1.Large recession of one lateral rectus muscle.

A large recession of one lateral rectus muscle for exotropia is an infrequently used procedure. In this prospective study, 27 patients (3 to 19 years, mean age of 7) with moderate-angle exodeviation (18-35delta) were treated with large recession (8mm-9mm) of one lateral rectus muscle on their non preferred eye. Initially, there was underaction of the lateral rectus muscle. Within 6 weeks, the lateral rectus muscle regained full abduction, incomitance resolved, and the deviation was eliminated or reduced to a small phoria. Since surgery is confined to the deviating eye alone, operating time, length of anesthesia, and postoperative discomfort is reduced. The average amount of prism diopters needed for correction following operation for 8mm, 8.5mm and 9mm were 20.4delta, 26.4delta, and 31.3delta respectively and esthetically satisfactory results (within 10delta exodeviatior) were obtained in 90.I% of the patients.

2.Effect of External Environments on Urinalysis and Bacteriologic Culture.

The diagnosis of urinary tract infection is established by clinical symptoms and signs, urinalysis and urine culture. Urinalysis and urine culture are influenced by external environmental factors, especially temperature and time. So we have performed an assay for these effects on urinalysis and urine culture in 30 consecutive patients with urinary tract infection. Urinary specific gravity, pH and chemical components (protein, sugar, bilirubin, ketone and nitrite) were not influenced with environmental factors (temperature and time), but cellular components (RBC, WBC) were gradually decreased in number with increasing time in all temperatures without significance(p>0.06). Urine colony counts were not influenced significantly at 4 degrees C, but were increased in 4 hours at 18 degrees C and in 2 hours at 30 degrees C significantly (p<0.05). So, bacteriologic survey for urinary tract infection must be performed at least within 2 hours after collection of urine.

6.Role of Protein Kinase C in Abnormal Proliferation of Vascular Endothelial Cell induced by 1,2-Dimethylhydrazine; Analysis of Isoform.

PURPOSE: Protein tyrosine kinase(PTK), protein kinase C(PKC), oxidase, as a mediator, have been known to take a role in signal transduction pathway of angiogenesis. The authors confirmed that PKC is the most noticeable mediator for abnormal proliferation of vascular endothelial cells through in vitro study model using the inhibitors, targeting the formation of three co-enzymes. In this study, we would investigate which isoform of PKC play an important role in abnormal angiogenesis of vascular endothelial cell. METHODS: In 96 well plates, 10(4) HUVECs(human umbilical vein endothelial cells) were evenly distributed. Two groups were established; the control group without administration of DMH(1,2-dimethylhydrazine) and the DMH group with administration of 7.5x10(-9)M DMH. RNA was extracted from vascular endothelial cell of each group and expression of the PKC isoform was analyzed by RT-PCR(reverse transcriptase-polymerase chain reaction) method. RESULTS: RT-PCR analysis showed that PKCalpha, -betaI, -betaII, -eta, -micron and -zeta were expressed in vascular endothelial cells of each group. DMH incresed the expression of PKCalpha and PKCmicron, and decreased PKCbetaI, PKCbetaII expression dominantly. CONCLUSION: Based on the result of this study, it was suggested that PKCalpha and PKCmicron may have significant role in abnormal proliferation of vascular endothelial cell.

7.Effect of Prostate Size on Cancer Detection Rate of Traditional Sextant Prostate Biopsy.

PURPOSE: We aimed to evaluate whether cancer detection rates could vary with prostate size using a systematic sextant biopsy. MATERIALS AND METHODS: We performed a systematic sextant biopsy in 225 consecutive men from January 1997 to August 2002. Indications for biopsy included an elevated prostate specific antigen(PSA) level(more than 4.0ng/ml), or the hypoechoic lesion on TRUS. Prostate volume was calculated using the following fomula: volume=0.52 x length x width x height. We examined biopsy yield according to gland-volume intervals of 10cc. RESULTS: Adenocarcinoma was detected in 68 out of 225 patients(30.22%). The mean volume of prostate in all patients was 51.5cc. The highest biopsy rate(65.7%) was recorded among men with prostates between 20 to 29.9cc. The lowest biopsy rate (17.2%) was recorded among men with prostates larger than 80cc. Decreasing yield of sextant biopsy was strongly associated with increasing gland volume(p<0.001). The biopsy rate was significantly higher in patients with smaller prostates(<30cc) than in those with larger prostates(> or =30cc)(62.63% and 22.09%, respectively, p<0.001). Age, serum PSA, PSAD, and Gleason grade were comparable between small (<30cc) and large(> or =30cc) prostate groups. CONCLUSION: Our cancer detection rate using a systematic sextant biopsy was higher in men with prostates less than 30cc. Our results suggest that significant sampling error may occur in men with large glands, and more biopsies may be needed under these circumstances.

9.4 Cases of Hemi-Kock Ileocystoplasty.

Augmentation cystoplasty is effective in the rehabilitation of the contracted bladder of small capacity caused by tuberculosis or interstitial cystitis refractory to more conservative methods of management. Genitourinary tuberculosis is the most common cause of contracted bladder in Korea and it may lead to considerable vesicoureteral reflux and progressive deterioration of renal function. The use of hemi-Kock ileal pouch with an afferent intussuscepted nipple is a suitable solution in patient in whom the bladder outflow tract provides adequate resistance and supratrigonal cystectomy is to be performed with construction of an antireflux mechanism. Herein, we report 4 cases of hemi-Kock ileocystoplasty.
