1.A Clinical Study on the Acetabular Fracture

Acetabular fracture is severe injury and is associated with other body injuries severely. It's complications are traumatic arthritis and avascular necrosis of femoral head which induce severe permanent disability and its treatment is equivocal. A clinical study was performed on 70 patients with 65 fresh fractures and 5 old fractures of the acetabulum, who were hospitalized and treated at Severance Hospital from January 1971 to December 1980 and following results were obtained. 1. The prevalent age distribution was between 20 and 50 years of age (75.7%), and the ratio between males and females was 2.2:1. The most common cause of injury was traffic accident (70.%). 2. 56 patients (80.0%) were associated injuries of other parts and the most common associated fracture was pelvic bone fracture and the most common associated soft tissue injury was urological injury. 3. A classification of acetabular fracture was induced from Judet and Letournel's anatomical classification; simple fracture was 45 cases (64.2%), associated fracture was 25 cases (35.8%) and most common hip dislocation was posterior dislocation (31.4%). 4. The methods of treatment were; closed management by using traction for 53 patients, surgical management by open reduction and internal fixation for 12 patients, and hip reconstructive surgery by total hip replacement(2 patients), cup arthroplasty (2 patients), acetabular roof formation by using iliac bone graft (1 patient). 5. The satisfactory result of conservative treatment was (57.8%), surgical treatment was (72.7%), hip reconstructive treatment was (80.0%). 6. The complications of the acetabular fracture were traumatic arthritis (36.1%), avascular necrosis (23.0%), myositis ossificans (6.6%), infection (3.3%). 7. Finally, the results of treatment depend largely on anatomical reduction, early joint motion.

4.Acute Pulmonary Edema Occurred During the Cesarean Section in a Patient with Ritodrine Pretreatment: A case report.

The mechanism of development of pulmonary edema associated with -sympathomimetic agent is still not fully understood. 2-sympathomimetic drugs such as ritodrine, terbutaline, and fenoterol are widely used for the treatment of premature uterine contraction. We describe a case of pulmonary edema that occurred suddenly during Cesarean section in a previously healthy parturient, associated with ritodrine pretreatment for 5 days in an attempt to arrest preterm labor. Pulmonary edema was completely treated with mechanical ventilation and diuretic therapy.

5.Thoracic Outlet Syndrome

Thoracic outlet syndrome is a collective term embracing previously described syndromes such as scalenus anticus, cervical rib, costoclavicular, hyperabduction and shoulder girdle compression syndromes. Its symptoms and signs are due to bony and soft tissue compression of the neurovascular bundle at the thoracic outlet. It is the purpose of this paper to evaluate the results of experience in treating the patient with thoracie outlet syndrome by surgical means. In this study, 3 cases: cervical rib (1 case), excessive callus formation after clavicular fracture (1 case), and hyperabduction syndrome with combined scalenus anticus syndrome (1 case), which had developed thoracic outlet syndrome were treated at Severance Hospital and gratifying results were obtained.

6.TCVO ) in the Extremities: Report of 4 Cases

Angiographic procedures and their applications maybe divided into following roups: I) procedures used to increased blood folw, as the angioplasty, II) procedures used to decreased blood flow, as the embolization and local infusion of vasoconstrictors and III) miscellaneous procedures. Transcatheter vessel occlusion was first performed in 1930 by Brook to occlude the carotid cavernous fistula by embolization of the surgically exposed carotid artery with muscle marked with a sliver clip injected directly into the internal carotid artery. Autologous clot, first used by Doppman(1968) to occlude a spinal arteriovenous malformation was quickly adopted use in traumatic hemorrhage with pelvic fracture, nonoperable aneurysm, gastrointerstinal hemorrhage and diagnosis and treatment of the tumors. In Kores, there have been several reports of transcatheter vessel occlusion after Han(1978) made a first report in the radiology part. In orthopedics part of Korea, Kim(1981) and Yoo(1982) reported the cases of surgical treatments of arteriovenous malformation without embolization. Also, in Korea, there has been no report of transcather embolizations of vascular diseases of the extremities, such as huge arterial aneurysm and dilated varicose vein. We reported the 4 cases who were treated at the Chonbuk National University Hospital between January 1987 and April 1988. Transcatheter vessel occlusion have such advantages : 1. The vascular status of the mass can be recognized prior to operation, such as biopsy. 2. Since the bleeding can be reduce, the operation can be performed easily. 3. Nonoperable huge arterial aneurysm could sometimes be treated only with transcatheter vessel occlusion adequately. 4. It can make a differential diagnosis with other vascular anomaly easy.

7.Effect of bromocriptine on 6-hydroxydopamine-induced lipid peroxidation and cytotoxicity in vitro and in vivo.

The present study was to evaluate the protective effects of bromocriptine, which is known as D2 dopamine receptor agonist and used for the treatment of patients with Parkinson's disease (PD), on 6-hydroxydopamine (6-OHDA)-induced neurotoxicity in vitro and in vivo. Lipid peroxidation product (malondialdehyde; MDA) produced by the administration of 6-OHDA was profoundly reduced following the treatment of bromocriptine in a dose-dependent manner in rabbit brain homogenate. Quinone formation by 6-OHDA autoxidation was also attenuated, and its effect was as potent as other antioxidants. Pretreatment of bromocriptine reduced the cytotoxicity of 6-OHDA on SH-SY5Y neuroblastoma cell lines dose-dependently. The loss of striatal dopamine and its metabolite, DOPAC (dihydroxyphenylacetic acid) as well as increase of MDA production caused by intrastriatal injection of 6-OHDA was significantly recovered following the treatment of bromocriptine. The present study clearly showed that bromocriptine had a protective action against 6-OHDA-induced neurotoxicity. These results suggest that bromocriptine has the antioxidant properties, which could be another advantage for delaying the progress of Parkinson's disease.

8.Electron Microscopic Study on Differentiation of Tracheal Epithelium in Human Fetus.

The human trachea is normally lined by a pseudostratified ciliated columnar epithelium where ciliated, goblet, intermediate and basal cells are mainly represented. However the fetal tracheal epithelium was found to be composed of ciliated, non-ciliated and basal cells. The present study was designed to characterize the development of ciliated cells in the fetal trachea at mid (19 weeks) and last (32 weeks) trimester of gestation. At 19 weeks of gestation, the tracheal epithelium, 35 µm in height, was composed of surface, intermediate and basal layers. The surface cells were subdivided into ciliated, immature ciliated, non-ciliated, granule containing and goblet cells. The ciliated cells covered approximately half of the luminal surface area. The immature ciliated cells contained basal bodies, but the apical membrane was not invested with cilia. The granule containing cells contained numerous dense granules, 0.3-0.7 µm in diameter, in the apical cytoplasm. The goblet cells contained less electron dense granules, 1-2 µm in diameter, in the apical cytoplasm. The cells in intermediate layer were relatively undifferentiated and contained poorly developed organelles. Submucosal gland were well differentiated and were composed of the mucous and serous cells. At 32 weeks of gestation, the tracheal epithelium, 50µm in height, was also composed of surface, intermediate and basal layer. The surface cells were composed of ciliated, non-ciliated and goblet cells. The ciliated cells, dominant type of surface cells, were subdivided into mitochondria-rich cells (type I) and smooth endoplasmic reticulum-rich cells (type II). The non-ciliated cell were of three subtypes : mitochondria-rich cells (type A), glycogen and microfilament-containing cells (type B) and cells with bulging apical surface into the lumen (type C). Small granule containing cell appeared in the basal layer. These cells contained clear vesicles, 50 ㎚m in diameter, and dense granules, 100-300 ㎚m in diameter. Submucosal gland were well developed and consisting of mucous, serous and myoepithelial cells. These results indicate that the cell populations of the tracheal epithelium at late stage of pregnancy have essential features similar to those of adult. and show that the different steps of ciliogenesis could be identified.

9.Effects of Intermittent Sequential Pneumatic Compression on Coagulation and Fibrinolysis in Multiple Trauma.

BACKGROUND: After multiple trauma, blood coagulation activity is enhanced and fibrinolytic activity is suppressed by overproduction of plasminogen activator inhibitor-1 (PAI-1). Intermittent sequential pneumatic compression (ISPC) is an effective method to prevent deep vein thrombosis. Its action is explained by the mechanical effect on blood flow, as well as by the enhancement of fibrinolysis by the reduction of PAI-1. The aim of this study was to determine the effects of ISPC on coagulation and fibrinolysis after multiple trauma. METHODS: Thirty-nine trauma patients were either treated with ISPC (ISPC group, 20 patients) or without ISPC (control group, 19 patients). We measured the plasma levels of the thrombin antithrombin III complex (TAT), the plasmin alpha 2 plasmin inhibitor complex (PIC), the tissue plasminogen activator (t-PA), and the plasminogen activator inhibitor-1 (PAI-1) on admission and at 1, 2, 3, 6, 12, and 24 hours after admission. RESULTS: The TAT was higher than normal in both groups, with no significant difference between the two groups throughout the study period. The PIC level of ISPC group was significantly higher than that of the control group. In the ISPC group, the PIC level increased gradually, reaching a peak at 3 hours and decreasing thereafter. In the control group, the PIC level increased to a peak level at 2 hours. The TAT/PIC ratio dropped in the first two hours and increased at 3 hours, dropping again thereafter. In the ISPC group, the ratio dropped gradually without an intermittent fluctuation. At 3 and 6 hours, the control group showed a significantly greater ratio compared to the ISPC group. PAI-1 was higher than normal in bothgroups, with a significantly lower level in the ISPC group from 2 hours to 24 hours. For the t-PA level, no difference was noted between the two groups, with the peak level occurring at 1 hour. The PAI-1/t-PA ratio was significantly greater in the control group from 2 hours to 12 hours than in the ISPC group, but the difference was not significant at 24 hours. CONCLUSIONS: In multiple trauma patients, ISPC does not seem to affect coagulation, but enhances fibrinolysis through suppressed PAI-1 production. This effect of ISPC may be maintained for 12 hours.

10.Ultrasonography of thyroid masses

Ultrasound studies of sixty five patients with thyroid mass histologically proven at KyungPook NationalUniversity Hospital from June 1983 to May were retrospectively analized. No specific sonographic characteristicsof thyroid mass lesions was found on the basis of histological diagnosis. Authors consider that ultrasound studymight be mere useful for detection and localization of a lesion, and guide to needle aspiration, monitoring thesize of a mass following therapy as well.
