2.Transrectal shear wave elastography combined with transition zone biopsy for detecting prostate cancer.

OBJECTIVETo evaluate the application of shear wave elastography (SWE) combined with transition zone biopsy in the detection of prostate cancer (PCa).
METHODSA total of 489 patients with suspected PCa underwent transrectal ultrasonography (TRUS) and SWE-guided prostatic biopsy. We evaluated the role of SWE combined with transition zone biopsy in promoting the detection rate in comparison with the results of biopsy pathology.
RESULTSThe pathological results confirmed 221 malignant and 268 benign cases. Based on systematic biopsy, SWE combined with transition zone biopsy achieved a detection rate of 45. 19% , significantly higher than that of systematic biopsy alone (33.13%) (P < 0.05). The diagnostic sensitivity, specificity, and accuracy of SWE were significantly better than those of TRUS (P < 0.05). The mean elasticity (Emean) of SWE was remarkably higher for malignant than for benign lesions ([40.1 ± 9.5] vs [21.6 ± 8.3] kPa, P < 0.05). With 28.5 kPa as the threshold of the Emean value, the area under the ROC curve was 0. 899, and the diagnostic sensitivity and specificity were 88.71% and 86.23%, respectively.
CONCLUSIONSWE combined with transition zone biopsy could significantly improve the detection rate of prostate cancer.

4.Modified urethral pull-through procedure for post-trauma posterior urethral stricture.

OBJECTIVETo evaluate the effect of the modified urethral pull-through procedure in the surgical treatment of posterior urethral stricture or obliteration caused by trauma.
METHODSFrom January 2001 to June 2005, modified urethral pull-through procedures were performed in 36 patients with post-trauma posterior urethral stricture or obliteration, 25 of them with post-operation urethral stricture or obliteration after urethral realignment by traction and the other 11 treated by cystostomy at the time of initial injury. The length of the urethral stricture segment was 1.0-4.5 cm, mean 2.2 cm. The patients were aged from 17 to 59 years (mean 44.5 years).
RESULTSAll the patients were followed up for at least 1 year. Success was achieved in 34 patients, among whom 25 had good voiding and no need of dilation, 6 were cured after 3 - 6 post-operative dilations, 3 required dilation every 2 or 3 months for over a year and 2 failed. No blood transfusion was needed during the operation, and no erectile dysfunction or incontinence occurred after it.
CONCLUSIONThe modified urethral pull-through procedure is effective and simple in the surgical treatment of post-trauma posterior urethral stricture or obliteration, with few complications and no consequent erectile dysfunction or incontinence.

5.Surgery for BPH with impaired detrusor contractility: a report of 12 cases.

OBJECTIVETo assess the outcome of prostatectomy simultaneously with suprapubic punctual cystostomy for BPH patients with impaired detrusor contractility.
METHODSTwelve cases of BPH with impaired detrusor contractility were diagnosed by urodynamic examination from 2002 to 2005. The patients underwent prostatectomy simultaneously with suprapubic punctual cystostomy and were followed up for a year.
RESULTSNine of the patients, at the average age of 69.00 +/- 5.13 years and with a short history of LUST, were restored to normal bladder function, while the other 3, older than 78 years and with a longer LUST history, failed to respond to the treatment.
CONCLUSIONBPH patients with impaired detrusor contractility that are younger and have a shorter LUST history could benefit from prostatectomy simultaneously with suprapubic punctual cystostomy, but the older ones with a longer LUST history should be treated by suprapubic punctual cystostomy alone.

6.Risk factors of the failure in digit replantation.

OBJECTIVETo explore the relative factors on the failure in digit replantation in order to take preventions to control the risk factors.
METHODSFrom January 2013 to December 2013, 236 consecutive patients (311 fingers) underwent digit replantation were collected to analyze retrospectively, involving 183 males and 53 females with an average age of 34.5 years old ranging from 2 to 62 years old (6 cases under 6 years old and 230 cases elder than 6 years old). There were 51 thumbs, 87 index fingers, 78 middle fingers, 63 ring fings and 32 little thumbs. Forty cases(forty fings) who were failured as the observation group, the others as the control group. The factors of age, gender, finger, cause of injury, smoking history, ischemia duration, plane of division, condition of venous drainage and condition of arterial repair we assessed.
RESULTSAll 236 cases with 311 fingers were replanted, 40 fingers were failured after operation. The relative factors on the failure in digit replantation included smoking history, cause of injury, plane of division, condition of venous drainage and condition of arterial repair (P< 0.05). There were no significant correlation between the failure and age, gender, finger and ischemia duration (P>0.05).
CONCLUSIONSmoking history, causes of injury, plane of division, condition of venous drainage and condition of arterial repair are risks of failure in digit replantation. Before choosing the type of operation, it should be think about the patient's general conditions, injury status, grasp firmly the operative indications and actively carry out surgical treatment.

7.A case-control study on depression and anxiety in hypertensive patients.

OBJECTIVETo study the current situation of depression and anxiety from patients with hypertension as well as to provide reference for the development of control and prevention program.
METHODSParticipants older than 35-year including both hypertensive patients and healthy controls were randomly selected in 2 communities of Pukou district in Nanjing through Health Behavior Survey. All the subjects were assessed by the Zung's self-rating depression scale (SDS) and the Zung's self-rating anxiety scale (SAS).
RESULTSRaw score and index score of SDS and SAS were both significantly (P < 0.01) greater in hypertensive patients than in healthy control group. The prevalence of depression of 17.9% and anxiety of 9.5% in patient group were found significantly higher than that in healthy control group as 11.5% and 4.3%. Data from logistic regression model analysis showed that depression and anxiety were possible risk factors of hypertension (OR = 1.677, 95% CI: 1.013-2.776; OR = 2.451, 95% CI: 1.228-4.894). There was a combined effect seen between depression and anxiety (OR = 5.238, 95% CI: 2.356-11.664) but interaction did not appear.
CONCLUSIONDepression and anxiety were possibly associated with hypertension and more attention needs to be paid to the mental health situation of hypertensive patients in order to improve their quality of life.

8.The prenatal diagnosis for absence of fetal cavum septum pellucidum by ultrasonography

Objective To explore the clinical significance of prenatal diagnosis for absence of fetal cavum septum pellucidum (CSP) by ultrasonography. Methods The ultrasonographic characteristics were retrospectively analyzed in 63 fetuses, whose CSPs were not detected in prenatal two and three dimensional ultrasonography in Nanjing Medical University Affiliated Suzhou Hospital. Results In 63 fetuses with absent CSP, the related malformations included:(1) Five cases were diagnosed as agenesis of corpus callosum (ACC, including four complete ACC and one partial ACC);(2) Twenty-seven cases were diagnosed as holoprosencephaly (HPE, including 18 alobar HPE, ifve semilobar HPE and four lobar HPE);(3) One case was diagnosed as schizencephaly;(4) Two cases were diagnosed as porencephaly;(5) Five cases were diagnosed as hydranencephaly;(6) Twenty-three cases were diagnosed as severe hydrocephalus, among which ifve cases were caused by aqueductal stenosis and the other 18 cases were caused by open spina biifda (Chiari Ⅱ malformation). These absent CSP fetuses had other abnormalities including one case of Dandy-Walker Malformation, one case of Dandy-Walker Variation, 14 cases of median cleft lip, ifve cases of single naris, two cases of proboscis nose, three cases of talipes foot and four cases of single umbilical artery, etc. These 63 cases were all followed up after prenatal ultrasonography but 4 cases were missed and one case of septo-optic dysplasia was misdiagnosed as lobar HPE. The diagnoses of other 58 fetuses by ultrasonography were conifrmed by follow up. Conclusions CSP is considered as an essential part in prenatal evaluations of the fetal central nervous system during second-and third-trimester. Absence of CSP is always accompanied with an extremely wide spectrum of fetal neuroanatomic malformations, including prosencephalon diseases, intracranial midline anomalies, etc. Prenatal ultrasonography is the best choice for the observation of CSP.
9.Variations of pulmonary venous drainage and venous ostium index detection in atrial fibrillation patients prior to radiofrequency catheter ablation by MDCT pulmonary venography.

OBJECTIVETo evaluate variations of pulmonary venous drainage and venous ostium index (VOI) in patients with atrial fibrillation (AF) prior to radio-frequency catheter ablation (RFCA) by MDCT pulmonary venography.
METHODS16-detector row CT pulmonary venography was performed in 64 AF patients referred to RFCA from June, 2005 to May, 2006. Variations in pulmonary venous drainage were observed in volume render imagines. Anterior-posterior and superior-inferior diameters of pulmonary venous ostium were measured on maximum intensity projection images. VOI derived from left superior, left inferior, right superior, right inferior pulmonary veins and variations in pulmonary venous drainage were calculated.
RESULTSClassic pulmonary veins anatomy was found in 11 patients (17.18%), early branching veins in 45 patients (70.31%), left common ostium in 5 patients (7.81%), right common ostia in 1 patient, right accessory (middle) pulmonary vein in 5 patients (7.81%) and left accessory (middle) pulmonary vein in 1 patient (1.56%). VOI of homolateral pulmonary veins and bilateral superior pulmonary veins were similar (P > 0.05) while there was a significant difference on VOIs derived from left superior and right inferior; two inferior, left inferior versus right superior veins (P < 0.05). Right inferior pulmonary venous ostium was most rounded and had the highest index (0.88) and left inferior pulmonary venous ostium was most oval and had the lowest index (0.72).
CONCLUSIONMultidetector row CT pulmonary venography (MDCT-PV) could provide valuable informations on pulmonary venous anatomy in AF patients referred to RFCA and should be used as a routine examination prior to the operation.

10.Effect of wound to growth of larva of host to Ophiocordyceps sinensis during artificial breeding.

To clear the effect of the wound to the growth of the larva of the host to the Ophiocordyceps sinensis, the wounds of same severity at the same position were made artificially to the larva and which were artificial fed at the same environment and condition. The results indicated that, over the winter, the survival rate of the wounded of the infection larva was lower than that of the healthy larva, but the weight had no significant difference between the wounded and the healthy larva. The survival rate of the wounded of the no infection larva was lower than that of the healthy larva, but except with black skin, the wounded larva with offwhite and dusty red had no influence on the variety of the weight. In summery, wound had no advantage to the survival rate, but had no influence to the weight. The result had provided theoretical basis to the reforming of the system of the artificial culture O. sinensis.
