2.A Clinical Study on Macrosomia.

Obstetric problems concerning macrosomia were evaluated by retrospective review of 91 pregnancies that resulted in the delivery of an infant weighing 4,000 gm or more at the Yeungnam University Hospital during 3 1/2 years from Jun 1983 to Oct 1986. The results obtained were as follows 1. Macrosomic infants weighing 4,000 gm or more occurred in 2.8% of the deliveries. 2. 65.9% of macrosomic infants and 53.5% of total infants were male. The ratio of male was statistically higher in the macrosomic infants than in the total infants (P<0.05) 3. The incidence of macrosomia was higher with increasing gestational age, and deliveries at 42 weeks or more gestation were more common in the macrosomic infants than in the total infants (P<0.01).

3.A Study of Serum HBV Markers in Term Pregnant Women and Their Neonates.

To evaluate the status of hepatitis B virus infection in the mothers and neonates and to determine the maternal-neonatal transmission of hepatitis B virus, 2,276 term pregnant women were screened for presence of serum HBsAg, at the Department of Obstetrics and Gynecology, Yeungnam University Hospital, during the period of 18 months from Jan. 1986 to Jun. 1987, and the sera of sixty-six HBsAg carrier mothers and their neonates were tested for HBV markers and liver enzymes. The results were as follows: 1. The prevalence rate of asymptomatic HBsAg carrier in the term pregnant women was 4.7% (53/1,279). 2. Positive rates of HBsAg and anti-HBs in the sera of sixty-six neonates born to asymptomatic HBsAg carrier mothers were 12.1% and 9.1%, respectively. Transient elevation of SGOT (three to four times of upper normal limit) was detected in one of eight HBsAg-positive neonates and one of six anti-HBs positive neonates. 3. Positive rates of anti-HBc, HBeAg and anti-HBe in the sera of sixty-six asymptomatic HBsAg carrier term pregnant women were 93.9%, 45.5%, 50%, respectively. The rates of transmission of maternal anti-HBc, HBeAg and anti-HBe to the neonates were 85.5 (53/62), 90% (27/30) and 87.9% (29/33), respectively. 4. Serum HBsAg was detected in four of thirty neonates born to HBeAg positive HBsAg carrier mother, three of thirty-three neonates born to anti-HBe positive HBsAg carrier mothers, and one of three neonates born to both HBeAg and anti-HBe negative HBsAg carrier mothers.

6.Conservative Chemoradiation Treatment for Epidermoid Carcinoma of the Anal Canal.

PURPOSE: Anal canal carcinoma is a rare clinical entity accounting for 1 to 3 percent of all gastrointestinal malignancies. Prevoiously, abdominoperineal resection was the primary modality of treatment. However, a recurrence rate of 20 to 40 percent has been observed following this procedure. In recent years, multimodal therapy with radiation and chemotherapy has been shown to at least equal the results obtained through surgery with the ability to preserve sphincter function. The objective of this study is to evaluate the survival, recurrence pattern, acute and late morbidity rates of a widely used combined chemotherapy and radiation therapy regimen given to patients with carcinoma of the anal canal. METHODS: Ten patients received radiation therapy (5,000~5,400 cGy) and chemotherapy (mitomycin-C plus 5-fluorouracil) as a primary treatment between 1988 and 1998. RESULTS: Patients with primary tumors had a complete response rate of 60 percent and a 10-year corrected survival rate and disease-free survival rate of 77.14 percent and 60 percent, respectively. There was a significant difference in survival rates and disease-free survival rates according to AJCC stage. Acute complications (grade III~IV, ECOG toxicity criteria) such as leukopenia, anemia, thrombocytopenia and radiation dermatitis were seen in 10 to 50 percent of cases. Furthermore, 20 percent of patients experienced late treatment-related symptoms including perineal fibrosis and incontinence due to sphincter dysfunction. CONCLUSION: The current treatment regimen is effective but carries a considerable risk of complications. Therefore, new chemotherapeutic modalities that may reduce such complications, but still potent, are needed.

7.Endoscopic Findings of Tuberculous Colitis.

The purpose of this paper is to review the colonofiberscopic findings in the 21 patients with tuberculous colitis, diagnosed by colonofiberscopic findings including histopathology and by follow-up observation after anti-tuberculosis chemotherapy. The results are as follows: 1) The male to female ratio was l. 33(R:4) and the average age of the patiets were 34. 9 years in men and women respectively. 2) The chief complaints were abdominal pain in 15 cases(71.4%), diarrhea in 14(66.7%), weight loss in 12(57. 1%), anorexia in 8(38, 1%), and so on. 3) Anemia and an increase in ESR were major abnormalities in hemogram. 4) On the chest X-ray examination, 65% of the cases showed active tuberculous lesions and the barium enema disclosed the changes suggesting tuberculous colitis in 52. 6% of the cases. 5) On colonofiberscopy 8 cases were ulcerative, 6 were hypertropbic and 7 were ulcerohypertrophic. 6) Histopathologic examination of the biopsy material from the colonofiberscopy revealed granulomata with caseation necrosis in 2 cases, granulomata without caseation necrosis in 8 cases, and the findings of chronic inflammation in 11 cases. 7) This study suggests importance of colonoscopic examination including histopathologic study of the biopsy material in the diagnosis of tuberculous colitis. The diagnostic efficieocy would be much improved if bacteriological examinations of the specimen could be developed.

8.A Case of Impetigo Herpetiformis in Pregnancy.

Ovarian tumor in pregnancy is not rare and benign cystic teratoma is most common ovarian tumor in pregnancy. Most ovarian tumor in pregnancy is found at first and second trimester but sometimes found at third trimester and it is difficult to make a decision for management. Moreover physician should consider fetal age and mother's condition, and weigh outcome of operation for complicated ovarian tumor in pregnancy. Torsion of ovarian tumor is not an uncommon complication during first trimester of pregnancy and puerperium but rare occurs during the third trimester of pregnancy. We have experienced a case of torsion of benign cystic teratoma of ovary which occurred at 37 weeks gestation in a 25 year-old woman. So we report this case with a brief review of literature.

9.The Clinical Significance of Absence of Umbilical Artery End-Diastolic Flow in Severe Pre-Eclampsia and Eclampsia.

To determine the perinatal mortality and morbidity of fetuses with absent end-diastolic velocities (AEDV) of the umbilical artery in severe pre-eclampsia and eclampsia, the outcome of 5 fetuses with AEDV was compared with that of 35 fetuses with positive end-diastolic velocities (PEDV). The study population comprised 38 cases of severe pre-eclampsia and 2 cases of eclampsia with structurally normal singletons, who had had umbilical artery Doppler velocimetry weekly from admission to delivery. The Doppler velocimetry result was not used for the clinical management. Perinatal death and neonatal morbidity from both groups were further examined in gestational age category to control the influence of preterm births. The incidence of AEDV of the umbilical artery Doppler velocimetry in severe pre-eclampsia and eclampsia was 12.5% (5/40). The AEDV group had a significantly higher incidence than the PEDV group in terms of ceasarean section due to fetal distress (60% : 17%), Apgar score < 7 at 5 minutes (60% : 14%), perinatal death (25% : 0%) and assisted mechanical ventilation (67% : 9%) both at 32-36 weeks. Time intervals from the detection of AEDV to delivery of live neonates varied from the day to 15 days. In conclusion, AEDV in the umbilical artery might be of clinical value in routine surveillance of pregnancies complicated by severe pre-eclampsia and eclampsia, and predict hypoxic fetal condition which needs operative interventions before or during labor and mechanical ventilation after birth.
