1.3 Cases Report of Myositis Ossificans Progressiva

Myositis Ossificans progressiva is a very rare disease and not more than five cases have been reported in this country. The symptoms usually appear before the age of six and no case has ever been reported in medical literature in which patient is older than the age of twenty. This author reports here that a 52 years old male was diagnoed as a myositis ossificans progressiva. The patient developed his symptoms when he was lynched by a club during the Korean conflict at the Korean conflict at the age of thirty. Another interesting fact is that is that his daughter has also developed similar symptoms which appeared following an accident when she was run over by an angry cow at the age of six.

2.A case of Melkersson-Rosenthal syndrome.

Melkersson-Rosenthal syndrome(MRS) is a rare neuro-muco-cutaneous disease of unknown origin. The classic triad of this clinically defined entity consists of orofacial swelling, facial nerve palsy, and lingua plitica. MRS may occur as a complete triad of symptoms or a combination of any features of the classic triad, termed monosymptomatic and oligosymptomatic forms. The complete triad has been reported to occur in only 10% to 20% in different series. Because of the rarity of reported cases in Korea, we report a case of complete form of MRS, in which clofazimine showed a partial response.

3.Effect of Persistent Mild Hyperglycemic Hyperinsulinemia on Development of Insulin Resistance in Rats.

The effect of persistant mild hyperglycemic hyperinsulinemia on the development of the insulin resistance in rats was studied in vivo. Also, the characteristics of the insulin resistance compared with the insulin resistance of STZ diabetic rats. Persistant mild hyperglycemic hyperinsulinemic rat model was produced by ingestion of glucose polymer for 8 days. The glucose disappearance and infusion rate was measured by hyperinsulinemic euglycemic clamp to"Imique at steady state of blood glucose and insulin levels. The clamped level of blood glucose was 100 mg/dl, and the clamped levels of insulin were 70 pU/ml (physiologic condition) and 3000 pU/ml (supramaximal condition). Hepatic glucose producticon rate was calculated using measured data. And the glycogen synthetic capacity of skeletal muscle(soleus) and liver was measured after 2 hours of hyperinsulinemic euglycemic clamp study. The glucose disappearance and glucose infusion rate in glucose polymer group was decreased in the both physiological and supramaximal insulin level compared to the rate of the normal control group. The rate of STZ diabetic group wase lowest at supramaximal insulin level among two another experimental groups. The hepatic glucose production rate of glucose polymer group was decreased compared to normal control but increased in STZ diabetic group.

4.A Study on the chemotherapy in clonorchiasis. Report 1. An experimental study on chemotherapy with Dithiazanine iodide and Bithionol sulfoxide in clonorchiasis.

Authors carried out experimental study on chemotherapy with oral administration of Dithiazanine iodide (D.I.) and Bithionol sulfoxide(B.S.) in rabbit clonorchiasis. And the following result was obtained. In change of the E.P.G (eggs per gram feces) by D.I. administration, it was rather increased in early stage of the administration than prior to administration, and thereafter decreased gradually. In the change of the C.S. worm body by D.I. administration, there was not only prominent change of supporting tissue but also the change of reproductive organ was found. In considering the wormicidal effect of D.I. from detecting rate of survival worms, the effect was slight in group of 100 mg(80 mg/kg) per day dosage, but the effects were very excellent and almost complete by proper times of administration in groups of 200 mg (130 mg/kg) per day or higher dosage. But the side effect and intoxication sign of D.I. were appearent in groups of 200 mg or higher dosage. By B.S. administration, E.P.G. was decreased gradually. In changes of the C.S. worm body by B.S. administration, prominent inhibitory chnnge was seen in egg formation ability. Slight wormicidal effect of B.S. was observed in groups of 140 mg (100 mg/kg) per day or higher dosage. Side effect and intoxication sign of B.S. were found little in groups of 140 mg or lesser dosage.

5.Insulin Resistance in Late Pregnant Rats.

The influence of normal late pregnancy on insulin action and insulin secretion was studied in the Sprague-Dawley female rats. On 20th day after mating, intravenous glucose tolerance test(IVGTI) was performed in non pregnant control and pregnant rats. As results of IVGTT, glucose disappearance rate was not significantly different in both groups, but secretory response of insulin was significantly(p<0.05) increased in pregnant rat. And the ratio of insulin/ .glucose was significantly higher in pregnant rats, which means existence of insulin resistance. These insulin resistance was overcomed by increased secretory response of pancreatic insulin. Insulinogenic index( A insulin/glucose - 5 min) was highly significantly (r=0.62, p<0.01) correlated with progesterone concentration. Glycogen level and amounts of "C-glucose incorporated into glycogen after IVGTT were significantly(p<0. 05) decreased in the liver, but were not changed significantly in soleus. Glycogen synthase activity of soleus and liver was not differ significantly in the both groups. Insulin binding at varying concentrations of insulin to crude membrane of pregnant liver was not significantly different from control. In conclusions, although these pregnant rats were normal glucose tolerance due to increased secretory response of insulin, that was correlated with progesterone concentration, pregnant rat had insulin resistance. The mechanisms of insulin resistance were not related to defect of insulin binding phase and glycogen synthase, but suggest pre-receptor and/or postreceptor phase.

6.Studies on paragonimiasis in Ko-Hoong County, Korea (Report 1.). An epidemiologic survey for human paragonimiasis by the use of intradermal screening test.

Authors carried out the survey for human paragonimiasis in Ko-Hoong County, Chun-Nam Province, Korea from July to September 1964 by the use of intradermal screening test with Veronal buffered saline antigen. Eight myuns (village) out of 13 myuns of this county were selected for the survey, regarding balanced selection. The number of 2,916 primary school children of high class and 764 residents of all age groups in these areas were examined (total 3,680; male 1,961; female 1,719). The following results were obtained in this survey. As a whole, 20.1 per cent of the examiners showed positive reaction, with sexual difference of 20.9 per cent in male and 19.1 per cent in female. In regional differences of positive rate, the highest rate(50.1 per cent) was showed at Po-Doo myun and lowest rate(3.6 per cent) was showed at Do-Yang myun. In age group, the highest positive rate was showed in 16 to 20 age group (46.7 per cent). In regional comparison of paragonimiasis infectivity of whole nation (province and county), Ko-Hoong County as nearly same as the Hea-Nam County, Chun-Nam Province follows next of Cheju Province, the highest prevalance area of paragonimiasis in Korea, in concentration of prevalence of paragonimiasis.

7.The effect of regular physical exercise on glucose uptake in soleus and intravenous glucose tolerance in streptozotocin diabetic rats.

The effect of exercise on plasma insulin, free fatty acid, and glucose uptake and glycogen concentration in soleus, and intravenous glucose tolerance of streptozotocin treated, diabetic Sprague-Dawley rats were studied. Diabetic-trained animals were subjected to a regular program of treadmill running for 4 weeks. Seventy-two hours after the last training session, basal and insulin-stimulated glucose uptake was studied in incubated strips (about 20 mg) of soleus muscle in vitro. Glucose tolerance was measured with intravenous infusion of 0.5 g glucose/kg body weight. In diabetic rats, training was associated with increase glucose uptake in basal and maximal insulin concentrations, decreased fasting glucose concentrations, and increased muscle glycogen levels, but there were no changes in glucose tolerance curve and plasma insulin concentrations. These results suggest that regular running program for 4 weeks improve responsiveness of insulin on soleus muscle, but fails to cause improvement of impaired intravenous glucose tolerance in mild degree streptozotocin induced diabetic rats.

9.Comparative Analysis of Simultaneous and Staged Bilateral Total Knee Arthroplasty

Between January 1982 and September 1986, at the Department of Orthopaedic Surgery of Kyung Hee University Hospital, 82 total knee arthroplasties were performed in 57 patients. We studied 25 patients (50 Knees) who had been operated by bilateral total knee arthroplasties to compare one stage and two stage operations. The follow-up ranged from 10 months to 5 years and 7 months (average 1 year and 8 months). All operations were performed by one of the authors (D.K.B.), but the follow-up study and the entire medical record were independently evaluated by another authors. The clinical results of preoperative and postoperative assessments and rehabilitation processes are as follows; 1. Group I (10 patients) had both kness replaced by one stage simultaneous operation under same anesthesia, and Group II(15 patients) had replaced by two stage operation at the intervals of 2 weeks. 2. The periods required to do bilateral weight bearing were 10.3 days for Group I, 23.6 days for Group II. 3. Complications in one stage operation was no more than the in two stage operation. 4. The postoperative pain and ROM were not significantly different in both groups. 5. Using the hospital for special surgery rating system, we found the clinical results to be similar in both groups. 6. Comparing to two stage operation, the advantages of one stage operation were shorter operative and tourniquet time, lesser use of antibiotics, reduced medical cost and hospital stay. In conclusion, if patient conditions do not prevent prolonged anesthetic time and surgeons have good surgical techniques, we recommend one stage simultaneous operation in bilateral total knee replacement.

10.Acute epiglottitis in adult

Acute epiglottitis is an inflammatory process of supraglottic region, which involving the epiglottis andhypopharynx region. This disease can be recognized easily radiologically and virtually all death are preventable.The lateral neck radiograph with soft tissuse technique is of critical value when the diagnosis is in doubt andusually characteristic when compared to the normal. Authors reviewed available lateral neck radiograph of 92 casesamong the 95 cases of clinically confirmed adult epiglottitis during 4 years in wallace Memorial Hospital. Theresults are as follows; 1. Male to female sex ratio was 13:2. 2. The most common age group was 4th decade. 3. Themost common clinical symptoms are sore-throat (87%) and dysphagia(87%). 4. The radiologic findings are swelling ofhypopharynx(92%), vallecular narrowing or obliteration (92%), uvula swelling (33%), narrowing of airway adjacentto epiglottis (17%), swelling of prevertebral soft tissue(8%).
