1.A Case of Albright's Syndrome.

Albright's syndrome consists of large pigmented macules of the skin and fibrodysplasia of the bones. The full syndrome with precocious puberty occurs particularly in girls. Other endocrinopathies including Cushing syndrome, hyperthyroidism and acromegaly can be accompanied. A case of Albright's syndrome is reported. The patient was a 4-year-old male child, who was born with multiple, dark brownish, irregularly bordered macules on the trunk and extremities. Deformities of facial skeleton and limping gait were noticed at the age of 2 years. He had pathological fractures twice, at the age of 3 and 4 years, respectively, due to fibrodysplasia of both femur bones. Especially, involvement of the sphenoid bones caused compression of the optic nerves and diminished vision of both eyes. On radiologic examinations, polyostotic fibrous dysplasia were noticed on the humerus, scapula, femur, tibia, fibula, pelvis, vertebra and skull base. The level of alkaline phosphatase was highly elevated.

2.Two Cases of Piroxicam-Induced Photoallergic Dermatitis.

Piroxicam is a new nonsteroidal anti-inflammatory drug(NSAID) and widely used in the management of rheumatoid arthritis and osteaorthritis. We experienced. two cases of photosensitive dermatitis developed while taking piroxicam. Both had pruritic and eczematous skin lesions on sun-exposed areas and showed positive reactions to 0. 1 % piroxicam solution in photopatch test in contrast with control group, which suggest a photoallergy mechanism. To our knowledge, these are the first reported cases in the Korean literature.

4.Malignant Schwannoma.

Malignant schwannoma is a malignant neoplasm of nerve sheath origin. It develops in an unaltered nerve or in a neurofibroma, and rarely, if ever, within a neurilernmoma. We experienced a malignant schwannoma in a 75-year-old man without von Recklinghausen's syndrome. The lesion was solitary and polypoid, fungating nodule in the posterior portion of the left thigh, The tumor cells had some features of Schwann cells and positively stained to S-100 protein and neuron-specific enolase. Complete excision was performed but the associated nerve could not be demonstrable.

5.A Case of Forehead Plaque as an Initial Sign of Tuberous Sclerosis.

Tuberous sclerosis is a systemic disorder of hamartoma format on includes facial angiofibromas, subungal fibromas, hypomelanic maciles Shagreens patches. The earliest skin manifestation of tuberous sileiule, but it may appear later. We present a case of fibrous plaqu a recognized skin signs. Forehead plaque can be the earliest skin near in many organs. Skin involvement less forehead fibrous plaques, and vsis is usually hypornelanotic inacipearing before any of these more festation of tuberous sclerosis. Its presence may lead to early diagnosis and appropriate genetic couise hng hut may be a poor prognostic sign.

6.A Case of Eyelid oil Granuloma Caused by High Pressure Injury.

Oil granuloma is a foreignbody reaction which occurs after injection of lipid usually for cosmetic or therapeutic reasons. I present a case of eyelid oil granuloma caused by accidental high pressure injury in a 39-year-old man. The patient had a 2 x 0.5cm sized, firm, normardear subcutaneous nodule on the Rt. upper eyelid. Histopathologic examination showed a characteris swiss-cheese appearance. Eyelid oil granuloma caused by high pressure injury is very rare and my case is the first described in Korean medical literatures.

7.Clinical and Histophthologic Review in Patients with Henoch: Schonlein Purpura.

Authors review the clinical and histopathologic findings of 41 EIenoch-Schonlein purpur.a patients and attempt, to clarify the association between abnorma] urinary findings and anti-streptolysin 0(ASLO) titer and performed the direct immunofluorescence(',E)II) study on skin lesion to investigate the pathogenetic mechanim of renal involvement. The results were summarized as follows : 1. Ag(, distribution was form 3 month to 63 years and mean age was 18.9 years. male to female ratio was 1.2:1(22 males '. 19 females). 2. In he past history, there were preceding upper respora.tory infections in 25(61:o)patients and drug history in 10(24.3%)patients. 3. (linical manifestations in the order of frequency were as follows : arthralgia (60.9%), hematuria or albuminuria(43.8%), abdominal pain(21.9%), and positive occult: blood test(7.3%)in stool examination. 4. In the laboratory fiindings, leukocytosis was cleveloped in 14.6%, anemia in 2.4%, increased eryttrocyte sec1imentation zate in 12.3% increased ASLG titer in 43.9% of patients, and serum IgG was increased in 4 of 20(20.6)patierits, IgA in 4 of 21(19%)patients, IgM in 1fo 19(5.3o)patients. 5. Because correlation between increased A.LO titer ancl ahnoormal urinary findings wa.s high(x-test, p< 0.0,5), increased ASI,O titer may be inclicator of renal involvement. 6. DIF stuc].y was per formed in 34 patients. All showed positive reactiviy. IgG(17%), IgA(29.4% ), Clq(14.6%), C3(12.2% ), fibrinog.n(82.4%) were deposited around the vessel wall in a granular pattern. 7. There was statistically significant increase in mean serum IgG value in patients with abnormal urinary findings( t test, p < 0.05 ).

8.A Case of Bullous Pemphigoid Associated with Graves' Disease.

We report herein a case of bullous pemphigoid(BP) associated with Graves' disease(GD) The patient was 43-year-old female who had suffered frorn GD for 8 years. She had annular, erythematous atrophic plaques with a margin of small vesicles on the frontal and temporal areas, Histopathologic findings of the skin lesion showed subepiderrnal bulla, and numerous eosinophilic infiltrate in the bulla cavity and dermis. Direct immunofluorescent study of perilesional skin revealed linear IgG deposition at the dermo epidermal junction. We think our case shows a higher than chance association of BP and GD because both are basecl on autoimmune mechanism.

9.Atypical Fibroxanthoma of the Skin.

An immunohistochemical and electron microscopic study of an atypical fibroxanthoma of the skin (AFX) was reported. The lesion developed on the left scapulararea in a 70-year-old man. The result using antibody to S-100 protein was negative and the tumor cells observed with electron microscope were undifferentiated, fibroblast-like, histiocyte-like and multinucleated giant cells. But Langerhans-like cells or Langerhans were not found.

10.A Study of Minimal Inhibitory Concentrations of Several Kind of Drugs against Pityrosporum orbiculare.

The bacteria as well as the fungi can build up resistance against different kinds of drugs or can decrease in sensitivity to these drugs. Therefore it is necessary that we use the MIC test for acertain period of time to determine the right concentration of drug to use in therapy. The culturing of P. orbiculare was successful in 1951. Up until that time, there were few drugs available to use in the MIC test. So, treatment for Tinea versicolor was dependent upon ones previous experience in determining the drug's effective concentration. Therefore, we performed the MIC test of clotrimazole, sodium thiosulfate, selenium sulfide, propylene glycol against P. orbiculare. The diagnos is for Tinea versicolor in the patient was confirmed by using KOH smear and by the clinical manifeststion. The fungi were then isolated in a culture from an olive oil rich media Mixture of isolated fungus and olive oil was applied to the lateral side of a healthy male's right upper extremity. After one week, P. orbiculare was confirmed by wood lamp examination, KOH smear and culture. After confirmation of P. orbiculare, a variety of concentrated drugs were used. Then media, containing propylene glycol, sodium thiosulfate, selenium sulfide and clotrimazole with a small amount of olive oil was used in inoculations of 10 viable unit/ml of P.orbiculare in attempt to estblish the MIC test. Test results are as follows: l) Propylene glycol MIC Range; 12. 5mg/ml~7. 5mg/ml. 2) Sodium thiosulfate MIC Range; 500mg/ml~7. 5mg/ml. 3) Selenium sulfide MIC Range; l. 25mg/ml~0. 04mg/ml. 4) Clotrimazole MIC Range; 0. 5mg/ml~ 0. 0005mg/ml.
