1.Correlation between Squamous Papilloma and Human Papillomavirus in Esophagus.

BACKGROUND/AIMS: Esophageal squamous papillorma is a rare benign lesion. This lesion is generally asymptomatic and solitary. Resent studies reveal the presence of human papil- lomavirus in certain cases of esophageal squamous papilloma, and human papillomavirus appears to be pathogenetically related to the esophageal squamous papilloma. The aim of this study was to evaluate the presence of human papillomavirus in esoplmgeal spuamous papilloma. METHODS: In situ hybridizations and direct in situ polymerase chain reactions for human papillomavirus type 6/11 were performed on 16 paraffin embedded tissues with esophageal squamous papilloma. RESULTS: Human papillomavirus type 6/11 was detected in 1 of 16 esophageal squamous papillomas on direct in situ polymerase chain reaction. All lesions were solitary, and 12 of 16 cases were located in distal one-third of the esophagus. Age range was 23-71 years (mean, 41 years). CONCLUSION: These results indicate that causal relation between human papillomavirus type 6/11 and esophageal squamous papilloma is inconspicuous.

2.Comparison of Cell Proliferation between Chronic Ulcerative Colitisand Acute Self-limited Colitis.

PURPOSE: The ulcerative colitis is a major chronic intestinal disease of unknown etiology affecting principally the rectum and left colon, and its incidence is markedly increasing in Korea. The main differential diagnosis of ulcerative colitis is an acute self-limited colitis. This study was performed to evaluate difference of cell kinetics between chronic ulcerative colitis and acute self-limited colitis. METHODS: The normal colon (n=25), acute self-limited colitis (n=25) and chronic ulcerative colitis (n=25) were investigated by using Ki-67 immunohistochemical staining for proliferation and TUNEL method for apoptosis. The Ki-67 labeling indices and TUNEL labeling indices were determined. RESULTS: The means of Ki-67 labeling indices in normal colon, acute self-limited colitis and chronic ulcerative colitis were 5.14 5.25%, 6.81 5.73%, 13.10 10.15%, respectively. And the means of TUNEL labeling indices in normal colon, acute self-limited colitis and chronic ulcerative colitis were 1.59 0.10%, 2.54 1.60%, 2.51 1.40%, respectively. CONCLUSIONS: The apoptosis is one of method of cell loss in both acute self-limited colitis and chronic ulcerative colitis. High proliferative activity of chronic ulcerative colitis may predispose to mutational events in colonic mucosa, therefore may be one of the increased cancer risk factors in chronic ulcerative colitis.

3.A Clinical Review of Ectopic Pregnancy.

This study was undertaken for the clinical evaluation and statistical analysis on the 88 women with histopathologically confirmed ectopic pregnancies who were admitted and treated from grand opening on May-28, 1983 to Sept.-30, 1985. Incidence of ectopic pregnancy was 1 in 22.2 deliveries and the most common age group was in 30~34 years of age. There was tendency of decreasing incidences as the gravidity, parity or artificial abortion were over 3. The most common etiologic factor was pelvic inflammatory disease and procedures for family planning were the next common. Common symptoms were lower abdominal pain (85.2%) and vaginal spotting (56.8%). At the admission, hemoglobin level under 10gm% were 44.3%. 69.5% of urine HCG tests were positive. Culdocentesis was positive only in 75.9%. Termination was frequent at 6~7 weeks of gestation mostly with rupture or abortion. Free blood in the abdominal cavity was averaged 1,224ml. 4 cases of clinicobiochemically suspected ectopic pregnancies with spontaneous regression were excluded, and there was no fatal case treated in hospital.

4.Role of Hydroxyl Radicals and Lipid Peroxidation in Cisplatin-induced Acute Renal Failure in Rabbits.

PURPOSE: This study was undertaken to determine whether lipid peroxidation induced by hydroxyl radicals play a critical role in cisplatin(cis-diamminedichloroplatinum II)-induced acute renal failure. METHODS: Animals received cisplatin at a single i.p. dose of 5 mg/kg, and changes in renal function were measured at 48 hr after cisplatin injection. RESULTS: Cisplatin caused an increase in serum creatinine level, which was accompanied by reduction in GFR. The fractional excretion of Na(+), glucose, and inorganic phosphate was increased in animals treated with cisplatin alone. Cisplatin treatment in vivo inhibited PAH uptake by renal cortical slices and Na(+)-K(+)-ATPase activity in microsomal fraction. Lipid peroxidation was increased in cisplatin-treated kidneys. When animals received the antioxidant N,N'-diphenyl-p-phenylenediamine(DPPD), the iron chelator deferoxamine, and hydroxyl radical scavengers dimethylthiourea and sodium benzoate before cisplatin injection, alterations in renal function and lipid peroxidation induced by cisplatin were significantly prevented. Exposure of renal cortical slices to cisplatin in vitro caused an increase in LDH release and lipid peroxidation, which were completely prevented by DPPD and deferoxamine. By contrast, hydroxyl radical scavengers(dimethylthiourea and thiourea) did not prevent cisplatin-induced LDH release despite they inhibited cisplatin-induced lipid peroxidation. CONCLUSION: These results suggest that the lipid peroxidation resulting from generation of hydroxyl radicals may play a role in cisplatin-induced acute renal failure. In addition, the protective effects of hydroxyl radical scavengers in vivo studies are different from the results obtained from in vitro studies using renal cortical slices.

5.A Case of Gastric Schwannoma.

Most submucosal tumors of the stomach are of mesenchymal origin. Gastric schwannoma, which is a subset of mesenchymal tumors, is a rare tumor taking origin from Schwann's cells. A 61-year-old woman whose endoscopy showed a well circumscribed submucosal mass measuring 2.5 cm on the midbody of the stomach. Endosonographically, the tumor was well circumscribed, low echoic submucosal mass with cystic lesion in the 4th layer of the gastric wall. The patient underwent wedge resection. Microscopically, the cells were made up of irregular fasciculating bundles of spindle cells featured with benign nuclear atypia and peripheral lymphoid cell cuffing, involving muscularis propria. Immunohistochemical staining showed positivity for S-100 protein and the neuron-specific enolase, but were negative to CD 34, desmin and smooth muscle actin. From these findings, this tumor was diagnosed as a schwannoma.

6.A Clinical Analysis of Gastric Candidiasis.

BACKGROUND AND AIMS: Gastric candidiasis is an uncommon disorder. But in recent years this diagnosis has increased as a result of the greater use of broad-spectrum antibiotics, immunosuppressive and chemotherapeutic agents or of the use of drugs that reduce the gastric acid production, such as H2 receptor antagonists. Our aim of the present study was to investigate clinical characteristics of gastric candidiasis including endoscopic findings and histopathology. METHODS: We reviewed 28 cases of gastric candidiasis who have under-went endoscopic biopsy at Wonkwang University Hospital from January 1995 to eptember 1996. RESULTS: 1) The prevalence rate of gastric candidiasis was 0.8% (28/3400). 2) In benign and malignant gastric ulcer patients, occurrence of other clinical findings were cardio-vascular disease in 5 cases, diabetes mellitus in 3 cases, hepatobiliary disease in 5 cases, bronchopulmonary disease in 10 cases, renal disease in 1 case, immunosuppressive therapy in 1 case, peptic ulcer therapy in 4 cases, and nongastric neoplasm in 1 case. 3) Locations of benign and malignant gastric ulcers in patients with candidial infection were prepyloric area in 8 cases, angle in 7 cases, body in 11 cases, cardia and fundus in 2 cases. 4) Sizes of ulcer cases were almost > or =2 cm in benign ulcer (76.5%), all cases were > or =3 cm in malignant ulcer. 5) According to endoscopic criteria, thrush type were in 22 cases, ulcerated type in 6 cases and depth of candidial infection in ulcer base were suppurative type in 18 cases, fibrinoid type in 10 cases. 6) After 6 weeks of therapy, endoscopic and clinical cure occurred in 60% (3/5) of patients treated by H2 blocker, antacid, and occurred in 100% (5/5) of patients treated by H2 blocker, antacid and antifungal agent. CONCLUSIONS: These results suggest that gastric candidiasis has a large and dirty ulcer base, white or green-white membrane and spread over inflammed area.

7.Mesocolon Laceration Following Colonoscopy.

Colonoscopic examination is a safe procedure, however, unexpected complications can sometimes occur. Bleeding and perforation of the colon have been reported as the most common complications. Hemoperitoneum after colonoscopy is an unusual complication, but it may be catastrophic. We report on a 20-year-old man who experienced left low quadrant pain after undergoing colonoscopy. Hemoperitoneum was diagnosed using abdominal CT. A laparoscopic exploration was urgently performed, revealing a lacerated mesocolon of the descending colon. Bleeding of the injured site was controlled without complication. The patient recovered fully without signs of recurrent bleeding. This report implies that if the patient has persistent abdominal pain after undergoing colonoscopy, we should consider hemoperitoneum as one of the causes. To the best of our knowledge, no case of isolated laceration of the mesocolon of the descending colon after colonoscopy has been reported.

Patients with LFP constitute a special group with clinical, endoscopic, and histological features unrelated to other types of inflammatory bowel diseases, and have been reported to be refractory to local steroid and/or oral sulfasalazine therapy. The aim of this study was to clarify whether mesalazine suppositories have a therapeutic effect in LFP. METHODS: The histologic slides of 8 cases indexed in our pathology files as "lymphoid follicular proctitis of the rectal mucosa" from January 2001 to November 2003 were reviewed retrospectively. RESULTS: The most common symptom in the patients with LFP was rectal bleeding. The endoscopic mucosal changes were discontinuous, sparing whole circumferential involvement, and were strictly confined to the rectum. Average period of medication was 12 months. All the symptomatic patients with LFP responded to mesalazine suppository therapy. In addition, these patients did not progress to other disease including ulcerative proctitis or lymphoma. CONCLUSIONS: Mesalazine suppository treatment is a useful therapeutic option for symptomatic patients with LFP.

10.A Case of Menetrier's Disease.

Menetrier's disease is a form of hypertrophic gastropathy occurring primarily in middle-aged males. Patients generally present clinically with dyspepsia and, on occasion, with hypoproteinemic edema and anemia. The latter feature, when combined with the radiographic appearance of the stomach in Menetrier's disease, can lend to confusion with carcinoma and malignant lymphoma. A 45-year-old male was admitted with chief complaint of epigastric dscomfort. Endoscopic examination revealed the appearance of enlarged gastric folds involving the body and fundus of the stomach with relative sparing of the antrum and endoscopic ultrasonography (EUS) reveals characteristic thickening with enhanced echogenicity of the second layer. Biopsies revealed foveolar hyperplasia with cystic dilatation of the glands, compatible with Menetrier's disease. We report a case of Menetrier's disease with brief review of literatures.
