1.Vitamin K-Deficient Hemorrhagic Disease in Infants with Acute Subdural Hematoma.

We had experienced 2 cases of acute subdural hematoma due to vitaimin K(vit. K) deficiency. They were 44-and 42-day-old. After the adminstration of vit.K1, prolonged prothrombin time(PT) and activated partial thromboplastin time(APTT) were corrected, so the surgical treatment ws performed. We supposed the predisposing factors were breast feeding and unknown hepatic pathology. No complication remained to one infant but the other multiple cerebral infarctions. The etiology, pathogenesis, diagnosis, prevention are discussed. We insist on neonatal administration of vit. K1 for the prevention of bleeding tendency. If intracranial hemorrhage develops, we must keep in mind that the early neurosurgical intervention may be needed.

2.Histochemical study on trematodes - Distribution of carbonic anhydrase activity.

The purpose of the present study is to demonstrate the distribution of carbonic anhydrase pattern in the various termatodes (Fasciola gigantica, Paramphistoma orthocoelium, Paragonimus westermani) by means of Kurada staining method, and to correlate these findings with the histochemical data and harboring location. The results are summarized as follows: In Fasciloa gigantica, carbonic anhydrase activity was positive in the vitelline gland cells and eggs in the uterus. In Paramphistoma orthocoelium, carbonic anhydrase activity was positive in the vitelline gland cells and eggs in the uterus. In Paragonimus westermani, carbonic anhydrase activity was positive in intestinal mucous membrane, vitelline gland cells and eggs.

6.Simple Advertent Hysterectomy in the Presence of Invasive Cervical Cancer.

To identify significant prognostic factors in patients undergoing simple hysterectomy in the presence of invasive cervical cancer, the records of 45 patients who had taken such a procedure between 1993 and 1997 were reviewed. Overall relapse-free survival and 5-year survival rates were 91.1 and 92.1%, respectively. Factors found to be significantly related to survival were the retrospectively determined stage(p=0.0000), the presence of residual disease(p=0.0001), and cell type(p=0.0000). By multivariate analysis, factor emerging as significantly detrimental to survival was the cell type. The presence of residual disease was a marginally significant factor(p=0.067). The expectations for survival of patients with residual tumor mass and/or with adenocarcinoma after simple hysterectomy appear to be markedly worse than those with others, so radical reoperation should be considered in those patients.

7.Study on the Incidence of Pulmonary Embolism before and after Hip and Knee Replacement Arthroplasties

Hip and knee replacement arthroplasty is one of the important causes of pulmonary embolism, of which the incidence was reported to be from 2 to 16% But, there are no reports about the incidence of pulmonary embolism after these operations in Korea. The authors' intention was to study the incidence and clinical manifestations of pulmonary embolism after hip and knee surgery. Lung perfusion scans and inhalation scans were done preoperatively and 7 days postoperatively for 139 hip or knee replacement arthrosplasty patients in Asan Medical Center. We studied 62 cases of avascular necrosis of the femoral head, 33 cases of femur neck fractures, 45 cases of degenerative arthritis of the hip and knee joints and 17 cases of other diseases. In this study, hip replacement arthroplasties were done in 114 cases, knee replacement arthrosplasties in 25 cases. On preoperative lung scan, high or intermediate probability findings of pulmonary embolism were shown in 18 cases and all of them were asymptomatic. 139 cases showed normal or low probability on preoperative lung scan, of which 5 cases were changed to high probability and 7 cases to intermediate probability on postoperative lung scan. Four of the 7 intermediate probability cases were confirmed to be pulmonary embolism on pulmonary angiography. Of these 9 cases, hip replacement arthroplasties were done in 6 cases and knee replacement arthroplasties in 3 cases. There were 5 cases of osteoarthritis of hip and knee, 3 cases of avascular necrosis of femoral head and 1 case of femoral neck fracture. We concluded that preoperative and postoperative incidences of pulmonary embolism in Korea were similar to those in western countires. Most of the pulmonary embolism patients were asymptomatic and so lung perfusion scans and inhalation scans were helpful in the early diagnosis of pulmonary embolism.

8.Statistical Analysis of Twin Pregnancy.

Twin pregnancies represent a high risk obstetric population with perinatal morbidity and mortality much greater than that of singleton pregnancies. A retrospective dinical evaluation was attempted to analyse 211 cases of twin pregnancies in Chunchon Sacred Heart Hospital of Hallym University from Jan. l. 1988 to Dec. 31. 1997. The results were as follows. 1) The incidence of twin pregnancy was 1 in 93.9 deliveries. 2) The age of mother ranged from 19 to 42 years and predominant age group was 26-30 which covers 43.1% of all. 3) Nullipara was 61.1% and multipara was 38.9%. 4) The predominant gestational age group at delivery was 36-37 weeks, 33.2%. 5) In presentation, both cephalic combination was 46.4%, cephalic-breech was 26.1%. 6) The birth weight was less than 2500gm in 64.9% of the 1st babies and 70.6% of the 2nd babies, respectively. 7) The rate of cesarean section was 89.0%. The indications of cesarean section were malpresention (48.9%), elective cesarean section(38.8%), and toxemia(9.0%). 8) In 23 cases of vaginal delivery, the average duration of labor was 7.0 hours and 43.5% of 2nd baby was delivered within 5 min after 1st baby delivery, 9) The most frequent maternal complications during pregnancy were anemia(36.0%), preterm labor(34.6%) and preeclamsia(17.5%). 10) The perinatal mortality rate of the first and second twins was 10.4% and 12.3%, respectively. The main causes of perinatal death were prematurity(52.1%) and stillborn(22.9%). 11) Both female was 43.6%, both male was 39.3%, and opposite sex was 17.1% in infant.
