2.Efficacy and Safety of Nifedipine Gastrointestinal Therapeutic System(Adalat OROS) in Patients with Mild to Moderate Essential Hypertension.

BACKGROUND: This study was designed to evaluate the antihypertensive efficacy and safety of nifedipine gastrointestinal system push-pull osmotic pump formulation in patients with mild to moderate essential hypertension. METHODS: After 2 weeks placebo run-in period, nifedipine 60 mg was administered once daily for 8 weeks in thirty-two patients with mild to moderate essential hypertension. RESULTS: At the end point of treatment, sitting blood pressure decreased as much as 33/16mmHg in average and rewarded 88% of efficacy and 69% of normalization. The ambulatoy blood pressure monitoring in 10 patients also revealed 11% decrease in 24 hr mean diastolic blood pressure and 32% decrease in % elevated blood pressure. The most frequent side reactions were constipation in 7 patients and polyuria in 6 patients which were tolerated during entire trial period. There were no significant changes in biochemical parameters and hematologic data, thus making the rate of safety 91% and overall rating of usefulness was 84%. CONCLUSION: Nifedipine GITS 60 mg once daily regimen is well tolerated and effective in the treatment of mild to moderate essential hypertension.

5.Transarticular Screw Fixation in Atlantoaxial Instability.

Atlantoaxial joint is a complex and unique structure which promotes rotation of head. Disruption of the transverse ligament and odontoid process fracture caused by trauma result in atlantoaxial instability. The instability must be corrected to relieve pain and to reduce the risk of neural damage and possibility of sudden death. The most recent and technologically advanced fixation method of C1-2 instability is the use of C1-2 transarticular screw fixation. The transarticular screw fixation augmented with interspinous wiring technique provide immediate multidirectional rigid fixation and increase the fusion rate of atlantoaxial instability. It can be employed in situations where the posterior arch of C1 is absent or fractured and in situations where standard wiring techniques pose risky posterior subluxation of C1 on C2, or for congenital odontoid anomalies. Twenty-one patients with atlantoaxial instability were operated with posterior transarticular screw fixations augmented with interspinous C1-2 strut graft and posterior wire fixation technique. All patients restored C1-2 stability without complication. This technique seems to be superior biomechanically when compared with other posterior fixation methods. Precaution is needed to avoid the vertebral artery injury.

Primary aldosteronism, characterized by hypertension, hypokalemia, and hyperaldosteronemia resulting from chronic oversecretion of aldosterone independent of normal renin-angiotensin regulatory system, is due mostly to aldosteronoma or to bilateral cortical nodular hyperplasia. We report two cases of primary aldosteronism due to adrenal cortical adenoma, which were diagnosed by clinical data and abdominal computed tomographic scan. Clinical symptoms and laboratory data returned to normal after surgical adrenalectomy.

10.Regrowth of Posterior Communicating Artery Aneurysm after 13 Years of the First Clipping: A Case Report.

Microsurgical neck clipping has been believed to be the most reliable treatment for the intracranial aneurysm. However, recurrence or regrowth from so called "completely clipped aneurysm" with catastrophic outcome has been infrequently reported. We describe an unusual case of regrowth of posterior communicating artery aneurysm which was found 13 years after the first successful clipping at the same site. Surgeons should pay attention to the possibility of such a regrowth even though the initial clipping seemed to be perfect. Additional deliberate postoperative angiography might resolve this late consequence.
