1.A Clinical Observation of Intussusception in Infants and Children.

Intussusception is most frequent and emergency condition of abdomen in infancy and childhood. But the causation of that is uncertain. In the method of treatment, primary barium reduction was recommended. This is a clinical observation of 51 cases of intussusception from January 1973 to December 1977 at Han-Il Hospital. The result were summarized as follows; 1) In age incidence, 72.55% of the patients were under 1 year of age. The highest morbidity (54.90%) occurred between 6 months to 12 months. 2) Males were affected more often than females ; the ratio was 1.8 : 1. 3) In seasonal incidence, 31.37% of patients occurred in Spring. 4) In artificial fed partients, the incidence was higher than others (54.35%). 5) Among siblings the first baby was most frequently affected (49.02%). 6) In development states(their body weight) 43.14% of them were over 91th precentile. 7) The cardinal symptoms and signs on admission were vomiting (82.35%), mucous bloody stool(78.43%), abdominal pain and irritability(66.67%), and abdominal mass(58.82%). 8) Most common type of intussusception was ileo-colic(50.98%) 9) Etiologically 90.20% of cases were idiopathic, but only 9.80% had organic causes. 10) In 50.98% of cases, bariumm enema was performed successfully, and surgical management with manual reduction and segmental resection was performed in 47.06%. 11) The recurrence rate was 3.92%.

2.A Case of Subcutaneous fat Necrosis Associated with Pancreatitis.

We report a case of subcutaneous fat necrosis associated with pancreatitis that may be the first case in Korean hterature as for we know. The patient was a 41-year-old man wbo presented erythema nodesum-like erythematous subcufaneous nodules on both lower extremities. Histopathologic findings showed subcutaneoms focal fat necrosis and ghoat-like cells. Granular basophilic material was deposited in and around the necrotic fat cells and stained positively with von Kossa stain. The lesioas subsided gradwdly without any specific treatment, as the underlying pancreatitis was ameliorated.

3.Statistical Observation for Admitted Patients during the Years of 1974 to 1977 at Ped. Dept. of Han-Il Hospital.

Statistical analysis according to W.H.O. classification for the patients admitted the Ped. Dept. of Han-Il Hospital was carried out during 4 years from Jan. 1974 to Dec. 1977. The following results were observed. 1. Total No. of patient during 4 years period were 1955, of which 1199 were male(61.33%, 756 were female(38.67%) and male to femal ratio was 1.57:1. 2. There was no significant variation. 3. According to age, preschool aged group as the most frequent group consistin of 488 cases(24.49%), school aged group and adolescent in the order of frequency. 4. On monthly distribution, there was no significant variation 5. Most frequent diseases in pediatric age group were respiratory tract one, 851 cases(39.29%) : infectious and parasitic ones, 553(25.53%) and neonatal disease, 204 cases(9.42%) in the order of frequency. 6. Among respiratory tract disease, pneumonia was the most frequent one, 587 cases(27.10%). 7. Among infectious and parasitic disease group, gastrointestinal infectious one was the most frequent, 224 cases(10.34%) : other viral disease and tuberculosis in the order of frequency. 8. Among neonatal diseases, prematurity was the most frequent one, 82 cases(3.79%).

5.A Clinical Study of Changes in INtracranial Pressure on Neurosurgical Patients under General Anesthesia with Nitrous Oxide, Thiopental Sodium and Lidocatine Hydrochloride .

In patients undergoing intracranial surgery, the drugs used for anesthesia and the opera-tive procedure can effect an increase in intracranial pressure. Especially in patients whose intracranial pressure has already increased significantly, if the pressure is further increased br these factors, then the patient's neurological condition may be aggravated and the post-operative prognosis may be affected. Thus it is desirable to use drugs which reduce intrac-ranial pressure during this type of surgery. In this study, the spinal subarachnoidal pressures of two groups were compared. One group received a combination of thiopental sodium and lidocaine which are known to relatively reduce intracranial pressure, white the other group was given halothane which is known to increase intracranial pressure. The summary of this study is as follows : 1) The spinal subarachnoidal pressure began to increase 1~4 minutes after the administra-tion of halothane, and the duration of increament was 15-38 minutes with a maximal pressure change from 346+/-63 mmHaO to 417+/-90 mmH2O. Thereafter the pressure returned to the pre-induction level spoataneously. 2) The spinal subarachnoidal pressure began to decrease 2~4 minutes after the admini-stration of thiopental sodium and lidocaine. The greatest decrease in pressure occurred within 10~15 minutes level as the Preseure fell form 324+/-41 mmH2O to 210+/-73 mmH2O, thereafter the pressure remained decreased.

8.Patency Rate of Internal A-V Fistula for Hemodialysis in ESRD Patients

Hemodialysis that to maintain life quality and may be preliminary stage of kidney transplantation is essential in end-stage renal disease(Esrd) patients. Since 1996, Brescia-cimino are used to internal radio- cephalic fistula and this fistula method was the most popular in world-wide. And then variable methods (e.g, Autogenous, PTFE, Dacron etc.) were usually tried instead of above standard fistula. We experienced 75 fistulas in 62 cases from Jan. 1993 to Dec. 1996 and among 75 fistula operations standard radio-cephalic fistula was 56, brachio-cephalic fistula 10, graft fistula 9. Early patency failure rate of A-V fistula was 9 cases, 14.5%. Significant factors to effect in patency of A-V fistula were propably diabetic mellitus, venous diameter, graft material and were not related to age/sex, blood pressure, BUN/creatinine etc. In conclusion the reduction of early patency failure rate in A-V fistula is to maintain long patency rate of A-V fistula

9.The Effect of High Frequency Sensorineural Hearing Loss on Auditory Temporal Resolution: Gaps-In-Noise Test Performance in Older and Young Adults with Normal Hearing.

BACKGROUND AND OBJECTIVES: Auditory temporal resolution, which refers to the time-related aspects of acoustic processing, can be evaluated by the Gaps-In-Noise (GIN) test. We investigated whether the presence of high frequency sensorineural hearing loss (HF-SNHL) affects the temporal resolution of GIN performance in older adults with normal hearing. SUBJECTS AND METHOD: Hearing tests including the GIN test were performed in 87 subjects with normal pure tone average. The GIN threshold and percentage of correct answers were compared among 4 groups of participants; older adults with normal hearing (n=18), older adults with HF-SNHL (n=24), young adults with normal hearing (n=24) and young adults with HF-SNHL (n=21). RESULTS: There was no significant difference in the mean GIN thresholds between the HF-SNHL group (5.8+/-0.8 msec) and the normal hearing group (6.0+/-0.8 msec) in older adults, whereas the mean GIN thresholds of HF-SNHL group was higher than that of the normal group in young adults (4.6+/-0.3 msec vs. 4.2+/-0.5 msec, p<0.05). The mean percentage of correct answers of HF-SNHL group (62.5+/-5.5%) was not significantly different from that of the normal hearing group (60.6+/-3.9%) in the old, unlike in the young (71.3+/-4.0% with HF-SNHL vs. 76.9+/-4.3% with normal hearing, p<0.05). Age only showed a significant correlation with the GIN performance. Neither the GIN threshold nor the GIN perception level had any relation with the presence of HF-SNHL in older adults. CONCLUSION: We found no evidence that supported the influence of HF-SNHL on auditory temporal resolution in older adults. These results imply that HF-SNHL may be of little importance in gap detection after age-related changes in central auditory system.

10.MR Measurement of the Normal Corpus Callosum in Children.

PURPOSE: To measure the mean size of the various portions of the corpus callosum in normal Korean children, using MR imaging. MATERIALS AND METHODS: Our subjects were 166 children (male : female=100 : 66) aged under 15 whose findings on MR imaging and neurologic examination were normal. Using midsagittal T1-weighted imaging, we measured the length of the brain and corpus callosum, the height of the latter, and the thickness of its genu, body, transitional zone and splenium. The measurements were statistically analysed according to age and sex. RESULTS: Brain length and the size of the various portions of the corpus callosum tended to increase relatively rapidly during the first three years of life, but the rate of growth tended to decrease according to age. The mean length of the brain and corpus callosum and the mean thickness of the splenium of the corpus callosum did not differ according to sex. The mean thickness of the genu, body and transitional zone of the corpus callosum was greater in males than in females. The ratio of the length of the corpus callosum to the anteroposterior diameter of the brain was significantly greater in females than in males (alpha = 0.05). CONCLUSION: Using MR imaging, we measured the mean sizes of the various portions of the corpus callosum in normal children ; these values may provide a useful basis for determing changes occurring in its structure.
