OBJECTIVE To investigate the uses of antibacterials, the characteristics of bacterial distribution and the drug resistance and provide a foundation for reasonable application of antibacterials.METHODS Germs isolation and cultivation were carried out according to National Clinical Testing Operation Procedures(2nd edition).Germs definition and resistance test were carried out by using VITEK 2 Compact microbiological assay system.Statistical analysis was processed by using WHONET 5.4.RESULTS Totally 7815 strains of bacteria were isolated from clinical samples, most of which were Gram-negatives(G-)(58.0%,56.3% and 59.5%,respectively in the 3 years). The tendency of Gram-positives(G+) didnot charee obviously(about 20%).Theresistance were increasing. The resistance rate of Staphylococcus aureus was more than 70.0 % to the most antibacterials.Up to now, there was no vancomycin-resistant isolate.CONCLUSIONS We should use antibacterials reasonably and efficiently to delay and control drug resistance, according to the susceptibility test.

Objective To study the technique and clinical significance of percutaneous biopsy of transplanted liver guided by CT.Methods 19 transplanted liver were undergone 25 times of percutaneous biopsy and the pathomorphologic changes were demonstrated by HE staining. Results The successful rate of the percutaneous biopsy was 100% for all the 25 times of this procedure, including acute rejection on 9 episodes, preservation perfusion retrauma in 6,bile duct strictures in 4,drug-induced injury in 4,chronic rejection in 1 and acute hepatic necrosis in 1.Conclusions CT-guided percutaneous biopsy is an important method for diagnosing transplanted liver injury and providing great value for distinguishment of the causes for transplanted liver injury.(J Intervent Radiol,2007,16:855-857)

Objective To detect the expressions of Lingo-1 and myelin associated protein in the white matter and hippocampus of ovariectomized rats after short-term estrogen replacement therapy in order to explore the possible mechanisms for the effects of estrogen on the brain myelin sheaths and cognitive function. Methods 24 middleaged (9-12 months) female Sprague-Dawley (SD) rats were bilaterally ovariectomized (OVX) and randomly divided into vehicle replacement (OVX+Veh) group and estrogen replacement (OVX+E) group. After one month ERT,The spatial learning and memory ability of all rats were assessed with Morris water maze.Then,10 rats were randomly selected from each group. The ultrastucture of myelin sheaths in the cerebral white matter and hippocampus were observed, and the protein expression of MBP and Lingo-1 were investigated with Western blot and immunohistochemical staining. Results The escape latencies of OVX+E rats in navigation test were significantly shorter than that of OVX+Veh rats(P<0.05).The myelin sheaths in the white matter and hippocampus of OVX+Veh rats showed obviously degeneration. In the OVX+E group, the expression of MBP in the white matter and hippocampus was significantly higher than that of OVX+Veh group(P<0.05),however,the expression of Lingo-1 was significantly lower than that of OVX +Veh group (P< 0.05).Conclusions One-month ERT has significant beneficial effects on the spatial learning capacity and myelin sheaths in the white matter and hippocampus. The protective effects may be related to estrogen-induced downregulation of the Lingo-1 expression in the white matter and hippocampus of rats.

Objective To observe the serum γ-glutamyltransferase (γ-GGT) levels in patients with chronic hepatitis B virus (HBV) infection in different immune status and investigate their relationship with HBV DNA loads and ALT levels. Methods Blood samples were collected from 191 patients with chronic HBV infection in different immune status, including inactive HBV carrier state (group B,n = 55), immune tolerance phase (group C, n=47), HBeAg-negative CHB (group D, n =17), immune-reactive phase (group E, n=72) and 61 healthy individuals ( group A) for the detection of the serum γ-GGT, ALT level and HBV DNA loads. Results γ-GGT level were obviously higher in groups D and E than those in groups of A, B and C (P<0.01). Correlation analysis showed that the γ-GGT levels were positively correlated with serum ALT , AST levels in HBeAg-negative CHB and immune-reactive phase , but not correlated with HBV DNA loads. Conclusions The levels of γ-GGT are different during different immune status in patients with chronic HBV infection. The increased serum γ-GGT level may be an indicator for patients with chronic HBV infection entering immune active phase. The liver inflammation is the major impact factor to the γ-GGT levels.

Objective To analyze the clinical significance of modified thyroidectomy by trans-isthmus approach. Methods The clinical data of 5 751 patients who underwent thyroidectomy from Jan.1996 to Jan.2006 were retrospectively analyzed. All cases were divided into two groups. The modified operation group(n=3288) was treated by trans-isthmus thyroidectomy.Another group(n=2463) was treated by routine approach. The operation time, bleeding volume, and complications were observed and compared. Results In modified operation group, the mean operation time, bleeding volume, temporary injury of recurrent laryngeal nerve, tracheal discomfort, and hypocalcemia were significantly decreased than those in routine approach group(P

Facing a transformation of society and new problems of health care system in the twenty-first century, traditional medical modals have many challenges. The new modes of modern medicine have been formed. The medical education reform must adapt new medical modes to cultivate high quality medical talents. And at the same time the course of pediatrics should be constantly innovated to construct the first class teaching staff,first-rank teaching contents, teaching methods, teaching material and teaching administration.

Objective To investigate the diagnosis and the microsurgical treatment of intramedullary hemangioblastoma in cervical spinal cord. Methods The signs of MRI,and the results of operations were analysed in 26 patients with the tumors.Rusults The tumors can be classified into two types:Solid type (14 cases)and cystic type(12 eases). All the tumors underwent total removal and were all hemangioblastoma confirmed by histopathologic examinations.Postoperatively, neurological status were improved in 17 patients, remained in 7 cases and worse in 2 cases. Conclusion For intrameduUary hemangioblastoma of cervical spinal cord MRI is of significant importance in the diagnosis of localization and the nature of the tumors which is conductive to selecting appropriate operative methods. There is high risk in operating at cervical section, but microsurgical total resection is the optimal method to stop the development of the clinical presentation.Opera- tive methods varied with the different typer of the tumor. It is the most important principal that dissection is performed along the correct interface and the tumor should be removed en bloc after it is devascularized.
