Herein reported were three cases of papillary eccrine adenoma also called as tubular apocrine adenoma, characterized by dilated ducts containing eosinophilic secretion or kerain and intraluminal papillations. Case 1. A 23 year old male had a well defined erythematous nodule, measuring 1x1 cm. On the ankle of right leg over ten years. Clinical impression was dermatofibroma. Microscopic features were those of papillary ecrine adenoma. Case 2. A 53 year old female had a well demarcated dermal nodule, measuring 1.5x1 cm. On the buttock. Cut surface exhibited relatively well circumscribed tumor with yellowish brown color, Microscopic features were those of papillary eccrine adenoma with alcian blue positive materials in their lumens. Case 3. A 22 year old female had a showly growing hard painless small bean sized nodule at the metatarsal head area of right sole, measuring 2.3x1.8x0.7 cm. Clinical impression was calcinosis cutis. Microscopic features were those of papillary eccrine adenoma with foreign body granuloma and dystrophic calcification.

Two cases of duodenal bezoars are presented. Both were within pseudodiverticula of the duodenal bulbs whichwere created by chronic active ulcer disease. It is felt that deformed duodenal bulb interfered with transit ofthe gastrointestinal content causing the formation of bezoar within pseudodiverticula.

Many reports have been published on the tissue damage of a shock wave with respect to histopatological changes in light microscopy and various imaging modalities. However, the studies on the electron microscopic findings and cause of renal functional change such as parenchymal obstructive pattern following extracorporeal shock wave lithotripsy (ESWL) have not been elucidated. In order to evaluate the microstructural changes after shock wave exposure, gross, light microscopic and trasmission electron microscopic findings were analyzed with rabbit kidneys. Preliminary study (n=2) was performed to determine the dosage intensity of shock waves to inflict damage, using a EDAP LT 01 piezoelectric extracorporeal shock wave lithotriptor. A shoke wave of various intensities were given to the left kidneys of 3 different groups of rabbits. Storage value of 100, 50, 25 at rate of 20/sec under 87% power were given to group I (n=4), group II (n=4), and group III(n=3) respectively. The right kidneys were preserved as the control group. The rabbits were killed 6-12 hours later. In gross, there were a few subcapsular hemorrhagic foci and mild congestion of corticomedullary junction without a large hematoma formation. No significant differences were noted between each group. Light microscopic findings were mainly hydropic changes in the proximal convoluted tubules and congestion without significant necrotic changes. The observed pathologic changes in the transmission electron microscopy were vacuolization of cytoplasm with swelling of epithelial cells especially porximal convoluted tubules. There were also tubular obstruction due to swelling and desquamation of epithelial cells into tubular lumen. The structural changes of intracellular organelles were not found at storage values of 25 and 50. But dilatation and structural alterations of endoplasmic reticulums were noted of 100 with cell membrane rupture. The findings of this study suggest that tubular obstructions with or without structural change of intracellular organelles is one cause of renal functional change such as parenchymal obstructive pattern after ESWL.

BACKGROUND: Dermatologists frequently see morbilliform rash in the clinic, drug eruptions and viral eruptions. Due to close clinical similarities, it is difficult to differentiate them. OBJECTIVE: The purpose of this study was to review the histopathologic differences between drug eruption and viral eruption. METHOD: In this study, 13 patients diagnosed as rubella and 13 patients diagnosed as drug eruption in Department of Dermatology, Presbyterian Medical Center from 1995 to 1997 were included. We reviewed the epidermal changes such as vacuolopathy, dyskeratotic cells, spongiosis and exocytosis and dermal changes by light microscopy. RESULTS: 1. Vacuolopathy of basal layer was observed in 92.3% of patients in both diseases. 2. Dyskeratotic cells, spongiosis and exocytosis were observed significantly highly in drug eruption. 3. Perivascular lymphohistiocytic infiltration was mild in both diseases, but eosinophilic infiltration was significantly higher in drug eruption(84.6%) than viral eruption(15.4%). 4. Atypical lymphocytes were observed in both diseases. 5. Red blood cell extravasation was observed only in viral eruption. 6. Perivascular edema was significantly higher in drug eruption(76.9%) than viral eruption(38.4%) CONCLUSION: There were statistically significant differences in dyskeratotic cells, spongiosis, dermal eosinophilic infiltration and Red blood cell extravasation. Considering these observation, histopathologic findings may be helpful in differentiating between drug eruption and viral eruption.

Tumor suppressive effect of dehydroepiandrosterone (DHEA) on the experimentally induced hepatocellular carcinoma was investigated, especially focusing on glutatione transferase and transglutaminase with aptosis in the carcinogenesis. The chemical hepatocarcinogenic procedure of Solt-Farber method was used on Sprague-Dawley rats. Experimental groups were divided into AA group treated by the standard Solt-Farber regimen of diethylnitrosamine (DEN) and 2-acetamidofluorene (AAF) and AD group treated with DHEA simultaneously with AAF and the AAD group treated by DHEA after treatment with AAF. Each group was divided by time sequence further into four subgroups, GI (8wk), G2 (16wk), G3 (28wk), and G4 (36wk). For neoplastic lesion, the immuno histochemical study with anti GSTP antibody was carried out, while the activity and expression of TGase was compared at the same time. The results were summarized as follows; GST-P positive foci detected in AD groups were significantly more suppressed by DHEA treatment than AA groups (P<0.05). AD groups. AD group showed higher activities of TGase than AA groups (P<0.05), which was confirmed by Western and Northern blot analysis. But the number of apoptotic bodies was not correlated with activity and expression of TGase in the nodule. These results suggest that the suppressive effect of DHEA on the murine hepatocellular carcinogenesis might be operating on the promotion process of carcinogenesis rather than regression process of transformed hyperplastic nodules.

Authors experienced a rare adenocarcinoma arising in presacral sacrococcygeal teratoma in a adult and herein reported. The patient was a 52 year old female admitted because of lower abdominal pain for 5 to 6 months. Past and familial history were unremarkable. On rectal examination a mass was palpated 3 cm above the anal verge. The large bowel was displaced by an extrinsic mass but there was no evidence of intrinsic tumor lesion on barium enema, CT was done and showed a large presacral tumor attached to the sacrum. Clinical impression was sacrococcygeal teratoma. Resected tumor mass was a relatively well circumscribed cystic mass, measuring 11 x 11 cm and the cystic content was previously evacuated. Inner surface showed ragged appearance, and focal nodular solid area was noted. On microscopic examination, the tumor revealed the derivatives of three germ layers and main components were tissues of respiratory tract. Sections from the solid area showed a well differentiated adenocarcinoma and with stromal, capsular and perineural invasion. Borderline malignant epithelial lesions are also noted suggesting the pathogenesis and progression of this tumor. There was no recurrence for one year after surgery in the follow up.

Esophageal granular cell tumor of is rare neoplesin, and 3 cases were reported in Korea so far. We report a case of esophageal granular cell tumor in a 28 year old man. The patient was a 28 year old man who visited this hospital because of upper abdominal pain, acid belching and weight loss. A small nodule, 5 mm in diameter, was found in mid esophagus on esophagoscopy. This nodule was proved to be a granular cell tumor.
