1.Evaluation of major methods of hemoglobin analysis

Hemoglobinopathy is a kind of hereditary hematonosis caused by the disproportion among normal hemoglobins or formation of abnormal hemoglobins.Hemoglobin analysis plays an important role in screening and diagnosing hemoglobinopathy.Emerging methods have been applied continuously to hemoglobinometry since the early 20th century.Nowadays,agarose gel electrophoresis,high performance liquid chromatography and capillary electrophoresis are widely used with mass spectrometry as the latest applied method.All these methods have their own features.Appropriate combination of these methods will lead to more scientific and reliable results.
2.Research advancement on measuring scale and influential factor of quality of life in patients with nasopharyngeal carcinoma

The quality of life(QOL) in patients with nasopharyngeal carcinoma ( NPC ) has attracted people' s attention increasingly. Most of the studies focus on measuring scale and influential factor. Both the disease itself and many non-somatic factors can affect the patients' quality of life. In this review, we summarize these research advancements on measuring scales and influential factors, considering that it need more studies regard to quality of life in NPC patients, and it is very urgent and important to work out the special measuring scale of NPCQOL that suits Chinese culture and value system.
3.Application status and progress of mediastinoscopy in minimally invasive esophagectomy

China is one of the countries with the highest incidence of esophageal cancer, which is still increasing year by year in recent years. Surgical treatment is the first choice for early and middle esophageal cancer. Surgeons have been exploring how to remove the tumor as completely as possible and reduce the trauma as far as possible. In recent years, with the rapid development of minimally invasive surgery and endoscopic technology, minimally invasive esophagectomy(MIE)has led the trend of radical surgery for esophageal cancer. At present, the mainstream minimally invasive surgery is thoracoscopic thoracoscopic(VATS)resection of esophageal cancer, which requires thoracotomy and anesthesia, resulting in large surgical trauma and more complications of postoperative circulatory respiratory system. Mediastinose-assisted esophagectomy(MAE), which eliminates a thoracotomy, is also an important part of MIE. Overseas MAE application started early, but the domestic development is relatively slow. This article summarizes the experience of different MAE surgical methods in China, and provides the basis for its promotion in China.
4.In vitro differentiation of human bone marrow mesenchymal stem cells into hepatocyte-like cells:Effect of hepatocyte growth factor and epidermal growth factor

BACKGROUND:Previous research has demonstrated human bone marrow mesenchymal stem cells(HMSCs)differentiate into hepatocyte-like cells;however,biological characteristics and differentiation mechanism remain unclear,and differentiation system remains immature.OBJECTIVE:To investigate the feasibility of hepatocyte growth factor(HGF)and epidermal growth factor(EGF)to induce the differentiation of HMSCs into hepatocyte-like cells.METHODS:HMSC5 were obtained from patients with esophageal cancer and were separated by density gradient centrifugation combined with attachment method.The phenotypes of MSCs were identified by flow cytometry.The third-passage HMSCs were divided into four groups:HGF(adding 20 μg/L HGF),EGF(adding 20 μg/L EGF),HGF+EGF,and blank control groups.Morphology was observed using inverted microscope.At days 7 and 14 after induction,α-fetoprotein and albumin mRNA expressions were detected using RT-PCR assay.RESULTS AND CONCLUSION:The HMSCs did not express hematopoietic cell CD34 and CD35,but strongly expressed β1-integrin CD29 and matrix receptor CD44.HMSCs changed from long fusiform shape to polygon or similar round shape in the HGF,EGF,and HGF+EGF groups.At days 7 and 14 after induction,α-fetoprotein and albumin mRNA expressions were positive.However,polygon cells were not observed in the blank control group,and α-fetoprotein and albumin mRNA expressions were negative.This suggested that HGF,EGF,and HGF+EGF could induce the differentiation of HMSCs into hepatocyte-like cells;however,their differentiation ability still needs to be further semi-quantitatively analyzed using immunohistochemical staining.
5.A comparison between C-reactive protein,adrenomedullin and endothelin-1 in patients of chronic congestive heart failure

Objective To study the changes of cytokine activation in patients with chronic congestive haert failure(CHF) as indicated by plasma levels of C-reactive protein(CRP)and its relation to adrenomedullin(ADM) and endthelin-1(ET-1).Methods The plasma levels of CRP,ADM and ET-1 of 60 decompensated haert failure(DHF) patients group and 30 essential hypertension patients with compensated cardiac function(CCF) group were tested. CRP was determined by immunoturbidometry,ET-1 and ADM were determined by radioimmunoassay.Results The plasma levels of ET-1 ,ADM and CRP of decompensated heart failure group were significantly higher than the compensated cardiac function group;In the decompensated heart failure group,the plasma levels of CRP and ADM were elevated according to worsening of heart failure(NYHA classification) ;In the decompensated heart failure group,the plasma levels of CRP were positively correlated with ADM(r=0.57).Conclusion There is extensive activation of cytokines,ADM and ET-1 in patients with chronic congestive heart failure.Cytokine activation might play a role in the synthesis of ADM,thus contribute to the hum oral regulation of heart failure,but may not be re- sponsible for the activation of ET-1.
6.Scoliosis secondary to osteoid osteoma of the spine:differential diagnosis and management

[Objective]To evaluate diagnostic and the therapeutic options of osteoid osteoma of the spine associated with scoliosis. [Methods]Nine patients with osteoid osteoma of the spine were treated from 1997 to 2006.All patients presented with spine deformity as their main complaint at the first consultation.The mean age was 15.2 years (range 8-21 years).Two patients had their osteoid osteoma in the thoracic area,2 in the thorcolumbar area ,and 5 in the lumbar area.Three were well controlled after long term administration of nonsteroidal anti-inflammatory drugs,and the other 6 were treated with en bloc excision.[Results]All the 6 patients with en bloc excision of the tumor were pain-free post-operatively.The scoliotic curve disappeared or was less than 15 degrees in 5,but the structural curve persisted in 4.[Conclusion]The spinal osteoid osteoma should be suspected when back pain or painful scoliosis occurs in adolescents.Computed tomography is necessary to evaluate the location and features of the lesion.En bloc excision of the tumor can be suggested if the symptoms are not well relieved with nonsteroidal anti-inflammatory drugs.
7.The treatment of open fracture and infection of tibia by grafting the musculocutaneous flap with uncovered in the special region

Objective To study the clinical value of grafting the musculocutaneous flap with uncovered in the special region to treat the open fracture and infection of tibia. Methods Twenty-eight patients including 19 males and 9 females with ankle and leg tissue defect were applied to graft the musculocutaneous flap with uncovered in the special region from July 2007 to January 2010. The tissue defeat in proximal part of the tibia were 6 cases, and 21 cases in middle and distal part, 1 case almost in whole part of the tibia. The bone occurred a defeat and infection in 23 cases, the bone necrosis occurred in 5 cases. The patients treated with iliac bone flap were 5 cases, anterolateral thigh flap were 21 cases, anterolateral thigh flap combined with fibula flap were 2 cases, the flap were transplanted by anastomosis in 26 cases. Results The tissue grafting were all survival and the survival rate were 100%. The callus formation were appear after operation from 3 to 6 months, and necrosis bone tissue almost were abstracted about 6 months. The wound were primary closure and not happened infection in 26 cases, the wound appeared infection under deep tissue in 2 cased,but being cured after 6 months by operation. The 21 patients were followed up from 3 months to one and helf year. The long-term was satisfaction and the donor area was perfect. Compared with closed graft, its area of the flap was small and need spend little time to treat the repair the defeat, and harness to recover the healthy of body and mind, also decrease the chance of infection in deep tissue. Conclusion It is demonstrated that grafting the tissue flap with uncovered in the special region is an ideal approach to treat the open fracture and infection of tibia.
8.Treatment of anterior canal benign paroxysmal positional vertigo by Yacovino repositioning maneuver.

OBJECTIVE:
To evaluate the efficacy of Yacovino repositioning maneuver in patients with anterior semicircular canal benign paroxysmal positional vertigo (ASC-BPPV).
METHOD:
Nine patients were diagnosed as ASC-BPPV from January 2013 to October 2014. All the patients were performed with Yacovino repositioning maneuver and the effective rate were evaluated by Dix-Hallpike tests.
RESULT:
Among the nine ASC-BPPV patients, 2 cases were successfully controlled by the first maneuver, 2 cases by the second time, and the nystagmus of 1 case was disappeared after 1 months' follow-up. The remaining 3 cases were respectively followed up till 7,8, 12 months with consistent positional downbeat nystagmus.
CONCLUSION
Being a relative low incidence disease, of ASC-BPPV also has low effective rate after Yacovino repositioning maneuver.

10.Count of Amnionic Fluid Lamellar Body to Predict the Maturity of Fetal Lung

Objective To discuss the value of amnionic fluid lamellar body count(LBC) as a predictor of fetal lung maturity. Methods Amnionic fluid was obtained during the cesarean section of 41 normal term pregnancies. LBC was performed on each specimen, and lecithin-sphingomyelin(L/S) ratio was detected simultaneously. Results (1)Amnionic fluid LBC was (86?43)?10 3 (72.5?10 3~99.4?10 3) in normal term pregnancy. (2) Positive correlation was shown between the amnionic fluid LBC and L/S ratio(r=0.66,P=0.001). Conclusion The LBC is a rapid and accurate method to determine the fetal lung maturity.
