1.The Effect of Functional Electrical Stimulation(FES) - Biofeedback on Sexual Activity and Quality of Life in Female Stress Urinary Incontinence.

PURPOSE: Pelvic floor muscles play an important role in female sexual function. Urinary incontinence is one of the complex symptoms of pelvic floor relaxation. The purpose of the present study was to evaluate the effects of behavioral treatment on the sexual life variables and quality of life in stress urinary incontinent women. MATERIALS AND METHODS: Fifty women with clinically and urodynamically proven stress urinary incontinence were randomly selected, and treated with functional electrical stimulation (FES) -Biofeedback. The FES-Biofeedback treatment was performed for 20 min per session, 2 sessions a week, for 6 weeks. The treatment consisted of electrical stimulation (35Hz and 50Hz, simultaneously) for 24 seconds, and the biofeedback, composed of 3 phases of contraction, lasted for 32 seconds. The outcome measures on the sexual life variables and quality of life scales were assessed by the Bristol Female Lower Urinary Tract Symptoms questionnaire and a self-developed questionnaire, based on the Brief index of sexual Functioning for women (BISF-W). RESULTS: The general quality of life scores were significantly improved after 6 weeks of FES-Biofeedback (p<0.05). The quality of orgasms, sexual interest, sexual activity and sexual anxiety were significantly improved after the FES-Biofeedback (p<0.05). Changes in the scores for the degree of satisfaction in those variables also showed statistical significance (p<0.05). CONCLUSIONS: Women with incontinence were dissatisfied with their sexual life due to various symptoms associated with their urinary symptoms or symptoms that result from pelvic floor relaxation. The FES-Biofeedback therapy showed positive effects on improving, not only stress incontinence, but also sexual life variables and the life quality.

3.Mycosis Fungoides Treated with PUVA.

We report a case of mycosis fungoides in a 37 year-old male patient who had relatively well defined, irregular, erythematous plaques covered with acales on the buttock, lower extremities, anterior chest and back. Histopathological findings showed epidermotropism and Pautrier microabscesses in the epidermis and infiltration of atypical lymphocytes in the upper dermis. Electron microscopic findings showed mycosis cells with convoluted nuclei and prominent nucleolei He had been received PUVA therapy combined with topical and systemic corticosteroid for 4 months. Skin lesions were improved markedly after treatment with total 262 joules/crn2 and then maintained with 14 joules/cm2 in every week.

5.A Case of Idiopathic Collapsing Glomerulopathy Showing Aggravation on a Chronic Progressive Course.

Collapsing glomerulopathy (CG) has become an important cause of end-stage renal disease (ESRD). First delineated from other proteinuric glomerular lesions in the 1980s, CG is now recognized as a common, distinct pattern of proliferative parenchymal injury that portends a rapid loss of renal function and poor responses to empirical therapy. The first cases in the literature trace back to human-immunodeficiency-virus (HIV)-negative patients who underwent biopsy in 1979. A 45-year-old male patient complained of hematuria and proteinuria eight years ago. He showed an abrupt serum creatinine increase from 1.75 to 2.65mg/dL in the last preceding months. Afterwards, his serum creatinine progressively increased up to 6.82mg/dL. Moreover, his 24 h urine protein level was determined to have reached 6,171 mg/day, as opposed to 670 mg/day a year earlier. Consequently, renal biopsy was performed, and its result showed collapsing glomerulopathy, compatible with the diagnosis. He has undergone continuous ambulatory peritoneal dialysis as renal replacement therapy. Thus, it is reported herein that a patient clinically diagnosed with chronic kidney disease eight years ago showed a sudden renal-function decrease and was clinicopathologically diagnosed with collapsing glomerulopathy based on the results of his renal biopsy.

6.Dysmenorrhea and Premenstrual Syndrome of School Girls in Ansan.

PURPOSE: Menstrual disorders are very common problems in adolescence. Most of them are minor, including mild dysmenorrhea, premenstrual syndrome(PMS) and minor variations in cycle, length, or amount of flow. Dysmenorrhea is the most prevalent among them while PMS is a nebulous condition characterized by a broad spectrum of symptoms. The purpose of this study is to investigate the correlation between dysmenorrhea and PMS in adolescents. METHODS: This cross sectional study on dysmenorrhea and PMS was performed in 1153 junior high school girls in Ansan. We researched the clinical aspects of PMS and investigated the correlation between menstrual characteristics and PMS in adolescents. RESULTS: The results are as follows : The incidence of PMS is 66.3%. The correlation of menstrual characteristics and severity of PMS was as follows : intensity of menstrual flow(P<0.01), duration of menstrual flow(P<0.05). The correlation of severity between dysmenorrhea and PMS is found(P<0.01). CONCLUSION: Korean adolescents have many problems related to dysmenorrhea and PMS. This study indicates that Korean adolescents need to be educated on the matter and treatment of their menstrual disorders are also needed.

7.Clinical Report of Effects of Pre and Post-partum Thyroiditis (PPT).

BACKGROUND: Excessive iodine intake increases the occurrence of autoimmune thyroid disorders by enhancing immunogenecity of iodine-rich thyroglobulin, In Korea, most of postpartum women take a large amount of iodine-rich seaweed. Although the excessive iodine intake may affect the thyroid function, only a few reports were available concering iodine intake, especially on postpartum period. METHODS: A prospective study was undertaken in 146 of normal delivered postpartum women. Dietary intake and urinary excretion of iodine, serum T3, T4, TSH, anti-TPO Ab and anti-Tg Ab were measured before and 1, 6, 12 and 24 weeks after delivery. Iodine intake was analyzed by one-to-one interview using 24hr recall and food frequency questionnaire. RESULTS: 1. PPT was occurred in 6 (10.3%) postparturn women, It presented as hypothyroidism alone in 1 (16.7%), transient thyrotoxicosis followed by hypothyroidisrn in 3 (50.0%), and thyrotoxicosis alone in 2 (33.3%) of the follwed-up patients. 2. During pregnancy, no difference was found in age, serum T3, T4 and TSH between PPT and normal thyroid function group. 3. In PPT group, anti-TPO and anti-Tg Ab were significantly higher than those of normal thyroid function group during pregnancy, and their sensitivity for PPT was 40% and 33%, respectively. But there was no correlation between dietary iodine intake and the titer of thyroid auto-antibodies. 4. There was no correlation between pre and post-partum dietary iodine intake and occurrence of PPT. CONCLUSION: In Korea, the incidence of PPT was slightly higher than other nations. The sensitivity of thyroid auto-antibodies was too low to use for prediction of PPT. Pre and post-partum iodine intake had no effect on the occurrence of PPT and post-partum thyroid function.

8.A Case of Mesalazine-induced Eosinophilic Pneumonia in a Patient with Ulcerative Colitis.

Measlazine(5-Aminosalicylic acid, 5-ASA), a component of Sulfasalazine (sulfapyridine bound to 5-ASA), is used to treat inflammatory bowel disease. Most adverse pulmonary effects caused by sulfasalazine have been attributed to sulfapyridine. However, lung toxicity associated with measlazine(5-ASA) is rare. Here we report a case of eosinophilic pneumonia in a 44-year-old woman with ulcerative colitis, who was treated with mesalazine. She developed symptoms of a dry cough, mild night fever, and evertional dyspnea. Bilateral peripheral pulmonary infiltrates, peripheral blood eosinophilia, and histologic features were consistent with eosinophilic pneumonia. The symptoms improved quite rapidly after the discontinuation of mesalazine and initiation of steroid therapy. Therefore, adverse pulmonary reactions to mesalazine must be considered in a differential diagnosis of pulmonary involvement in patients with inflammatory bowel disease who are receiving with measlazine therapy.

9.Effect of Testosterone on the Relaxation of Rabbit Corpus Cavernosum.

Androgen was thought to be linked to sexual activity in man but it did not identify the peripheral action and its mechanism of androgen. The present study was designed to determine the role of testosterone in the relaxation of the corpus cavernosum. Effect of testosterone on the relaxation of rabbit corpus cavernosum was investigated from three groups of rabbit; two groups were castrated and the third underwent a sham operation (control). After castration, one group, received subcutaneous injection of testosterone (testosterone propionate 10 mg/day). Strips of rabbit corpus cavernosum were isolated and mounted in 10 ml organ chambers. Isometric tension measurement of acetylcholine (Ach: 10(-5)M) and papaverine (10(-5)M) in the precontracted cavernosal muscle strips with phenylephrine (5 x 10(-6)M) were performed in all groups; control, castration group (four sub-groups: 1, 2, 4 and 8 weeks after castration) and testosterone replacement group (four sub-groups: testosterone was given from 1, 2, 4 and 8 weeks after castration for 14 days respectively). The relaxation response of corpus cavernosal muscle to acetylcholine and papaverine was significantly decreased in castrated group and restored up to the level of control after testosterone replacement. The maximum inhibitory effect of castration on the relaxation of corpus cavernosal muscle began to be shown at 4 weeks after castration. Based on these results, we conclude that testosterone regulates the tone of cavernosal smooth muscle through the pathway including cholinergic endothelium mediated relaxation.

Isoconazole nitrate is one of the broad-spectrum antimycotic agents recently developed from imidazole derivatives. Authors performed ulticenter trials to evaluate the therapeutic effect of Travogen and Travocort in superficial fungal diseases. In the usual superficial fungal diseases, Travogen was applied 4 weeks. In the inflammatory and eczematoid superficial fungal diseases, Travocort was applied 2 weeks initially and then followed by 2 weeks application of Travogen. One hundred and ninty one patients with superficial fungal diseases were included in this study during g months from December, 1982 to August, 19$3 The obtained results were as follows; The overall cure rate of Travogen and Travocort in each superficial fungal diseases were gl 7g (tinea pedis), 98. L% (tinea cruris), 1ppg, (tinea corporis), 9g. 100% (tinea manus), 1ppg (tinea capitis) and 1pgg (candidiasis) respectively.
