2.A Clinical Study of Hyperbaric Oxygenation Therapy .

Authors observed the 242 patients who were treated with hyperbaric oxygenation therapy from September, 1972 to May, 1978 at our department of B.N.U.H. The results were as follows. 1) Sex distribution was 106 male and 136 female, and the age group was between 10 and 39 year old, which occupied 71. l% of the total cases (172 cases). 2) With regard to distribution by month, 206 cases were in November to April, using coal briquette as the main fuel of heating systems. 3) The patients with CO intoxication were 232 cases(95. 9%) and the other 10 cases included Buergers disease, gas gangrene, sudden deafness and uncommon dermatologic disorder's such as scleroderma and pyoderma gangrenosum. 4) 188 cases(77. 7%) were treated by one time hyperbaric oxygenation therapy and 30 cases(12. 4%) twice, reflecting that most cases were able to be treated only once or twice by hyperbaric oxygenation therapy. Therapy 10 to 49 times was applied to Buerger's disease and sudden deafness, and over 50 times to 2 cases of dermatologic disorders. 5) We could not notice convulsion due to oxygen toxicity, a complication of hyperbaric oxygenation, but tinnitus and aspiration pneumonia were observed in 6 cases(2. 5%).

3.Ghrelin Levels During Puberty: Relationship with Anthropometric Measures and Other Hormones.

PURPOSE: Puberty is a rapid changing periods of body composition and endocrine system. Already leptin was known to be closely related to body composition and was known to change according to Tanner stage. Ghrelin, a natural GH secretagogue, was also known to be related with weight change, fat utilization, insulin secretion, and influenced by sex steroid. But the relationship between ghrelin and other parameter according to pubertal development is not established yet. METHODS: Cross-sectional study of 101 peripubertal children (54 males and 47 females) aged 8.5 to 17.0 years. We examined body composition (free fat mass, fat mass, % body fat) by bioelectric impendence measurements. Total ghrelin, leptin, insulin was measured by radioimmunoassay (RIA). Relationship between ghrelin, leptin, insulin and body composition data was analysed according to Tanner stage controlling gender using SPSS ver 11.0. RESULTS: Ghrelin concentration is only correlated with Tanner stage (r=-0.21, P<0.05) and GIR (r=0.29, P<0.01). When we divided subjects by sex, even though there was no difference of ghrelin level between both sex, only male is correlated with Tanner stage (r=-0.28, P<0.05) and GIR (r=0.36, P<0.01). There was no difference of ghrelin level between each Tanner stage and no correlation was observed with other body composition and endocrine parameter including leptin. CONCLUSION: These data suggest that Ghrelin levels decrease during puberty at least in boys as pubertal development proceeds but it is minimal. During puberty, ghrelin levels are stable. Ghrelin may have no relationship with other pubertal change. The relation of ghrelin and insulin sensitivity need to be further investigated.

5.Simple Radiographic Finding of Subacromial Impingement Syndrome.

PURPOSE: We evaluated both the patients and the normal volunteers to determine the diagnostic criteria of subacromial impingement syndrome. MATERIALS AND METHODS: We analyzed the radiologic finding of Thirty degree of caudal tilt view (TCTV) and Supraspinatus outlet view (SOV) of 100 shoulders from 85 patients with clinically proved subacromial impingement syndrome and normal 100 shoulders from 60 volunteers. RESULT: In TCTV, the protrusion of acromion below the line of extension from inferior surface of clavicle was shown in 94% of the patient group and 48% in normal group. Sharp tip of acromial protrusion was detectable in 55.3% of the patient group and 10.4% in normal group. In SOV, curved type of acromion was seen in 53% of the normal and 50% in patient group. Hooked type of acromion was detected in 3% and 31% of the normal and patient group, respectively. CONCLUSION: Protrusion of acromion at TCTV itself was not a criteria of subacromial impingement syndrome, but more than 7 mm below the line of extension from inferior surface of clavicle was meanigful. In SOV, hooked type of acromion was a criteria of subacromial impingement syndrome but curved type is was not a finding of diagnostic significence. Acromial spur formation on TCTV and SOV was important criteria of subacromial impingement syndrome.

7.The Effect of Coculture with Human Oviductal cells on In Vitro Development of ICR Mouse Embryo.

OBJECTIVE: To examine the effects of coculture with human oviductal cells regarding the development of 1-cell stage ICR mouse embryos and to investigate the effects of duration and start time of coculture. MATERIALS AND METHODS: ICR mice were superovulated with PMSG/hCG and 1-cell stage mouse embryos were recruited. 1-cell mouse embryos were cocultured on human oviductal cells in a CO2 incubator(coculture group) and were cultured on 0.4 % BSA+HTF media(control group)(Experiment 1). 1-cell mouse embryos were cocultured on human oviductal cells for 36, 44, 52, 60 hours after hCG IP respectively, and then were transferred to 0.4 % BSA+HTF media(Experiment 2). In comparison, 1-cell mouse embryos were cultured by using 0.4 % BSA+HTF media, and then were transferred to human oviductal cell coculture system using the same schedule(Experiment 3). Afterward, they were examined regarding the development to 2-cell, 4~8 cell stage mouse embryos, morulas and blastocysts. RESULTS: In experiment 1, the developmental rates to 2-cell embryos of coculture group and control group were 97.3 % and 98.7 %, respectively. After 2-cell embryos, coculture group showed significantly higher developmental rate than control group (p<0.05). In experiment 2, the developmental rates after 2-cell embryos showed the significant differences. The groups with coculture effects removed before post-hCG 60 hours showed significantly lower developmental rates (p<0.05). In experiment 3, the developmental rates after 2-cell embryos were higher when the coculture started at an earlier stage. Furthermore, the groups which were cocultured from post-hCG 52 hours exhibited significant lower developmental rate than the groups which were cocultured continuously (p<0.05). CONCLUSION: The coculture with human oviductal cell could improve the development of the embryos in vitro and might mimic the natural physiological condition better than media environment. The degree of improvement was more pronounced when the coculture started at an earlier stage and the duration of coculture was longer. More importantly, the changes of culture condition at post-hCG 52 hours in which secondary mitosis occurs, have significant detrimental effects on growth and development of mouse embryos.

8.Expression of p53 and MDM-2 Protein in Cervical Neoplasia.

OBJECTIVES: MDM-2 is an oncoprotein that inhibits p53 tumor suppressor protein. Amplication and over- expression of its protein have been observed in human malignancies, and these abnormalities have a role in tumorigenesis through inactivation of p53 function. To elucidate the role of p53 and MDM-2 protein in cervical neoplasia we investigated the expression rates of MDM-2 and p53 protein in surgically resected specimens. METHEDS: Immunohistochemical studies using anti-p53 and anti-MDM-2 protein in the paraffin embedded section of 62 cases including cervical intraepithelial neoplasm(CIN) and invasive cervical cancer were performed. RESULTS: Expression rates of p53 protein were 25% in CIN I& CINII, 20% in CINIII, and 44% in invasive carcinoma, respectively. The MDM-2 protein were 33% in CIN I & CIN II, 16% in CIN III, and 48% in invasive carcinoma, respectively. There was no evident correlation between p53 positivity and MDM-2 positivity(p>0.05). However, correlation between MDM-2 negativity and p53 negativity was statistically significant(p=0.002) CONCLUSION: These data suggest that the expression of p53 protein is presumed to be necessarily correlated with MDM-2 protein expression in cervical neoplasia.

10.Patency Rate of Internal A-V Fistula for Hemodialysis in ESRD Patients

Hemodialysis that to maintain life quality and may be preliminary stage of kidney transplantation is essential in end-stage renal disease(Esrd) patients. Since 1996, Brescia-cimino are used to internal radio- cephalic fistula and this fistula method was the most popular in world-wide. And then variable methods (e.g, Autogenous, PTFE, Dacron etc.) were usually tried instead of above standard fistula. We experienced 75 fistulas in 62 cases from Jan. 1993 to Dec. 1996 and among 75 fistula operations standard radio-cephalic fistula was 56, brachio-cephalic fistula 10, graft fistula 9. Early patency failure rate of A-V fistula was 9 cases, 14.5%. Significant factors to effect in patency of A-V fistula were propably diabetic mellitus, venous diameter, graft material and were not related to age/sex, blood pressure, BUN/creatinine etc. In conclusion the reduction of early patency failure rate in A-V fistula is to maintain long patency rate of A-V fistula
