1.Analysis of risk factors in elderly type 2 diabetes mellitus with cerebral infarction

Objective To explore the risk factors in elderly type 2 diabetes mellitus with cerebral infarction. Methods Retrospective investigarion was performed on 148 elderly hospitalized patients with type 2 diabetes.The patients were classified based on the presence or absence of cerebral infarction and compared with 60 controls.Logis- tic regression analysis was used to reveal the risk factors for cerebral infarction.Results The levels of systolic blood pressure(SBP),body mass index (BMI),fasting blood glucose (FBG),total cholesterol (TC),triglyceride (TG) and plasma fibrinogen(Fg) were higher in the patients with cerebral infarction[141.15?17.46)mmHg,(23.81?3.53)kg/m~2,(8.82?2.81)mmol/L,(5.69?1.15)mmol/L,(2.08?0.75)mmol/L and (4.08?0.65)g/L] than those without cerebral infarction[(129.78?14.65) mmHg,(22.18?3.22)kg/m~2,(7.06?1.72 )mmol/L,(5.09?1.12)mmol/L,(1.62?0.43)mmol/L and (3.48?0.58)g/L].The logistic analysis showed COUR,SBP,FBG, TC,TG and Fg were the independent risk factors for cerebral infarction.Conclusion Early intervention of the inde- pendent risk factors including SBP,FBG,TC,TG and Fg in elderly patients with type 2 diabetes was important for reduction and postponement of cerebral infarction.

3.Activin receptor-like kinase 1 and its roles in angiogenesis

Activin receptor-like kinase (ALK) 1 is a transforming growth factor-β/bone morphogenetic pro-teins superfamily type Ⅰ receptor, predominantly expressed in active endothelial cells. ALK1 has been shown to play a piv-otal role in regulating angiogenesis, which is involved in vascular formation during embryonic and early postnatal develop-ment and angiogenesis-related diseases, such as cardiovascular disorders and tumor. Understanding the exact function of ALK1 in angiogenesis will provide theoretical basis for anti-angiogenic strategy of ALK1 inhibition. In the present study, we briefly recapitulate ALK1 signaling pathway and its role in blood vessel formation and pathological neovascularization.
4.Expression of NFkappaB p65 and its target genes in gastric cancer and precancerous lesions.

OBJECTIVETo study the expression of NFkappaB p65 and its target genes in intestinal metaplasia (IM), dysplasia (Dys), gastric cancer (GC) infected with Helicobacter pylori (Hp) and explore the mechanism of infection by cytotoxin-associated antigen A expressing Hp (CagA(+)Hp) in the development of gastric cancer.
METHODSCagA antibody in blood sample of 289 patients was determined by ELISA. Hp was detected by rapid urease test and Warthin starry staining. Expression of NFkappaB p65 and its target genes in IM, Dys and GC was examined by immunohistochemistry.
RESULTSIn IMI approximately II, IMIII, DysI, DysII approximately III and GC, the expression of NFkappaB p65 was significantly higher in patients with CagA(+)Hp infection than those without CagA Hp infection. In IMIII and DysII approximately III, the expression of NFkappaB p65, c-myc, CyclinD(1) and bcl-xl was significantly higher in patients with CagA Hp infection than those without CagA Hp infection. In gastric cancer infected with CagA(+)Hp, the expression of NFkappaB p65, c-myc, CyclinD(1) and bcl-xl was significantly higher in intestinal type than in diffuse type.
CONCLUSIONThere are different mechanisms in intestinal type and diffuse type in the development of gastric cancer. The occurrence of intestinal type gastric cancer is associated with CagA(+)Hp infection which by NFkappaB p65 upregulating the expression of c-myc, CyclinD(1),bcl-xl in patients with IMIII, DysII approximately III. It may be an effective method to prevent gastric cancer by inhibiting NFkappaB p65.

5.Polymorphism of c-Ha-ras 3'VNTR in hydatidiform mole and its relationship with the development of postmolar tumor.

OBJECTIVETo study the polymorphism of c-Ha-ras 3' variable number of tandem repeat (VNTR) and its relationship with development of postmolar tumor.
METHODSThe cases of hydatidiform mole (HM) were retrospectively analyzed by amplified fragment length polymorphism-polyacrylamide gel electrophoresis. The DNA origin of HM was determined by comparison with the parents' DNA amplified results.
RESULTSAmong the samples from 15 cases, DNA from only paternal origin was found in 2 cases. DNA from both parents was in 13 cases, and of these 13 cases, 2 were found to be had balanced DNA origin, 11 had more DNA form paternal origin than DNA from maternal origin.
CONCLUSIONThe HM which has DNA from both parents origin and predominantly from paternal origin developed to postmolar tumor more frequently.

6.Mechanism of Dynamic Near-infrared Fluorescence Cholangiography of Extrahepatic Bile Ducts and Applications in Detecting Bile Duct Injuries Using Indocyanine Green in Animal Models

Fluorescence intraoperative cholangiography (IOC) is a potential alternative for identifying anatomical variation and preventing iatrogenic bile duct injuries by using the near-infrared probe indocyanine green (ICG).However,the dynamic process and mechanism of fluorescenceIOC have not been elucidated in previous publications.Herein,the optical properties of the complex of ICG and bile,dynamic fluorescence cholangiography and iatrogenic bile duct injuries were investigated.The emission spectrum of ICG in bile peaked at 844 nm and ICG had higher tissue penetration.Extrahepatic bile ducts could fluoresce 2 min after intravenous injection,and the fluorescence intensity reached a peak at 8 min.Inaddition,biliary dynamics were observed owing to ICG excretion from the bile ducts into the duodenum.Quantitative analysis indicated that ICG-guided fluorescence IOC possessed a high signal to noise ratio compared to the surrounding peripheral tissue and the portal vein.Fluorescence IOC was based on rapid uptake of circulating ICG in plasma by hepatic cells,excretion of ICG into the bile and then its interaction with protein molecules in the bile.Moreover,fluorescence IOC was sensitive to detect bile duct ligation and acute bile duct perforation using ICG in rat models.All of the results indicated that fluorescence IOC using ICG is a valid alternative for the cholangiography of extrahepatic bile ducts and has potential for measurement of biliary dynamics.
7.Primary nasal clear cell carcinoma:a case report and literature review.

OBJECTIVE:
To explore the clinical and pathological characteristics of the primary nasal-sinonasal clear cell carcinoma, and then summarize the diagnosis and treatment.
METHOD:
The clinical, imaging and pathological data were analyzed in one patient with primary nasal-sinonasal clear cell carcinoma. The domestic and foreign related literatures from 1992 were reviewed and summarized.
RESULT:
In 12 cases of primary nasal-sinonasal clear cell carcinoma, 8 cases (66.7%) mainly presented with nasal bleeding, 4 cases (33.3%) with destroyed bone. CT scan showed the sheet shadow similar to soft tissue density in the nasal cavity and paranasal sinuses. Three cases (25.0%) were only treated by surgical resection, 7 cases (58.3%) were treated by surgical resection combined with radiotherapy and chemotherapy, 2 cases (16.7%) were treated by radiotherapy alone. During the 6 months to 10 years follow up, only one patient died of lung metastasis.
CONCLUSION
Primary nasal-sinonasal clear cell carcinoma is rare. Nasal bleeding is commonly one of the early symptoms. It must be differentiated from metastatic clear cell carcinoma and a variety of salivary gland tumors with clear cell in pathology. Surgery combined with radiotherapy and chemotherapy is effective treatment. Short-term prognosis is good. Endoscopic surgery can be used to the tumor with a base limited in the nasal cavity and sinuses in early time. The regular check after surgery should be taken to the patients every half a year.

8.Evaluation of bone marrow biopsy and smear examinations in differential diagnosis for patients with pancytopenia by simultaneously compartive study.

To simultaneously compare and evaluate the examinations of bone marrow smear and trephine biopsy in differential diagnosis and therapeutical effect of pancytopenia patients, the differences between bone marrow smear and trephine biopsy in degree of cellularity, misdiagnosis rate and therapeutical effect for 71 patients with pancytopenia were analyzed. The results showed that the degree of cellularity in bone marrow biopsy for cytologic morphology was higher than that from the smear, but misdiagnosis rate in the biopsy was lower than that in the smear. In conclusion, bone marrow biopsy is necessary to the differential diagnosis and more valuable for evaluation of therapeutical effect and prognosis of pancytopenia patients.

9.Analysis of urinary iodine level of children aged 8-10 years in Yunyang and Bishan County of Chongqing in 2007

Objective To determine the urinary iodine level of people in Yunyang and Bishan County of Chongqing and explore into its influencing factors. Methods Using multistage cluster stratified simple random sample method, Yunyang and Bishan County were chosen as research spots, then thirty children aged 8-10 in each 3 primary school of the 2 counties were selected using stratified randomization sampling method to inspected their urine and household salt for iodine and the iodine content in drinking water. Results Five hundred and seventy-one urine samples were inspected and the urinary iodine median was 261.47 μg/L. 5.78% (33/571) and 37.48%(214/571) of samples had an urinary iodine median less than 100 μg/L and more than 300 μg/L. The urinary iodine median of Yunyang County was higher than that of Bishan (H = 7.42, P < 0.01). The iodine salt coverage rate, the qualified rate and edible qualified iodine salt rate respectively were 99.64%(554/556), 94.22% (522/554) and 93.88% (522/556) in 556 samples of family table salt. Eighty-seven samples of drinking water were inspected, resulting an averaged iodine content of 8.81 and 2.97 μg/L, respectively in the 2 counties. Conclusions The 2 counties are all the area of iodine deficiency. The urinary iodine level, although meeting the demand of eliminating iodine deficiency diseases, is a little bit higher given that iodized salt of present doage has been taken for a long time. The content of iodized salt should be adjusted accordingly.
10.Clinical trial of dexamethasone combined with methimazole in the treatment of patients with chronic autoimmune thyroiditis

Objective To investigate the clinical efficacy and safety profile of dexamethasone combined with methimazole in patients with chronic autoimmune thyroiditis.Methods A total of 36 patients with chronic autoimmune thyroiditis were randomly divided into treatment group (n =18) and control group (n =18).The control group received oral administration of methimazole 10 mg,tid.The treatment group were given dexamethasone injection 5 mg at day 1,8,15 and 22,on the basis of the control group.Both groups were treated for 28 d.After treatment,the clinical efficacy,the levels of serum T lymphocyte,tumor necrosis factor-α (TNF-α),thyroid stimulating hormone (TSH),free triiodothyronine (FT3),free thyroxine (FT4),antithyroperoxidase antibody (TPOAb),antithyroglobulin antibody (TGAb) and the adverse drug reactions were compared between the two groups.Results The total effective rates of the treatment group and the control group were 94.44% (17 cases/18 cases) and 66.67% (12 cases/18 cases),respectively,and the difference was statistically significant (P < 0.05).After treatment,levels of serum CD4 +/CD8 + were 1.31 ± 0.07,1.00 ± 0.08 in treatment group and control group.The levels of TNF-α were (1.57 ± 0.16),(1.92 ± 0.16) ng · mL-1.The levels of FT3 were(2.85 ±0.78),(5.19 ± 1.04) pmol · L-1.The levels of FT4 were(13.08 ± 1.59),(18.96 ±2.21)pmol · L-1.The levels of TSH were(2.88 ±0.60),(3.85 ±0.81) U · mL-1.The levels of TPOAb were(22.92 ± 3.30),(27.43 ±4.45) U · mL-1.The levels of TGAb were (74.30 ± 11.22),(92.74 ± 13.46) U · mL-1,respectively.All the parameters between the two groups were statistically significant (P < 0.05).The adverse drug reactions in patients of the treatment group were agitation,nausea and vomiting,and the incidence of adverse drug reactions was 11.11% (2 cases/18 cases).The adverse drug reactions in patients of the control group were dizziness headache,nausea and vomiting,hypogeusia,and the incidence of adverse drug reaction was 22.22% (4 cases/18 cases).There was no statistically significant differences in the incidence of adverse drug reactions between the two groups (P > 0.05).Conclusion Dexamethasone combined with sulfhydryl imidazole exerted satisfying clinical efficacy and high safety profile on the treatment of chronic autoimmune thyroiditis.
