1.Feature of clinic and coronary angiography of acute myocardial infarction for young people less than 30 years old

Objective:To explore feature of clinic and coronary angiography (CAG)of acute myocardial infarction (AMI)for young adults less than 30 years old.Methods:A total of 41 AMI patients,who were under 30 years old and hospitalzed in our hospital from 2004 to 2012,were enrolled as young group.Another 360 AMI patients >50 years were randomly selected as middle-aged group during the same time.Clinical features and CAG results were compared between two groups.Results:Compared with middle-aged group,there were significant rise in percenta-ges of male (68.1% vs.97.6%),smokers (50.6% vs.80.5%)and hypercholesterolemia (59.2% vs.65.9%)and body mass index [(22.3±6.6)kg/m2 vs.(26.3±9.6)kg/m2 ],and significant reduction in percentages of hyper-tension (63.1% vs.24.4%)and diabetes mellitus (30.6% vs.4.9%)in young group,P<0.05 or <0.01;for ex-citing cause,percentages of psychological factors such as tension and anxiety (29.3% vs.3.6%),staying up late (39.0% vs.2.8%)and heavy drinking (14.6% vs.4.7%)in young group were significantly higher than those of middle-aged group,while percentages of agitation (8.3% vs.0)in middle-aged group was significantly higher than that of young group (P<0.05 or <0.01).For CAG results,young group was mainly single vessel coronary disease (56.1%),which were mainly type A (53.7%)and type B1 (29.3%)lesions,while middle-aged group was mainly multi-vessel coronary disease (48.1%),which were mainly type B2 (29.7%)and type C (33.9%)lesions,and there were significant difference between two groups (P<0.05 or <0.01).Conclusion:Smoking,obesity and hy-perlipidemia are the most important risk factors for acute myocardial infarction;the major exciting causes are sta-ying up late,alcohol drinking and high mental stress etc.in young AMI patients;their coronary lesions are mainly single vessel coronary disease,and of type A and type B1 .
2.Prolongation of murine cardiac allograft survival by dendritic cells treated with NF-?B decoy

Objective To investigate the function of donor-derived dendritic cells (DCs) treated with NF-?B decoy in prolonging murine cardiac allograft survival time.Methods Donor bone marrow- derived DCs were treated with NF-?B decoy in vitro.Heterotopic abdominal heart transplantation was performed from BALB/c to C57BL/6 mice.Recipients were grouped according to different pretreat ments as follows:(1) Control group,infusion of PBS (0.2 ml) alone intravenously via the portal vein 7 days before heart transplantation;(2) CsA group,treated with sub-therapeutic CsA only for 7 days (10 mg?kg~(-1)?day~(-1)) through intraperitoneal injection after transplantation,and the same as control group before transplantation;(3) Control DCs group,infusion of only cultured 5th-day recipient DCs untreated with NF-?B ODN decoy;(4) Treated DCs group,infusion of recipient DCs pretreated with NF-~cB ODN decoy;(5) Combined treatment group,infusion of recipient DCs treated with NF-~cB ODN decoy before transplantation and intraperitoneal injection of sub-therapeutic CsA for 7 days (10 mg?kg~(-1)?day~(-1)) after transplantation;(6) Third party donor group,C3H/HeJ mice used as donor, and recipient (C57BL/6) was treated the same as combined treatment group.Every group had 8 mice and graft survival time was observed.Cytokines (IL-2,INF-?,IL-4 and IL-10) in recipient serums were analyzed by ELISA at 7th day after transplantation.Results The graft mean survival time (MST) in control group,CsA group,Control DCs group,treated DCs group,combined treatment group and third party donor group was 7 days,10.3 days,7.6 days,21.4 days,53.6 days and 9 days,respectively.There was significant difference in MST between treated DCs group and control group or control DCs group (P
3.Clinical application of the modified open-door laminoplasty perserving semispinalis cervicis insert into axis

[Objective]To verify whether or not the modified open-door laminoplasy preserving semispinalis cervicis insert into axis is effective in preventing postoperative axial symptom and maintaining postoperative cervical sagittal alignment.[Method]Since March 2002 to December 2003,forty-eight patients underwent modified open-door laminoplasy preserving semispinalis cervicis insert into Axis,among them,thirty-two patients who were followed up for more than 2 years were included in this study.Preoperative and postoperative JOA score,degree of axial symptom,ranges of neck motion,cervical curvature index were recorded and compared.[Result]Preoperative and postoperative JOA score was(9.04?2.9)and(12.5?3.1)respectively,with recovery rate as(44.9?26.9).There was significant difference between preoperative evident axil symptom rate(46.8%)and postoperative evident axil symptom rate(18.7%),while no significant difference between preoperative and postoperative ranges of neck motion,cervical curvature index was seen.[Conclusion]Modified open-door laminoplasy is less invasive to the posterior extensor mechanism especially the semispinalis cervicis than conventional open-door laminoplasy.This new procedure is effective in preventing postoperative morbidities often seen after conventional laminoplasy with adequate decompression of the spinal cord.

Beta-Elemene is an antitumor drug which is isolated from the traditional Chinese medicinal herb Curcumae Phaeocaulis Rhizoma, it is the main component of elemene which is extracted from the plant and delivered via blood circulation after intravenous injection. The antitumor effect of beta-elemene in vitro and in vivo was definite, and beta-elemene could improve the patient immunity and no sever side effect, drug resistance or bone marrow suppression were found during the clinical studies. And human serum albumin (HSA) is a primary extracellular protein which has a high concentration distribution in blood plasma and has many characteristic physiological functions. Therefore, the binding of beta-elemene to protein may be very important for absorption, distribution, metabolism and elimination. Therefore, the study on the interaction of beta-elemene with drug-carrying protein is very important. In this work, molecular binding of beta-elemene to human serum albumin (HSA) was investigated by using spectrofluorometer. the binding constants suggested that a strong interaction and the formation of a complex between beta-elemene and HSA. This clearly implies that beta-elemene can be stored and removed by the proteins in the body. Furthermore, the fluorescence quenching results showed that the HSA fluorescence was quenched by beta-elemene through static quenching mechanism. Thermodynamic parameters showed that hydrophobic interactions play a role in the binding of beta-elemene to HSA. The negative deltaH(0) and positive deltaS(0) in case of beta-elemene therefore showed that electrostatic attraction play a role in the binding of beta-elemene to HSA.

8.Closed reduction and three Kirschner pin fixation at the radial side for the treatment of supracondylar fracture in children with Gartland type II and type III.

OBJECTIVETo investigate the characteristics of supracondylar fracture of humerus in children and to explore the effect of closed reduction and internal fixation at radial side on the fracture.
METHODSThe children with fractures of Gartland type II and type III in our hospital from June 2010 to June 2013 were reviewed. There were 28 males and 7 females, ranging in age from 1 year and 1 month to 2 years and 6 months, with an average of 2 years and 1 month. According to Gartland classification, 19 cases were type II, 16 cases were type III. There were 3 patients with radial nerve injuries, and 5 patients with anterior interosseous nerve injuries. There were no vascular injuries. All the patients were treated with closed reduction and three Kirschner fixation at the radial side, followed by the plaster external fixation with elbowed flexion at 90 °. The X-ray examination was performed at the second day after operation. The joint function exercise began about at 2 to 3 weeks after operation when the plaster fixation was removed, and opportune time for removal of Kirschners depends on the situation of fracture healing. The operation time, nerve recovery, and the elbow joint function were observed.
RESULTSAll the children were followed up, and all the fractures had bony union. According to Flynn score system at the final follow-up, 28 patients got an excellent result, 4 good, 1 poor and 2 bad.
CONCLUSIONThree Kirschner fixation at the radial side for the treatment of supracondylar fracture of humerus has advantages of minimally invasive, shorter time of hospitalization, simple removal of the internal fixation and reliable therapeutic effects.

10.Biomechanical study on effect of upper cervical spine structural injury on stability of C1-C2 and C2-C3 segments

Objective To evaluate the effect of type Ⅱ odontoid fracture,type Ⅰ Hangman fracture,C2-C3 disc injury on stability of C1-C2 and C2-C3 segments and investigate the clinical significance.Methods Ten fresh-frozen cadaveric cervical specimens (5 men and 5 women; 25-45 years of age,mean 35.7 years) were selected to test the stability of C1-C2 and C2-C3 segments in the settings of intact condition (control group),type Ⅰ Hangman fracture,type Ⅱ odontoid fracture,type Ⅰ Hangman fracture + type Ⅱ odontoid fracture,type Ⅰ Hangman fracture + type Ⅱ odontoid fracture + C2-C3 disc injury.Range of motion (ROM) and neutral zone (NZ) of those segments were measured.Results Compared with the intact condition,type Ⅰ Hangman fracture produced no significant variations in C1-C2 ROM in all loading modes and C2-C3 ROM during left and right lateral bending; type Ⅱ odontoid fracture produced no significant variations in C2-C3 ROM in all loading modes and C1-C2 ROM during left and right rotation; type Ⅰ Hangman fracture + type Ⅱ odontoid fracture revealed no significant variations in C1-C2 ROM during left and right rotation and C2-C3 ROM during extention; type Ⅰ Hangman fracture + type Ⅱ odontoid fracture + C2-C3 disc injury produced no significant variations in C1-C2 ROM during left and right lateral bending and extension-flextion and C2-C3 ROM in all loading modes (P < 0.05).Conclusions Type Ⅰ Hangman fracture can reserve C1-C2 segmental left and right rotation and extension-flextion; type Ⅱ odontoid fracture can reserve C1-C2 segmental left and right lateral bending and extension-flextion; type Ⅰ Hangman fracture + type Ⅱ odontoid fracture + C2-C3 disc injury can reverse atlantoaxial rotationary stability and C1-C3 segmental stability in all directions.This study provides the biomechanical basis for clinical treatments and the related researches of internal fixation.
