrow and lesions of 3 cases with vascular flow void phenomenon.In 15 cases (45.5%)without osseous change,lesions of 3 cases were adjacent to bone,and lesions of 12 cases were with fat and muscle septum between the bone and lesions.Conclusion The hemangiomas and vascular malformation in the deep soft-tissue of the extremities may cause changes in adjacent bones.Familiarity with the performance of reactive bone changes on X-ray and MRI may help to improve the diagnosis and avoid misdiagnosis.
4.Imaging analysis of simple cochlear nerve canal stenosis

Objective To study the imaging manifestations and clinical significance of simple cochlear nerve canal(CNC)stenosis. Methods The HRCT findings of 14 patients(1 7 ears)with simple CNC stenosis diagnosed sensorineural hearing loss(SNHL)were retrospectively analyzed.The width of CNC and internal auditory canal was measured and compared with the normal ears,the data were statisti-cally analyzed by SPSS1 6.0.Thin-sliced MRI of internal auditory canal (axial and oblique sagittal position)was performed in 8 cases. Results HRCT displayed obvious CNC stenosis in all 14 patients(1 7 ears),2 ears with cochlear nerve canal atresia.Width of CNC and internal auditory canal was (0.5±0.3)mm and (4.6±1.0)mm,it was (2.1±0.2)mm and (5.1 ±1.3)mm in normal ears (1 1 ears),the width of CNC was of significant difference between normal ears and ears with SNHL,and there was no statistical sig-nificant difference in width of internal auditory canal.12 ears(from 1 7 ears with simple CNC stenosis)were lacking of fissure in the cribriform area,1 1 ears(normal ear)were existence of fissure in the cribriform area;cochlear nerves were not shown in 8 patients with MRI examination.Conclusion CNC stenosis can be isolated from internal auditory canal stenosis,MRI could display cochlear nerve dysplasia(CND).

OBJECTIVE To explore the potential neurotoxicity of nano-alu mina (<50 n m)in vivo, we treated the ICR mouse with the nano-alu mina to investigate the mitochondrial da mage of nerve cells on morphology and function.METHODS Adult male mice were exposed to nano-alu mina (<50 n m)of 0,25,50 and 75 mg·kg -1 by nasal instillation for 1 month.Then we observed the mitochondrial ultra-structure of the nerve cells in CA3 region of hippoca mpus,and measured the mean dia meter in every group.The activities of Na +-K +-ATPase and Ca2 +-Mg2 +-ATPase were tested by the determination of the inorganic phosphorus,which was the deco mposition product of ATPase.Western blot analysis was used to detect the expression of COX-Ⅳ,Beclin1 ,LC3Ιand LC3Ⅱ.RESULTS Co mpared with 0 and 25 mg·kg -1 groups exposed to Al2 O3 nanopartilces (Al2 O3 NPs),the mitochondria of CA3 region in hip-poca mpus in 50 mg·kg -1 group beca me ede matous and swollen with sparse and broken cristae sur-rounding the nuclear,and the mean dia meter was higher(0.49 ±0.02 μm,P <0.05).But co mpared with 50 mg·kg -1 group,the mitochondria in 75 mg·kg -1 group beca me s maller with inner cristae of high density,and the mean dia meter was lower(0.36 ±0.02 μm,P<0.05).The enzy me activity of the mito-chondria in cerebral cortex decreased dose-dependently with exposure,the activities of Na +-K +-ATPase in 50 and 75 mg·kg -1 groups(6.37 ±0.22 kU·g -1 protein,5.48 ±1 .53 kU·g -1 protein)and Ca2 +-Mg2 +-ATPase in 50 and 75 mg·kg -1 groups (3.21 ±0.99 kU·g -1 protein,3.28 ±0.15 kU·g -1 protein)were lower than the 0 mg·kg -1 group(P<0.05).Meanwhile,the Ca2 +-Mg2 +-ATPase in 50 and 75 mg·kg -1 groups showed lower activities in co mparison with the 25 mg·kg -1 group.The 75 mg·kg -1 group expressed higher level of the COX-Ⅳ protein 1 .35 ±0.66(P<0.05)than other groups.Both expression of Beclin1 protein and rate of LC3Ⅱ/LC3Ⅰin 75 mg·kg -1 group were more than the 0 mg·kg -1 group. CONCLUSION The mitochondrial dysfunction may be the potential neurotoxicity of nano-alu mina,and the da maged mitochondria were cleared by autophagy.
7.Computer-aided design and rapid prototyping technique for individualized artificial semi-knee joint prosthesis: Establishment of three-dimensional model of femoral condyle

BACKGROUND:At present, there are several limb-preserving methods,such as xenogenic bone joint replacement, artificial metal prothesis replacement and inactivation and replantation of tumor bone segment, etc.Each method has its advantages and disadvantages. In addition, mismatch of xenogenic and autogeneic joints and articular cartilage necrosis in the late stage, which affects joint function, are two important problems for large frozen bone-joint allograft at ultra-low temperature in repair of bone defect following bone tumor resections in extremities.OBJECTIVE:To evaluate a method to obtain outline information on the cartilage surface through spiral CT scanning data, so as to lay foundation for study on individualized artifical semi-knee joint based on rapid prototyping (RP) technique.DESIGN: Open experiment.SETTING: First Department of Trauma, Urumqi General Hospital of Lanzhou Military Area Command of Chinese PLA.MATERIALS: This experiment was carried out at Institute of Orthopaedics, Xijing Hospital, Fourth Military Medical University of Chinese PLA, and Institute of Advanced Manufacturing, Xi'an Jiaotong University from September 2001 to May 2003. CT scanning subject was a 25-year-old healthy male volunteer.METHODS:Distal femur was performed 1 mm in thickness scanning with Picker 6 000 spiral CT. Three-dimensional volume reconstruction was conducted at the Voxel Q Image workstation on Picker 6000 CT, then, two-dimensional tomography image of reconstruction data were downloaded with spacing of 0.1 mm. Data format converter software was self-developed. The downloaded image was treated through smoothing, de-noise and so on.Vector data of two-dimensional borderline outline of tomography image was calculated and input into Surfacer 9.0 software (American Imageware company) for vectorial three-dimensional reconstruction. According to recognition of articular cartilage outline and request of prothesis design, the threedimensional images of interested articular cartilage surface outline were extracted and used in the computer-aided design for individualized artificial semi-knee joint.MAIN OUTCOME MEASURES:Vector converting of CT image and vector image of three-dimensional reconstruction of femoral condyle.RESULTS:Vector converting of CT image dafa had been come true using self-developed medical image vector converter software. Three-dimensional solid model of individualized femoral condyle was constructed with Surfacer9.0 three-dimensional processing software and edited according to design request. Three-dimensional model of articular cartilage requested by artifical semi-knee joint prothesis was extracted for computer-aided design. The constructed articular surface outline could be treated further to complete computer-aided design of artificial semi-knee joint prothesis; The file format was .stl, which could be recognized by RP software and used in engineering.CONCLUSION:Vector reconstruction of articular cartilage outline is performed based on spiral CT data that can obtain precise three-dimensional solid model of articular cartilage outline. The three-dimensional model can be edited, which lays foundation for computer-aided design and RP manufacturing of artificial semi-knee joint prothesis compounded with large segment xenogenic bone; It is easy and practicable in vector converting of medical image information with this method, and it also has good application prospect in biomanufacturing field in orthopaedic and oral maxillofacial surgery.
8.Clinical analysis of transradial versus transfemoral approach for percutaneous coronary intervention

Objective To analyze the clinical effect and complication of trans-radial and femoral artery for percutaneous coronary intervention (PCI) in patients with coronary heart disease.Methods Totally 153 patients with coronary heart disease undergoing PCI were divided into radial artery and femoral artery groups.X-ray exposure time,operation time,the success rates of puncture and operation,in-bed time and complication were recorded. Results There were no significant differences in X-ray exposure time[(17±5)min vs.(16±6)min,t=0.61,P=0.57],operation time [(49± 9) min vs. (48 ± 11) min,t=0.59,P =0.61],the success rates of puncture (98.7% vs.100.0%,x2 =0.47,P=0.53) and operation (96.0% vs.96.2%,x2 =0.14,P=0.64) between radial artery and femoral artery groups.However,the complication rates was higher in femoral artery group than in radial artery group (17.9% vs.2.7 %,x2 =9.54,P=0.002),in-bed time was shorter in radial artery group than in femoral artery group [(4.5 ± 1.2)h vs.(13.2 ±4.6)h,t=2.12,P =0.003]. Conclusions The trans-radial artery PCI is safe,effective and feasible,with less complications and shorter in-bed time.
9.The effect evaluation of a nutrition and food safety science experience among primary and secondary school students

Objective To evaluate the effectiveness of an activity of nutrition science experience on nutrition and food safety cog-nition among primary and secondary school students .Methods With stratified cluster sampling ,students of grade 5 and 6 in one primary school and students of grade 1 and 2 in one middle school in Chongqing were selected ,and randomly divided into interven-tion group(n= 501) and control group(n= 522) .Only conducted the activity in the intervention group .Baseline data of all the students were investigated before the intervention .Effect evaluation was performed instantly in the intervention group and control group ,and a follow-up survey carried out in the intervention group after 9 months(n= 472) .Results The nutrition knowledge scores of instant intervention group were 9 .03 ± 2 .75 and 14 .70 ± 3 .28 before and after intervention respectively (U=29 .78 ,P<0 .01);the knowledge scores of the nine months later intervention group were 12 .35 ± 2 .89 ,which were lower than instant interven-tion group(U=12 .40 ,P<0 .01) ,but higher than before intervention(U=18 .04 ,P<0 .01) .The food safety scores of instant inter-vention group ,which were higher than control group ,nine months later intervention group and before intervention(P<0 .01) .Con-clusion It is feasible and effective to conduct a nutrition science experience among primary and secondary school students .
10.Changes of sympathovagal balance in endotoxemia rats

Objective To investigate the changes of sympathovagal balance and the effects of va- gus stimulation on sympathovagal balance in endotoxemia rats.Methods Twenty-four Spragne Dawley rats were randomly divided into four groups.The frequency domain of heart rate variability(HRV)com- ponent was analyzed at 0 min,2 ,4 and 6 hours after intravenous injection of lipopolysaccharide(LPS, 5mg/kg)or physiologic saline,and cervical vagal nerve was stimulated(5mv,2ms,1Hz,5 min lasted, 20 min interval)when LPS or physiologic saline was injected.The levels of Noepinephine(NE)and Ace- tylcholin(ACh)were measured in liver tissues.Results Normalized low frequency(LFnm),hormali- zed high frequency(HFnm),very low frequency(VLF),LF/HF values and liver ACh were significantly increased(P<0.05)and the level of liver NE was significantly decreased (P<0.05)after LPS admin- istration.Vagal nerve stimulation markedly increased HFnm but decreased LFnm,VLF,LF/HF values, and the liver ACh also significant increased(P<0.05 ).Conclusion The results suggest that the ac- tivity of sympathetic and vagal nerve was increased during endotoxemia,but the sympathetic activity was more excitable than that of vagal nerve.Vagal nerve stimulation increased the tone of vagus nerve while the tone of sympathetic nerve was decreased in this study.This may be beneficial for anti-inflammatory activity of vagal nerve.
