1.A Case of Asymmetric Septal Hypertrophy Combined with Conn's Syndrome.

We report a case of a 44 year old femele with unilateral aldosterone-proudcing adrenal adenoma characterized by hypertension, plasma aldosterone excess, and low plasma renin, commonly but not invariably with hypokalemia. She also had asymmetric septal hypertrophy of left ventricle established with two-dimensional echocardiography. The electrocardiogram showed inverted T wave and prominent U wave with high QRS voltage on precordial leads. In the case of this patient, we are not sure whether asymmetric septal hypertrophy was caused by secondary hypertension and chronic aldosterone excess of primary aldosteronism, or hypertrophic cardiomyopathy per se, so further long=term follow-up is required to determine it. Following the successful unilateral adrenalectomy, however, the systemic pressure fell down to the normal level and electrolyte abnormalities were corrected immediaterly within a few days and the modest regression in septal hypertrophy was noted in one year, suggesting that the promary aldosteronism contributes to the development or porgression of asymmetric septal hepertrophy.

Chronic cavities of the dorsum of the foot often have bare bone and are unsuitable for free skin grafting. Local Flaps are often impossible because of surrounding scar tissue and dead space. A simple method is presented; it consists of the excision of the lesion, filling the cavity by a two staged random-pattern de-epithelialised turn over flap, and skin graft.

6.A Case of Pacemaker Syndrome.

Although ventricular pacing alone initially had deemed adequate for most clinical situations, some patients did not do well after ventricular pacing was initiated, and developed various symptoms attributed to this mode of pacing. The pacemaker syndrome is complex of clinical signs and symptoms related to the adverse hemodynamic and electrophysiologic consequences of ventricular pacing in the absence of other causes. Neurologic symptoms or those congestive heart failure predominated. We recently experienced a case of pacemaker syndrome in a 44-year-old female who had suffered sick sinus syndrome and was implanted with dual chamber pacing system being programmed to VVI pacing. She complained of chest discomfort, dyspnea, and near-fainting in a day after being programmed to VVI. Blood pressure was decreased to 9/60mmHg. Electrocardiography showed toPwave onT wave, representing retrograde ventriculoatrial conduction. The symptoms and signs were disappeared immediately after the pacing system was programmed to DDD pacing.

7.Postoperative Mucoceles of Frontal, Ethmoid, or Sphenoid Sinus.

BACKGROUND AND OBJECTIVES: Incidences of postoperative mucoceles of the frontal, ethmoid, or sphenoid sinuses are currently on the rise. The clinical rogression of the postoperative mucoceles in the frontal, ethmoid, or sphenoid sinuses are different from that of the maxillary mucoceles. In this study, we investigated the clinical patterns and treatments of postoperative mucoceles, which have developed in the frontal, ethmoid, or sphenoid sinuses. MATERIALS AND METHODS: Between June,1994 and August, 2005, twenty-six patients with thirty postoperative mucoceles were treated surgically. They had a previous history of operation and incidences of maxillary mucoceles were excluded from the study. A review of the clinical subjects were performed using their clinical records and radiological findings. RESULTS: The locations of the postoperative mucoceles were, in the order of frequency, the ethmoid, frontal, fronto-ethmoid, spheno- ethmoid, sphenoid, and fronto-ethmoidsphenoid sinus. Postoperative mucoceles occurs mostly in the form of ophthalmic symptoms and headache. The ophthalmic manifestations were different between the anterior, posterior and the antero-posterior mucocele group. The mean duration until diagnosed as postoperative mucocele since the previous sinus surgery was 9.9 years, and endoscopic sinus surgeries and intranasal ethmoidectomies with Caldwell-Luc operations were the majority among previous surgeries. Most postoperative mucoceles could be successfully treated with an endoscopic marsupialization. CONCLUSION: Frontal, ethmoid, or sphenoid postoperative mucoceles developed mostly on the ethmoid sinus and occurs mainly in the form of ophthalmic symptoms. Most postoperative mucoceles were successfully treated with an endoscopic marsupialization.

8.Techniques of gynecologic single-port access laparoscopic surgery.

Since 2008, various single-port access laparoscopic surgeries in gynecologic diseases have been performed in Korea. Single-port laparoscopic surgery has some advantages. It has less visible scars, less pain, and a faster recovery. Single-port laparoscopic surgery also has a drawback that is hard to handle instruments compared to conventional multi-port laparoscopic surgery. However, single-port laparoscopic surgery can be performed safely by practitioners who have experiences in single-port laparoscopic techniques. Based on experiences of gynecologic single-port laparoscopic surgery for 1000 cases in Incheon St. Mary's Hospital until August 2011, we describe techniques for single-port access laparoscopic surgery.

9.Kimura's Disease in the Groin: A case report .

Kimura's disease is a rare benign disease characterized by subcutaneous or dermal tumors occurring predominantly on the head and the neck. It usually occurs in young adults without constitutional symptoms, except for peripheral blood eosinophilia. The histopathologic features of the tumor are characterized by dense lymphoid aggregates containing a prominent germinal center and by the proliferation of endothelial cells associated with varying degrees of lymphocytic, histiocytic, and eosinophilic infiltration. We report a case of Kimura's disease without peripheral blood eosinophilia in a 24-year-old male who had had a painless and slowly growing tumor-like swelling on his right groin for one year. The mass was excised, and the specimen was confirmed as Kimura's disease. After surgical excision, the lesion recurred, so 30 mg of oral prednisone was given daily for one month and then decreased gradually for another one month. Now, the lesion is completely healed.

10.Matrix Metalloproteinase-2 and -7 Expression in Colorectal Cancer.

PURPOSE: Matrix metalloproteinase-2 (MMP-2) and MMP-7 have been implicated in tumor growth and metastasis. This study aimed to investigate the expressions of MMP-2 and -7 in colorectal cancer and to evaluate their values as prognostic markers. METHODS: Immunohistochemical staining for MMP-2 and -7 was done in 144 resected colorectal cancer specimens. Clinicopathological data and survival results were compared with regard to the expression results. RESULTS: The expression rates of MMP-2 in tumor cells in the tumor center and the tumor border were 16.7% and 38.9%, respectively. That of MMP-2 in stromal cells was 27.8%. MMP-7 immunoreactivities of tumor cells in the tumor center and the tumor border were 6.9% and 23.6%. The expressions of MMP-2 and MMP-7 were correlated. MMP-2 expression in stromal cells was more increased in the distal part of the colorectum: 8.8% in right colon cancer, 29.5% in left colon cancer and 36.4% in rectal cancer. MMP-2 expression of tumor cells in the tumor border was correlated with T-stage. MMP-7 expression of tumor cells in the tumor border was increased in case of infiltrative cancer compared with fungating tumor. The expression patterns of MMP-2 and -7 were not correlated with other clinicopathological factors, including tumor markers, node metastasis, distant metastasis, lymphatic invasion, tumor differentiation, and recurrence. No significant associations between the overall and disease-free survival rates and the MMP-2 and -7 expression patterns were noted. CONCLUSION: The high expression rates of MMP-2 and -7 in tumor borders suggest that MMP-2 and -7 have some role in tumor invasion, but in this study, MMP-2 and -7 did not appear to be significant predictors of prognosis in colorectal cancer.
