1.Long-term Follow-up Study of Ascending Aortic Diameter Changes in Patients After Mechanical Aortic Valve Replacement

Objective: To retrospectively analyze the ascending aortic diameter (AAD) changes in patients after mechanical aortic valve replacement (AVR).
Methods: The medical records and echocardiography reports in patients who received AVR or bivalve valve replacement (BVR) in our hospital from 2000-01 to 2001-12 were retrieved, the retrieval conditions were as aortic valve structure must be mechanical and the follow-up echocardiography examination should be more than 9 years. The clinical information, pre- and post-operative 2-dimentional transthoracic echocardiography reports were collected, the follow-up echocardiography periods were ≤3-year, 3-year<-≤6-year, 6-year<-≤9-year and >9-year. AAD changes at different periods were compared. According to baseline AAD, the patients were divided into AAD<35 mm group and AAD≥35 mm group in order to observe the ascending aortic events.
Results: A total of 141 patients were enrolled form 595 echocardiography reports which included 75 male, the patients were at the mean age of (45.5 ± 11.2) years with mean follow-up time of (7.59 ± 3.38) years. Compared with baseline level, the follow-up AAD was similar between ≤3-year and 3-year<-≤6-year patients,P>0.05; while the follow-up AAD was different between 6-year<-≤9-year and >9-year patients,P<0.05. The patients with second operation in AAD≥35 mm group were much higher than those in AAD<35 mm group (24.0% vs 12.9%). There were 5 (20%) patients suffered from ascending aortic events in AAD≥35 mm group.
Conclusion: AAD dilatation were gradually occurring after mechanical AVR, the patients with AAD≥35 mm had the higher risk for ascending aortic events, therefore special attention should be taken in patients with aortic valve disease combining AAD dilatation during surgical treatment.
2.Effect of aluminum trichloride on dissociated Ca2+ in Hippocampus neuron cell as well as learning and memory.

OBJECTIVETo investigate the effect of aluminum chloride on dissociated Ca(2+) in hippocampus neuron cells in mice and the relationship to the learning and memory.
METHODSMale ICR mice in the three intoxicated groups were administered with the double distilled water containing AlCl(3) (10, 50, 300 mg.kg(-1).d(-1)) while those in the control group were administered with the double distilled water for 100 days. The methods of behavior toxicology such as Morris swim maze were used for studying the effect of aluminum chloride on the changes of learning and memory in mice. With calcium sensitive fluorescence indicator Fura-2 as the fluorescent probe, the influence of the subchronic exposure to Al on the dissociated Ca(2+) in hippocampus neuron cells was observed.
RESULTSThe dissociated Ca(2+) in hippocampus neuron cells in the middle dosage group and the high dosage group [(412.25 +/- 53.20), (467.37 +/- 32.85) times] was lower than those in the control group [(293.91 +/- 32.21) times] respectively (P < 0.01), and correlated positively with the dose and dissociated Ca(2+) (r = 0.861, P < 0.01). Compared with the control group, the latent period was lengthened (P < 0.05) in the middle dosage and the high dosage group.
CONCLUSIONThe subchronic exposure to AlCl(3) in mice affects the dissociated Ca(2+) in hippocampus neuron cells. The increase of dissociated Ca(2+) in hippocampus neuron cells may be correlated with the disfunction of cognition in the aluminium intoxicated mice.

3.A follow-up study about self-acceptance and automatic thoughts in permanent colostomy patients

Objective To explore the dynamic changes and correlation of self-acceptance and automatic thoughts in permanent colostomy patients. Methods There were 89 low position rectal cancer patients after Miles and permanent colostomy were recruited from the Department of Oncological Surgery of the First Affiliated Hospital of Xi'an Jiaotong UniversityfromFebruary 2016 to October 2016. Self-acceptance Questionnaire and Automatic Thoughts Questionnaire were used to collect data in 1 month, 3 months, 6 months and 12 months after colostomy. Results The self-acceptance scores were 33.64 ± 5.52, 33.24 ± 5.71, 34.56 ± 6.04 and 38.91 ± 6.33 respectively, which had significant differences (F=11.012, P<0.01). The automatic thoughts scores were 63.20 ± 16.78, 60.88 ± 16.53, 58.64 ± 15.87 and 55.32 ± 14.76 respectively, which had significant differences (F=3.462, P<0.05). The correlation between self-acceptance and automatic thoughts were-0.576--0.372, which were all significant (P<0.01). Conclusion There were negative correlations between self-acceptance and automatic thoughts in permanent colostomy patients within 1 year after operation. Health professionals should learn to identify patients′automatic thoughts accurately, lead off negative emotions, promote early self-acceptance and improve their quality of life.
4.Directed evolution of D-lactonohydrolase by error prone PCR and DNA shuffling.

D-lactonohydrolase is useful in the procedure of resolution of racemic pantolactone to produce D-pantolactone, but the activity and stability under low pH of the wild type enzyme is not satisfactory enough to be applied to industrial production. The expected properties of wild type enzyme were enhanced by directed evolution. According to the formation of products and pH indicators, a screening system was designed. After three sequential error prone PCR and one round DNA shuffling followed by screening, Mut E-861, the best mutant with improved activity and stability under low pH situation was obtained. Gene analysis of the Mut E-861 mutant indicated that the mutant enzyme had A352C, G721A mutations and a silent mutation of position 1038. Moreover, the activity and stability of Mut E-861 were determined. The results showed that the activity of this mutant was 5.5-fold higher than that of wild type, and the stability under low pH was improved at no expense of D-lactonohydrolase activity. After incubated at pH 6.0 and pH 5.0 the activity of D-lactonohydrolase could be retained 75% to 50%, however, compared with 40% to 20% for wild type.

5.Feasibility of peptide mass fingerprinting for differential diagnosis of IgA and non-IgA nephropathy.

OBJECTIVETo investigate the feasibility of peptide mass fingerprinting for non-invasive differential diagnosis of IgA nephropathy (IgAN) from the non-IgA nephropathy (IgAN).?
METHODSAccording to the results of renal biopsy, 56 patients were divided into IgAN group (n=28) and non-IgAN group (n=28), and peptide mass fingerprints were acquired from these patients using matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS).
RESULTSNine different peptides were identified between IgAN and non-IgAN. The two most distinctive differentially expressed peptides, with peptide peak values of 4476.46 and 1968.10, showed area under curve values of 86.18% and 79.77%. Principal component analysis demonstrated that the accumulated explained variance of the first 8 differential peptides reached 95%, suggesting the feasibility of differential diagnosis of IgAN from non-IgAN. Comparison with the Matrix protein database identified the peptide with a relative molecular mass of 5338.08 as a fragment of mucin 4 inform and the 2082.77 peptide as fragment of α1-II type collagen inform.
CONCLUSIONMALDI-TOF MS is feasible for differential diagnosis of IgAN and non-IgAN and also has great potentials in the classification of the subtypes of other systemic diseases.

6.Gentiopicroside,originated from Gentiana macrophylla Pall,possesses anti-arthritic efficacy in adjuvant-induced arthritis rats

OBJECTIVE This work aimed to investigate the anti-rheumatoid arthritic effect of gentio-picroside from Gentiana macrophylla Pall using an animal model of adjuvant induced arthritis. METH-ODS Adjuvant arthritis was induced in fifty SD male rats,which were randomly divided into five groups (n=10):control(0.5% CMC-Na)group,AIA(rats with CFA)group,dexamethasone(1 mg·kg-1)group, gentiopicroside(50 mg·kg-1)group,and gentiopicroside(100 mg·kg-1)group.Rats were administered intragastrically with drugs or CMC-Na once a day for a period of 2 weeks.Paw swelling,arthritic index, histological changes were assessed to evaluate the anti-arthritic effect.Weight growth,spleen and thymus indexes were also investigated in.RESULTS Gentiopicroside at dose of 100 mg·kg-1significantly inhibited the secondary paw swelling(P<0.05)and arthritis index(P<0.05),decreased synovial inflammatory infil-tration, synovial hyperplasia and bone erosion. Furthermore, gentiopicroside showed no immunosup-pressive adverse effects in body weight, index of spleen and thyums compared with dexamethasone administration (P<0.05, P<0.01). CONCLUSION Gentiopicroside possessed anti-arthritic efficacy in AIA rats without immunosuppressive effects.
7.Effect of Radix astragali injection on the cytochrome P450 activities with ultrahigh performance-LC-MS/MS in rats

Objective To study the effect of Radix astragali injection (RA) on the activities of cytochrome P450 2D6(CYP2D6),2C19,3A4,1A2 and 2C9 by ultrahigh performance-LC and tandem mass spectrome try(UHPLC-MS/MS).Methods The activities of CYP2D6,2C19,3A4,1A2 and 2C9 were evaluated based on pharmacokinetics of metopmlol,omeprazole,midazolam,phenacetin and tolbutamide (five probe drugs) respectively.SD rats were assigned to two groups:control group and experimental group and the rats in the two groups were given 0.9% NaCl and RA 10 mL · kg-1 by intraperitoneal injection for consecutive 7 days respectively.On the 7th day,after intraperitoneal injection for 30 min,a mix dose of five probe drugs was intragastric administration to all rats.Blood samples were collected at different times for UHPLC-MS/MS analysis.Pharmacokinetic parameters of five probe drugs were assessed between the two groups respectively.Results For metoprolol,omeprazole,phenacetin and tolbutamide,the AUG0-t in RA group decreased by 37.8%,55.5%,40.9% and 38.9% respectively,the Cmax in experimental group decreased by 25.4%,33.6%,14.6% and 40.2% respectively.While the AUC0-t and Cmax of midazolam in experimental group increased significandy by 60.1% and 195% respectively.RA could increase the total amount of midazolam absorbed into blood,while decreasing the total amounts of metoprolol,omeprazole,phenacetin and tolbutamide absorbed compared with control group respectively.The method of validation conformed to the requirements of the relevant rat pharmacokinetic guidehnes.Conclusion This method was successfully employed to assess the effect of RA on the activities of CYP2D6,2C19,3A4,1A2 and 2C9 in vivo.RA could inhibit the activity of CYP3A4,while inducing the activities of CYP2D6,2C19,1A2 and 2C9.
8.Determination of caderofloxacin concentration in healthy human plasma by RP-HPLC

Objective To establish a sensitive and specific RP-HPLC method for determination caderofloxacin concentration in healthy human plasma. Methods Human plasma with internal standard was precipitated by perchloric acid. The separation was analyzed in gradient elution system and the detection wave was 286 nm. Results The linear range of pseudoephedrine was 0.1 ~ 10. 0 mg · L-1. The relative re-covery were within 96.41% ~ 109.52%. The within - day and the be-tween day RSD were all less than 15%. After single infusion cadero-floxacin of 400 mg/3 h in two healthy volunteers. It was found to be fit-ted to a two - compartment open model and its pharmacokinetic param-eters were as follows: Cmaxwas, 3.39, 4.04 mg·L-1, AUC0-t was 32.35,28.26 mg · h·L-1, t1/2β was 7.74,6.73 h, respectively.Conclusion The method is accurate, convenient, sensitive, specific,and was suitable for the study of clinical pharmacokinetics.
9.Effects of oral carbohydrate drinks before cesarean section on maternal and infant outcomes

Objective To study the effect of drinking carbohydrate drinks before cesarean section on mothers and neonates,and to explore the application value of drinking carbohydrate drinks before cesarean section.Methods The clinical data of 206 singleton women who underwent selective cesarean section in Obstetrics and Gynecology Hospital,Fudan University from Jun 2020 to Jun 2021 were retrospectively studied.Patients were divided into enhanced recovery after delivery(ERAD)group and control group according to whether drinking carbohydrate drinks before cesarean section.A retrospective cohort study was conducted to analyze the effect of preoperative carb drinks on preoperative fluid supplementation,postoperative rehabilitation and neonatal prognosis.Results Among patients who fasted for less than 12 hours,the ERAD group had a lower fluid supplementation rate and a smaller average fluid supplementation volume compared to the control group(P<0.05).The ERAD group had a lower rate of prokinetic agent using after surgery(P<0.05).Among women without a history of abdominal surgery,the ERAD group had less blood loss 24 hours after surgery(P<0.05).There were no significant differences in postoperative fever rate,incidence of nausea and vomiting,time of first flatus,neonatal apgar score,exit observation room neonatal blood,and neonatal neonatal intensive care unit(NICU)admission rate between the two groups.Among newborns with high-risk factors for hypoglycemia,the ERAD group had lower enter observation room neonatal blood compared to the control group,and a higher incidence of hypoglycemia(P<0.05).Conclusion Oral intake of carbohydrate drinks before cesarean section may be beneficial in reducing fluid supplementation before elective cesarean section,promoting postoperative gastrointestinal function recovery,and reducing postoperative bleeding.However,it may be related to the occurrence of neonatal hypoglycemia.
10.Expressions of E-cadherin and beta-catenin in LNCaP and ARCaP cell lines and their significance.

OBJECTIVETo observe the expressions of E-cadherin and beta-catenin in LNCaP and ARCaP cell lines and explore their relationship with the metastasis of human prostate cancer.
METHODSThe expressions and distribution of E-cadherin and beta-catenin in LNCaP and ARCaP cell lines (IF11 and IA8) were detected by Western blot and immunofluorescent staining.
RESULTSThe expression of E-cadherin was high in LNCaP, but absent in IF11 and IA8, while beta-catenin was expressed highly in IF11 and LA8, but lowly in LNCaP. Immunofluorescent staining showed that E-cadherin was mainly in the membrane of LNCaP, while beta-catenin both in the membrane of LNCaP and in the nuclei of IF11 and IA8.
CONCLUSIONE-cadherin and beta-catenin are differently expressed and distributed in prostate cancer cell lines with different characteristics of epithelial-mesenchymal transition (EMT), and the abnormal activation of the beta-catenin signal pathway may be involved in the EMT of prostate cancer cells.
