43 cases of pen heart surgery anesthesia done at Kyungpook National University hospital from Jun 1983 to August 1984, were measured for the concentrations of serum Na, K, Cl and CO2 before, during and after bypass. Also these values were analysed according to disease such as A(V)SD, TOF and M(S)I groups. The results were as follows. The mean values for serum sodium level before, during and after bypass were observed to be 124.41+/-8.62, 127.56+/-8.63, and 130.73+/-29.04mEq/G. These values revealed a tendency to elevation and the value after bypass was significantly increased comparied with the value before bypass(p<0.01). The mean values for serum potassium levels of all the cases before, during and after bypass were observed to be 3.84+/-1.01, 4.38+/-1.48, and 3.59+/-0.95mEq/G. The value after bypass was significantly decreased compared with the value during bypass(p<0.001). The mean values for serum chloride level of all the cases before, during and after bypass were observed to be 98.26+/-6.87, 97.10+/-6.59 and 98.35+/-7.94mEq/G and these values showed no significant change. The mean values for serum carbon dioxide levels of all the cases before, during and after bypass were observed to be 20.78+/-3.70, 22.33+/-4.53 and 24.87+/-3.72 mEq/G and these values continuously increased. The value after bypass was significantly increased. The value after bypass was significantly increased compared with the value before bypass. In the changes of serum electrolytes before, during and after bypass according to disease, the serum potassium level after bypass of the A(V)SD group and during bypass of TOF group showed a significant change(p<0.05). The changed values in the serum electrolytes before and during bypass became within normal range after bypass.

PURPOSE: To compare the success rates, complications and cost effectiveness of extracorporeal shock wave lithotripsy (ESWL) and ureteroscopic lithotripsy (URS) using a Holmium: YAG laser to establish the more effective treatment for upper and mid ureter stones. MATERIALS AND METHODS: This retrospective study included 83 and 286 patients treated with URS and ESWL, respectively, for upper and mid ureter stones. URS was performed with a 7.5Fr semi-rigid ureterorenoscope and Holmium: YAG laser. ESWL was performed with Stonelith V.5 (PCK, Turkey). The stone sizes, success rates, postoperative complications and cost effectiveness were analyzed for each group. RESULTS: The overall success rates of the URS and ESWL were 97.6 and 82.5%, respectively. Success rates of URS were 98.4 and 95.2% when the stones were smaller and larger than 10mm, respectively. In the ESWL group, the success rates after the third session were 86.3 and 65.4% with stones smaller and larger than 10mm, respectively. The complication rates associated with URS and ESWL were 6 and 4.2%, respectively. The average cost of URS was lower than that of ESWL. CONCLISIONS: In our study, URS with Holmium: YAG laser was more efficacious than ESWL. Considering both the success rate and cost effectiveness, URS should remain the recommended first-line treatment for upper and mid ureter stones.

Recently, the 5-year survival rate of rhabdomyosarcoma has been greatly increased by combining therapy with radical surgery, radiation and chemotherapy. But it still has poor prognosis and there are few case reports of rhabdomyosarcoma of head and neck with long term survival rate. Here, we treated a rhabdomyosarcoma patient with radical surgery, radiation and chemotherpy. The patient visited our hospital in 1990 with a complaint of left facial protruding mass, diplopia and exopthalmos. She was diagnosed as rhabdomyosarcoma involving maxillary sinus and orbit. In september 1990, she received left total maxillectomy and eyeball exentration. After the surgery, we applied a 5000 cGy neutron therapy, 2000 cGy external radiation and 9 cycle pulse, along with VAC chemotherapy. She remains free of disease as of her last follow-up in February 1998.

A spinal extradural arachnoid cyst (SEAC) is a rare disease. Multiple SEAC is a very rare disease that has been reported worldwide, mainly in children, but there are few case reports in Korea. The authors report a marked reduction in cyst size on magnetic resonance (MR) imaging at the follow-up after surgery in surgically treated case of a patient diagnosed with thoracic type I and lumbar type II SEAC. After localization of the dural defect using preoperative MR myelograms, a minimally invasive laminectomy and cyst fenestration could prevent many surgical-related complications.

Odontogenic keratocysts are developmental odontogenic cysts which derived from rests of dental laminas and often found in posterior area of mandible. Because this lesion has tendency of frequent recurrence, treatment of choice is often radical removal of the lesion. Sometimes in case of large cysts, however, conservative therapy like marsupialization is often selected in treatment plan. A 39-years old woman referred to our department for evaluation of large radiolucent lesion that occupies the areas from mandible angle to upper part of ramus and condyle. In cytology, the lesion was identified as odontogenic keratocyst. Marsupialization was our treatment of choice, and the result was so favorable. 2 years later, there was small radiolucent lesion on upper part of mandibular ramus on panoramic view. It was suspected as recurred lesion, and excisional biopsy was done. On biopsy result, it was not a cystic lesion but fibrosis.
