3.A case of giant cell tumor of tendon sheath.

Giant cell tumor of tendon sheath(GCTTS) is a benign mesenchymal lesion composed of cytologically bland mononuclear cells admixed with multinucleated giant cells. Our report describes the clinical and pathologic features of a 36-year-old female's tumor with a two years history over the lateral side of the left little finger, Especially, we could observe the characteristic thre types of cells in an electromicroscopic study, histiocyte-like cells, fibroblast-like cells and multinucleated giant cells. In lmmunohistochemical studies, we demonstrated that the tumor cells of GCTTS exhibit a phenotype consistent with histiocyte differentiated to Langerhan's cell.

6.Rapid Diagnosis of Isoniazid Resistance by Detection of Mutations in katG and inhA of Mycobacterium tuberculosis from Korea.

29 isoniazid (INH) resistant isolated strains and INH sensitive reference strain (H37Rv) of Mycobacterium tuberculosis were analysed by polymerase chain reaction-single strand conformational polymorphism (PCR-SSCP) and NciI restriction mapping for the detection of mutations in katG gene and inhA gene. The katG gene was divided into 3 parts (Akat, Bkat, Ckat; each part is about 800 bp) and amplified, inhA gene was amplified as a whole. Each of the amplified 800 bp DNA was digested into small fragments of less than 400 bp with restriction enzymes for the direct PCR-SSCP analysis. Firstly, 10 strains were analysed. All the 10 isolates showed clearly distinct SSCP patterns in Bkat from that of the reference strain, but only two isolates showed distinct SSCP patterns in Akat, and no isolated strain showed any distinct SSCP patterns in Ckat. 10 isolates also showed distinct SSCP patterns in inhA. NciI restriction mapping of Bkat showed mutation in codon 463 in 7 strains among 10 isolated strains. With these results an early detection strategy for the INH resistant M. tuberculosis was applied to the rest of 19 isolated INH resistant strains. Firstly, isolates were screened by Ncsl mapping in Bkat, and 13 strains showed mutations in codon 463. Secondly, the rest of 6 INH resistant isolates were analysed by PCR-SSCP with restriction enzyme digestion (PCR-SSCP-RE) in Bkat, and all the strains showed distinct SSCP patterns from that of the INH sensitive reference strain. This proved our strategy as effective and economic and time saving method in early detection of INH resistant M. tuberculosis.

7.Toxic Shock Syndrome following Tattooing.

Toxic shock syndrome (TSS) is a rare but life-threatening illness that is mainly caused by toxigenic strains of Staphylococcus aureus. Although TSS is classically known to be associated with tampon use, the number of TSS cases with non-menstrual causes such as skin and soft tissue infection has been increasing. Tattooing can result in several complications such as localized and systemic infections, inflammatory skin eruptions and neoplasms. We recently experienced a 26-year-old man diagnosed with typical TSS following tattooing. He complained of fever, chills and erythematous rash at tattoo site. Subsequently, the patient developed sign of shock. The skin cultures on the tattoo site were positive for methicillin-sensitive Staphylococcus aureus. The patient was successfully treated with vasopressor infusion and intravenous antibiotics and was discharged without complications. On discharge from the hospital 7 days later, desquamations on the tattoo site, fingers and toes were observed.

8.A Case of Hemorrhagic Renal Cyst.

We have experienced an unusual case of hemorrhagic renal cyst in the rt. kidney at 58 years old woman. The Rt. kidney was enlarged, and showed a few subcapsular ischemic areas The color of kidney was dark brown. As the cyst at lower pole of rt. kidney was dissected, hemorrhagic fluid visualized in the cystic cavity. The final pathologic finding was renal cyst associated with malignant change and renal cell carcinoma at another renal parenchyma. Herein, hemorrhagic renal cyst, a rare clinical entity, was reported with review of literatures.

9.Exophytic Adenocarcinoma of the Rete Testis Mimicking Paratesticular Mass: A Case Report.

Adenocarcinoma of the rete testis arising in the mediastinum testis is a rare, highly malignant neoplasm. Wedescribe one such case in a 25-year-old male who presented with a palpable right scrotal mass which metasta-sizedto the right inguinal lymph node eight months later. Ultrasonography of this tumor showed a complex cystic andsolid mass in the swollen right scrotum, which was separate from the right testis. Ultrasonographic and clinicalfindings suggested that the mass was, indeed, paratesticular. Surgical pathology following right radicalorchiectomy and hemiscrotectomy, however, confirmed the presence of an exophytic adenocarcinoma of the retetestis. A pelvic CT performed to evaluate a right inguinal mass, which was palpated eight months later, revealed amainly cystic mass in the right inguinal area. Metastasis was confirmed by biopsy.

10.Management of systemic lupus erythematosus.

Systemic lupus erythematosus (SLE) is an autoimmune disorder which affects the musculoskelectal system, skin, kidney, central nervous system, gastrointestinal tract, and vascular system. The therapeutic level depends on the severity of SLE activity and affected organ damage. The prognosis of SLE has greatly improved since treatment regimens combining corticosteroids and immunosuppressive agents have been widely adopted in therapeutic strategies. Immune suppression is evidently effective, however, sometimes leads to higher susceptibility to infectious and malignant diseases. Toxic effects and unexpectedly fatal complications of current therapies have been progressively reported. Currently, identifying novel molecular targets such as hyperactive B cells could be an important issue in the treatment of SLE. The aim of this review is the introduction of previous adopted therapies as well as emerging pharmacological options.
