Rabies is a fetal viral encephalitis to which all mammals, including man are susceptible. This virus, the genus of Rhabdovirus, is usually present in the saliva of infected animals and is transmitted by their bite. As a rule the virus enter the CNS by ascending along peripheral nervous system and extremely variable in extent. Negri bodies are appear in about 75% of cases. These bodies are eosinophilic, usually rounded inclusions and is located in the cystoplasm of the neurons, most frequently in the pyramidal neurons of the Hippocampus and the Purkinje cells of the cerebellum. We have experienced a case of canine rabies that showed Negri bodies in the Purkinje cells along with diffuse degenerative encephalitis and perivascular mononuclear cells infiltration. The Negri bodies were best demonstrated by Negri body staining although routine H-E staind was also useful. There inclusion bodies were located exclusively in the cytoplasm, and were found occasionally. Electron microscopic findings of the Negri bodies showed aggregates of bulletshaped nucleocapsids. We report a Negri body found in the brain of rabid dog with the light and electron microscopic findings.

After the description of the necessity of denitrogenation in rebreathing anesthesia circuits some decades ago, many methods were recommended for techniques of ventilation with oxygen before induction of anesthesia for the purpose of maintaining arterial oxygenation during laryngoscopy and tracheal intubation . In this point of view, we compared PaO2, MAP, PaCO2, pH of non-preoxgenation group(oxygen was given after succinylcholine was injected) with those of preoxygenation group(owygen was given after thiopental sodium was injected). The cases were selected randomly who blongs to ASA class l or ll(without cardiopulmonary abnormalities) and ages betweens 27 years and 66 years old. Before induction of general anesthesia, we cannulated radial artery after Allen's test and sampled arterial blood before anesthesia, after thiopenthal and succinylcholine were injected respectively, when tracheal intubation was done in the two groups and measured PaO2, PaCO2, MAP, pH respectively. PaCO2, MAP, pH changes in the two groups were not significant and of no clinical significanses. PaO2 after tracheal intubation showed marked increase in preoxygenation group compared to non-preoxygenation group but mean PaO2 was maintained within normal range in non-preoxygenation group with the apnetic period of tracheal intubation. WE found that non-preoxygenated patients also had normal range of PaO2 during the whole process of induction of anesthesis. But I think preoxygenation technique of any method can reserve more time and will do more effectively, especially when times consuming events of difficult intubation of any reason occurs.

Purpose : Early colorectal cancer is defined as the depth of tumor invasion limited to mucosa or submucosa regardless of the presence or absence of lymph node metastasis. We performed a retrospective study to determine the chronological changes in frequencies of early colorectal cancer and clinicopathologic differences between early colorectal cancer (ECC) and advanced colorectal cancer (ACC). Methods : We reviewed hospital records of the patients with colorectal cancer operated between January 1990 and December 1995. We classified the patients into two groups, ECC and ACC, according to the depth of tumor invasion and compared the clinicopathologic characteristics. Results : Fifty eight patients (5.2%) were diagnosed with early colorectal cancer among 1113 colorectal cancer patients operated at the same period. The frequency of ECC has increased from 1.9% in 1970~1989 to 5.2% in 1990~1995. The average age of patients with ECC at the time of surgery was 55.8 compared to 56.5 for patients with ACC group (p>0.05). Most patients (72.4%) with ECC had bleeding symptoms and majority of the ECCs were located in the rectum (72.4%). The mean size of tumors was 2.6 cm in its greatest diameter and was significantly smaller than that of ACC (5.7 cm). Compared to ACC, ECC had better histologic differentiation and fewer lymph node metastases (p<0.05). Thirty six of the ECC patients underwent bowel resection and remaining 22 underwent local excision. After a mean follow up period of 39.1 months (range 2~81months), recurrence was detected in one case. There was no death during the follow up period. Conclusion : The frequency of ECC has increased recently. Compared to patients with ACC, patients with ECC had more favorable clinicopathologic characteristics and better outcome. In selected patients, minimal operation can be applied without compromising the clinical outcome.

Since the time of Jonathan Hutchinson(1858~63), it has been known that over 90% of cases of diffuse interstitial keratitis occur in syphilitic patients. Interstitial keratitis is due to acquired syphilis in 4% of cases and the other 6% of cases due to tuberculosis, sarcoid, trypanosomiasis, onchocerciasis and other rare infections. After the introduction of penicillin by Mahoney et al. in 1943 for syphilis therapy, syphilis epidemy was sharply decreased throughout the world. However, during the middle 1950's the incidence of syphilis began to increase throughout the world. In Korea. the syphilis incidence has been increased since around 1963-1965 in parallel with the world trends(Kim and Lew 1968). A 6 year old Korean girl has been found to have bilateral acute interstitial keratitis and a positive serological test for syphilis.

Pancreatoduodenectomy has been a standard procedure for periampullary cancer. Even when the benign lesion is limited to the pancreatic head, the same procedure has been applied if the lesion should be removed surgically. The duodenum has a key role in the digestive physiologic function. However, removal of the pancreatic head while preserving the duodenum has been considered a very risky procedure because of duodenal ischemia. Since the vascular anatomy of the peripancreatic area was thoroughly examined and a meticulous dissection technique was developed, duodenum-preserving procedure has been tried by several surgeons with success. In this paper we report our experience of duodenum-preserving pancreatic head resection. To our knowledge this is the first report in Korea. The lesions were insulinoma and two serous cystadenomas located at the pancreatic head. Total removal of the pancreatic head was performed trying to preserve pancreaticoduodenal vascular arcade without a Kocher maneuver. The common bile duct was totally preserved in all cases and cholecystectomy with T-tube choledochostomy was performed in one case. The pancreatic duct was ligated just distal to the ampullary portion and the distal pancreas was anastomosed to the posterior wall of the stomach. Operation time was 7 hours 30 minutes in one and 5 hours and 30 minutes in two cases. Transfusion was not required at all. Pancreatic leakage was developed in one case and cured with conservative management. Any complication related to the preservation of the duodenum(leakage or obstruction) had not developed. The patients were discharged on postoperative 46 day(due to pancreatic leakage), 18 days and 11 days respectively. In conclusion, duodenum-preserving pancreatic head resection is recommended for benign pancreatic head lesions, such as benign cystic tumor and localized chronic inflammation.
