A case of thumb-in-palm deformity due to burn scar contracture. Recently it is felt that the incidence of thumb-in-palm deformity due to old burn scar contracture is far less than that of other causes; for example carebral palsy, trauma, Co. poisning etc; This case is a 23 years old male soldier who had sustained burn on the left palm at his age of 4, and the left thumb has been gradually contracted, resulting to established thumb-in-palm deformity evantually. There is no active or passive motion in the metacarpho-phalangeal phalangeal joint and carpho-metacarpal joint of left thumb. The distal phalanx is moderately atrophied and its nail is deformed. On act 6th, 1975 he was operated on soft-tissue releasing with Z-plasty fasciotomy of palmar aponeurosis and myotomy of adductor pollicis and flexor pollicis brevis with split thickness skin graft. Postoperatively he could have full extension of the left thumb as well as active flexion of metacrpho-phalangeal and interphalangeal joint with slight adducted state. He is able to take opposition of left thumb at the present time postoperative four weeks.

Between May 1982 and October 1983, arthroscopic meniscectomy had been performed in 54 knees between ages of 11 and 70 years. There were 32 male patients and 22 female patients in this series. The mean follow-up was 20 months, ranging from 15 months to 33 months. Postoperative complications developed were retropatellar tenderness and effusion. No infection, thrombophlebitis or instability occurred postoperatively. The aim of this study was to evaluate the effect of the various possible prognostic factors upon the developtnent of the above mentioned complications. The factors included were 1) shape of tear, 2) ligamentous injury, 3) degenerative arthritia, 4) discoid lateral meniscus, 5) quadriceps atrophy, 6) sites of meniscus(medial or lateral) & extent of excision, 7) preoperative retropatellar tenderness and 8) method of rehabilitation. The findings were statistically analyzed by the Chi-square test (df-1) & Fisher's direct probability method. The results obtained were as follows; 1. The rate of development of postoperative effusion was significantly high in cases of quadriceps atrophy (P<0.01) and preoperative retropatellar tenderness(P<0.01). 2. The rate of deveopment of postoperative retropatellar tenderness was significantly high in case of discoid lateral meniscus (P <0.01). 3. There was no significant correlation between complications and other factors including shape of tear, preoperative degerative arthritis, site of meniscus & extent of excision and methods of rehabilitation.

The May-Hegglin anomaly is a rare autosomal dominant trait characterized by platelet abnormalities in the peripheral blood and large (up to 5mu) pale blue staining inclusions in the cytoplasm of neutrophils, eosinophils, basophils, and monocytes. We experienced a case of May-Hegglin anomaly in a 7 year old male and another case out of his family. His initial complaints at the admission were petechiae and intermittent epistaxis. Diagnosis was confirmed by peripheral blood smear and family study. We report the case with brief review of related literature.

Bronchiolitis obliterans is a distinct pathologic entity, characterized by the accumulation of pigmented macrophages within respiratory bronchioles and adjacent to air spaces, and thickening of the peribronchial interstitium. It has been reported to be associated with viral infection, drug, toxic fume, bone marrow transplantation, and connective tissue disorders such as rheumatoid arthritis. The etiology of bronchiolitis obliterans in the post-renal transplantation state is not yet clear, although several possibilities such as drug toxicity, graft-versus-host disease or postinfectious condition have been postulated. We presented three patient who had bronchiolitis obliterans, as a complication following renal transplantation. Chest radiograph showed bilateral perihilar reticular infiltration or ground glass appearances that progressed to either diffuse alveolar consolidations or solitary nodule. The main finding in each lung biopsy was the presence of macrophages within respiratory bronchioles as well as in the neighboring alveolar ducts and alveoli. Alveolar septa in these areas often showed nonspecific thickening by fibrosis, mild chronic inflammatory cell infiltrate, and hyperplasia of alveolar lining cells and type II pneumocytes.

PURPOSE: The inhibitory effect of TGFbeta1 on survivals of L1210 and anticancer drug- resistant L1210 sublines was investigated and the gene expression of TGFbeta1 in these cells was examined. MATERIALS AND METHODS: The survivals of L1210, adriamycin-resistant(L1210AdR), vincristine-resistant(L1210VcR) or cisplatin-resistant(L1210Cis) cells were measured by MTT assay after treatment of TGFbeta1. Northern analysis was performed for TGFbeta1 gene expression in L1210, L1210AdR, L1210VcR or L1210Cis. RESULTS: There was no different survival ratio between two groups, control and TGFbeta1(10 ng/ml) treated groups in L1210 cells. However, the survival ratio of L1210AdR was 59% in TGFbeta1 treated group for 96 hours. The survival ratio of L1210VcR was 61% for 96 hours in TGFbeta1 treated group. The survival ratio of L1210Cis was 40% for 96 hours in TGFbeta1 treated group. Expressions of TGFbeta1 gene in drug-resistant sublines were significantly decreased than that of L1210 cells. CONCLUSION: Growth of anticancer drug-resistant L1210 sublines were inhibited by TGFbeta1 but not in L1210 cells. So, it is suggested that TGFbeta1 gene expression may have a part in anticancer drug-resistance.
