2.Air Shadow Containing Mass Lesion in the Chest.

A 70-year-old female Patient was admitted due to the impression of lung abscess via local clinic. Chest P-A showed air shadow containing huge mass lesion in the right lower lung field, pericardio phrenic angle. But the patient complained of only mild upper respiratioy tract infection symptoms and the laboratory tests were within normal limits. Barium enema and chest C-T taken right after barium enema were performad in suggesting of the diaphragmatic hernia. Barium tilled transverse colon and associated omentum were in the right anterior hemithorax surrounded by the hernial sac. The mass lesion shown in the chest P-A was compatible with the diaphragmatic hemia, Morgagni type. Operative reduction of the herniated bowel and simple closure of the diaphragm was performed.

3.A Case Report of Asthmatic Breathing with Lidocaine .

Lidocaine is considered a safe and effective local anesthetic in anesthesiology and is the standard to which new amide anesthetics are compared. As it an amide derivative, and the frequency with which lidocaine elicits an allergic response is much less than that of anesthetics derived from paraaminobenroic acid esters. Case reports od sensitivity to lidoeaine have been reported, but they are rare. This 29 year old male patient had on episode of asthmatic breathing after supraclavicular brachial plexus block using 30 ml of 1% lidocaine with epinephrine(1:200,000). We report a case of asthmatic breathing due to lidocaine and a review of the literature for allergy local anesthetics.

5.Dietary assessment according to frequency of food consumed away from home among children and adolescents: Based on the 2010~2012 Korea National Health and Nutrition Examination Survey.

PURPOSE: The aim of this study was to investigate the dietary quality of food consumed away from home among Korean children and adolescents. METHODS: Data were obtained from the 2010~2012 KNHANES (Korea National Health and Nutrition Examination Survey) and included 3,583 subjects aged 7 to 18 years old. The frequency of food consumed away from home was dichotomized into more than once or less than once per day. RESULTS: Thirty percent of subjects were shown to consume food away from home more than once per day, and older children from higher income households showed a higher frequency of food consumed away from home compared to other children. The percentage of children and adolescents cited as regularly skipping breakfast was 19%. Individuals who ate out more than once per day showed higher consumption of energy and sodium than those who ate out less than once per day. CONCLUSION: Excessive intakes of energy and sodium are associated with obesity, high blood pressure, diabetes, and obesity-related diseases. Thus, there is a need for nutritional intervention and educational efforts to improve child nutrition and prevent obesity.

6.Dietary assessment according to intake of Korean soup and stew in Korean adults: Based on the 2011~2014 Korea National Health and Nutrition Examination Survey.

PURPOSE: This study was conducted to conduct dietary assessment of Korean adults according to intake of Korean soup and stew. METHODS: To accomplish this study, 20,926 adults aged 19 yr or higher who participated in the dietary intake survey (24 h recall method) were analyzed from the data of the 2011~2014 Korea National Health and Nutrition Examination Survey (KNHANES). The items included in the soup and stew were guk, tang, jjigae, jijimi, and jeongol. RESULTS: Intakes of soup and stew of subjects were divided into quartiles; intake range by quartile was Q1: < 22.96 g, Q2: 22.96~98.75 g, Q3: 98.75~212.23 g, and Q4: ≥ 212.24 g. In the case of the Q4 group, male, married, employed, higher educated, and high income subjects showed increased intakes of soup and stew. In addition, sodium intake among nutrient intakes increased from 3,849.04 mg in Q1 group to 5,363.57 mg in Q4 group. Intakes of cereals/grains, potatoes/starches, legumes, seeds/nuts, vegetables, mushrooms, fruits, meat, fishes/shell fishes, milks/dairy products, oils/fats, and seasonings among all foods significantly increased from Q1 group to Q4 group. Lastly, in the multivariable regression analysis, male, higher age, married, eating breakfast, consumption of snacks, prepared meals from home/institution, average eating-out frequency per week, energy consumption, and sodium intake related parameters increased intake of Korean soup and stew. CONCLUSION: Excessive intake of sodium was related to high blood pressure and metabolic syndrome. Thus, there are needs to improve dietary guidelines and nutrition education for balanced intake of soup and stew.

7.Recognition and attitude to fundtional division between physicians and pharmacists of practising physicians and pharmacists in Taegu city.

Mail questionnaire was administrated to 370 practising physicians and 388 pharmacists in Taegu city selected by systematic sampling to examine utilization states and opinion of pharmacy under medical care insurance programme and the attitude to the functional division between physicians and pharmacists from April to May 1992. Regarding the opinion on the outcome of drug-store under medical insurance, 71.2 percent of practicing physician answered failure but 13.4 percent of practicing pharmacists answered failure in contrast. Fifty percent of practicing physician asserted introducing functional division between physician and pharmacist while 66.9 percent of practicing pharmacist answered drug-store under medical insurance itself is successful programme. Average daily numbers of preparation of medicine was 32.2 case. Percentage of utilization of drug-store under medical insurance to average daily cases of preparing of medicine was 20 percent, percentage of utilization with physician's prescription was 0.7 percent. And 58.7 percent of practicing physician experienced outside the institute prescription. Regarding the opinion on the pros and cons of enforcing functional division between physician and pharmacist, 59.2 percent of practicing physician preferred pros and 17.7 percent cons ,but 38 percent of practicing pharmacist preferred pros and 45.5 percent cons. And pharmacist know better the content of functional division between physician and pharmacist, practicing emphasized to prevent misuse or abuse of medicine but practicing pharmacist emphasized to display physician and pharmacist's professional ability. And as an opinion on implementation style of functional division between physician and pharmacist in pros respondents, practicing physician favored mandatory enforcement (52.3%), while practicing pharmacist favored partial incomplete functional division (81.7%). As the method of prescription if functional division between physician and pharmacist will be enforced, both practicing physician and pharmacist preferred generic name (44.0%, 89%) mostly, but physician preferred brand name (35.3%) secondly. Regarding the reason for not implementing functional division between physician and pharmacist up to date, both physician and pharmacist answered problem of business right between physician and pharmacist, followed by lack of recognition, and interest of people and lack of the governmental willness. Regarding the opinion on prior decision of condition for enforcing functional division between physician and pharmacist, practicing physician and pharmacist named uneven distribution of medical facilities and drug-store between rural and urban, inequality of physician and pharmacist manpower and the problem of manpower demand and supply mostly, and practicing physician pointed out establishing attitude of acceptance on the part of pharmacist and practicing pharmacist favored establishing attitude of acceptance on the part of physician, which was different attitudes between physician and pharmacist. Following conclusion was reached; 1. Current drug-store under medical insurance program yield insufficient outcome, so we should consider program conversion from drug-store under medical insurance program to functional division between physician and pharmacist. 2. There were problem of business right and conflicts between physician and pharmacist at enforcing functional division between physician and pharmacist, so the government should search for formulating plan to resolve the problem and have neutral willness for the protection of the national health.

9.Total Spondylectomy of a Lumber Vertebra with Giant Cell Tumor: One Case Report

Giant cell tumor of the spine is regarded as a cllnically mallgnant tumor because of its difficulty in diagnosis, cord compression and its frequent recurrence. The consensus of current report on giant cell tumor is that local resection is the treatment of choice where it is feasible. In the spine, however, because of its inoperable location, irradiation has become an accepted mode of treatment, although the results of which are at best uncertain. Total spondylectomy Is a formidable undertaking and its report is scarce. In 1966, Lievre reported total spondylectomy of the fourth lumber vertebra. In 1968, Stener and Johnsen performed total spondylectomy of three vertebral bodies, the eleventh thoracic through the first lumbar. Recently, in the summer of 1979, Edwards reported prosthetic replacement after total spondylectomy. This report presents our experience with giant cell tumor of the thlrd lumbar vertebra, which was successively treated by an excislonal biopsy through anterior retroperitoneal approach, complete removal of the vertebral body and interbody fusion of the adjoining vertebrae through an anterior medlan transperitoneal approach, and finally by excislon of the remaining posterior elemsnt and a posterior H-graft supplimented by loop wire flxation. At six month follow-up, fusion was solid and there was no exidence of recurrence of the tumor.

10.Volar Plate Arthroplasty of Proximal Interphalangeal Joint of Finger

Pain, premature degenerative arthritis, stiffness and persisting subluxation are common sequalae of fracture dislocations of the proximal interphalangeal joints of the fingers. With disruption of greater than 40% of the volar articular surface of the middle phalanx, it is un- likely that congrous stable reduction can be achieved. And the most severe injuries are caused by longitudinal compression force, which drives the middle phalanx where the middle phalanx eventually comes to rest. Many authors had reported a variety of method for the treatment of the unstable fracture dislocation with no satisfactory results except Eaton(1980). In his ten years experience the volar plate arthroplasty for acute and chronic fracture dislocations got a good results with satisfactory range of motion and no pain on motion. Authors had been treated two cases of chronic fracture dislocations of the proximal interphalangeal joint with Eatons volar plate arthroplasty followed by good results in one year followed-up study.
