Objective To discuss the early diagnosis of severe perioperative biliary infection complicated with DIC and efficacy of comprehensive treatment for it. Methods Comprehensive treat-ment was conducted in 21 patients who had intensive biliary infection complicated with DIC. The he-moglutination function was monitored before and after treatment. Results Of the 21 patients, 19 were cured and 2 dead. The curative rated was 90.5 %. The hemorrhage tendency was controlled, abdomi-nal bleeding stopped, prothrombin time (PT) obviously reduced, content of fibrinogen (Fbg)in-creased, level of D-dimer and FDPs obviously dropped. Conclusion The patients who have biliary in-fection in the perioperative period are easy to suffer from DIC and the mortality is high. Early diagno-sis and comprehensive therapy, especially the application of the cryofibrinogen and heparin, are very important.

Objective:Common troubleshooting methods of networking malfunction for central monitoring system were discussed in order to realize its safe application of networking central monitoring system.Methods: Time calibration server and necessary conditions of network for central monitoring system were discussed and analyzed. In addition, practical troubleshooting and maintenance methods were proposed to solve the networking malfunction between a bedside monitor of Mindray iPM-9800 and its central workstation.Results: Smooth operation of networking central monitoring system could simultaneously obtain physiological parameters of several patients, thus largely improving the efficiency of medical staffs.Conclusion: Safe operation of networking central monitoring system could ensure the accuracy of real-time monitoring information of patients and improve hospital’s management level of network and informatization.

Objective To investigate the correlation between the D-dimer level and the acute exacerbation of chronic obstructive pulmonary disease (AECOPD).Methods The changes of D-dimer levels in 156 patients (AECOPD group) before and after treatment were measured and compared with the D-dimer levels of 156 healthy subjects(control group).Results The concentration of plasma D-dimer in the AECOPD group was (1.35 ± 1.46) mg/L,which was significantly higher than (0.25 ± 0.16)mg/L in the control group,and the difference was statistically significant(t =9.354,P ＜0.05).The level of plasma D-dimer in the AECOPD group was (0.69 ± 0.87)mg/L,and compared with (1.35 ± 1.46)mg/L before treatment,the difference was statistically significant(t =4.850,P ＜ 0.05).Conclusion D-dimer level is correlated to the condition of patients with chronic obstructive pulmonary disease,it can be used as one of the indicators to diagnose whether a patient is in hypercoagnlable state or suffers thrombotic diseases.

Objective To investigate the protective effect and it's possible mechanism of Tanshinone ⅡA in radiation-induced pulmonary fibrosis.Methods Having the right hemithorax of female Wistar rats irradiated(30?Gy) in 10 fractions within 14 days by 6 ?MV photons,the radiation-induced pulmonary fibrosis animal model was established.In the treatment group,sodium Tanshinone ⅡA sulfonate(15?mg/kg) was given by intraperitoneal injection 1 hour before each fraction of irradiation.Five months after irradiation,the difference of the histopathological changes,the hydroxyproline content and expression of TGF-?1 between the radiation alone group,tanshinone plus radiation and control group were analyzed by HE stain,Massion stain,immunohistochemical methor and reverse transcriptase polymerase chain reaction(RT-PCR) method.Results The histopathological comparison revealed the protective effect of Tanshinone ⅡA.The content of hydroxyproline was(21.99?3.96),((38.25?)(7.18)),(28.94?4.29)??g/g in the control group,radiation alone group and radiation plus Tanshinone ⅡA.The expression of TGF-?1(mRNA and protein) was reduced by Tanshinone ⅡA.Pathological changes of the pulmonary fibrosis was reduced by Tanshinone ⅡA yet.Conclusions Our study shows that Tanshinone ⅡA can inhibit radiation-induced pulmonary fibrosis,and the possible mechanism of its may be made possible through down-regulating the expression of TGF-?1 in the irritated lung tissue.

Objective To summarize the research results at home and abroad of raltitrexed on the patients with advanced gastric cancer,and to evaluate the clinical efficacy and adverse reactions of raltitrexed. Methods Clinical controlled trials of evaluating effects of raltitrexed on the advanced gastric cancer were searched in PubMed,EMBase,Ovid,Springer,Wanfang database,VIP database and China Biology Medicine disc. Besides,references and conference abstracts were searched manually. Literature screening and data extraction were performed according to inclusion and exclusion criteria. Then Meta-analysis was performed using RevMan 4. 2. Results Forty-four studies were found and 5 clinical controlled trails including 338 patients were included in this analysis. The results of Meta-analysis showed that there was a significant beneficial effect of raltitrexed on objective effective rate for the patients with advanced gastric cancer(RR = 1. 55,95% CI:1. 21-1. 99,Z = 3. 44,P = 0. 00)compared with other treatments. Objective To understand the reliability and validity on SF-36 scale and the quality of life (QoL) among patients with advanced-schistosomiasis in Jiangsu province.Methods Cluster sampling method was used to choose 586 advanced-schistosomiasis patients who were registered in Jiangsu province.Questionnaire including SF-36 module,was used for face to face interview on the patients.Statistical analyses were made to assess the results,including correlation analysis,reliability analysis,factor analysis,t-test and one-way ANOVA.Results The split-half reliability coefficient was 0.92,P＜0.001 and the internal consistency reliability was 0.90,with the range of Cronbach' α coefficient on each dimension was from 0.69 to 0.98.Through principal component analysis,two factors were identified,with the accumulative contribution rate as 67.37％.SF-36 showed certain discriminant validity to distinguish groups in different economic levels.Compared with the domestic norm data,the scores of SF-36 on those patients were generally lows.Conclusion SF-36 had good reliability and good criterion validity,with a certain degree of discriminant validity,but with poor constructs validity.Level of QoL of patients with advanced schistosomiasis in Jiangsu was low because of being chronically ill,that called for more strategies to improve the QoL of patients.

Objective The aim of this study is to evaluate the efficacy and safety of preoperative CT-guided hardening agent localization.Methods From December 2010 to January 2012,27 patients with 29 solitary pulmonary nodules who had undergone CT-guided hardening agent localization and video-assisted thoracoscopic surgery (VATS) were studied.Results All cases were underwent CT-guided hardening agent localization successfully,and no patient had serious complication that required any intervention.The diameter of nodules ranged from 3 to 21 mm as measured by CT[mean (11.27 ± 6.32) mm].The distance between the center of nodule and visceral pleural ranged from 4 to 38 mm[mean (14.45 ± 4.32) mm].Conversion from VATS to thoracotomies was not necessary during the diagnostic resection procedure nodules.29 solitary pulmonary nodules underwent thoracoscopic wedge resection,and no intra-or postoperative mortality or morbidity was recorded.Conclusion CT-guided hardening agent localization before video-assisted thoracoscopic solitary pulmonary nodule resection is a safe and effective procedure for accurate diagnosis and resection of indeterminate solitary pulmonary nodules.

Objective To survey the biliary flora in patients with obstructive jaundice due to pancreatic head cancer,also the multiple factors which affect the positive findings of bile culture in these patients.Methods The information of 65 patients with obstruetive jaundice due to pancreatic head eancer,who admitted to surgery in Huashan Hospital from Oetober 2007 to October 2008 were reviewed retrospectively.The factors which may potentially affect the detection of bile pathogen in patients with malignant obstructive jaundice were studied with univarite analysis and muhivariate analysis,including age,history of biliary surgery,yellow stained time,serum alanihe aminotransferase level,serum bilirubin level,CA19-9 level,tumor size,site of obstruction,with or without clinical manifestations of biliary infection,and APACHE Ⅱ score.Results Twenty-five positive cultures happened in 65 bile samples (38.5％),including 21 strains of Gram-negative baeilli (72.4％),6 strains of Gram-positive bacteria (20.7％),and 2 strains of fungi (6.9％).Univariate analysis showed that the relevant factors which may affect the rate of positive bile culture in patients with malignant obstructive jaundice were age,history of biliary surgery,biliary obstruction site,biliary tract infection symptoms and APACHE Ⅱ score.Multivariate analysis showed that age,history of biliary surgery,biliary obstruction site and APACHE Ⅱ seore were independent risk factors.Conctusion Age,history of biliary surgery,biliary obstruction site and APACHE Ⅱ score were independent risk factors which led to positive findings of bile cultures in patients with obstructive jaundice due to pancreatic head cancer.
