1.Ipsilateral Tibial Shaft Fractures and Knee Ligament Injuries

Nine patients with ipsilateral tibial shaft fractures and knee ligament injuries were retrospectively reviewed to determine the methods of the diagnosis and the results of the treatment. The incidence of this combined injuries was 5% in one hundred and eighty two tibial shaft fractures. The injuries of posterior cruciate ligament were combined most freqently. Five of nine patients were open fractures. Bone union was obtained in all cases and the mean duration of bone union was 22.5 weeks. Athroscopy and stress X-ray for the cruciate ligament were helpful to diagnose this combined injures. The ligament injuries were surgically managed in eight and conservatively in one. Better results were obtained when this combined injuries were operated early, but four of nine patients were disabled. The results suggest that the examination of the knee is necessary to document ligamentous injuries that occur with tibial shaft fractures.

7.Clinical Observation of Double Outlet Right Ventricle.

Eighteen cases of double outlet right ventricle were observed during past six years at the department of pediatrics, Seoul National University Hospital. 1. Therw were 13males and 5 females. Sex ratio showed male predominance with the ratio of 2, 6:1. 2. According to the Sondheimer's classification, 6 cases were in group l, none in group ll, 10cases in group lll and 2 cases in group lV. 3. Presenting symtoms and signs were cyanosis(18/18), dyspnea(17/18), growth retardtion(10/18), and cyanotic spell(6/18). 4. The range of hemoglonin concentration was 14 to 21 gm%(mean 18.2gm%). Simple chest X-ray showed cardiomegaly in 10 patients. On routine ECG examination, RVH were noted in all 18 cases. 5. In group l patients, mean oxygen saturation of the aorta was 74.5%, mean oxygen saturation of the pulmonary artery was 86.7%. But on group lll, the results were variable. And in guoup lV, oxygen saturation difference were negligible in both great arteries. 6. Pulmonic outflow obstruction were noted in 12 cases in which 8 cases were combined type. 7. Operation was done in 8 cases. Out of the 6 cases who were receive corrective surgery, 3 cases died of the right heart failure. 8. Associated anomalies were as follows : bilateral SVC in 6 cases, PDA in 5 cases, ASD in 4 cases and dextrocardia in 4 cases.

9.Inferior vena cava thrombosis: US and CT evaluation.

Forty five patients with inferior vena cava (IVC) thrombosis were studied with the use of ultrasonography (US) and computed tomography (CT). Thirty seven cases were cased by tumor extension and the primary neoplasms were hepatocellular Ca. (26cases), renal cell Ca. (6 cases), Wilms' tumor (1 case), IVC leiomyosarcoma (1 case) and retroperitoneal metastatic tumor (3 cases). Non-tumor thrombus were 8 cases which included 5 cases of Budd-Chiari syndrome and 3 cases of thrombophlebitis. US and CT both were good for the diagnosis of IVC thrombosis. Cranial extension was better demonstrated by US whereas CT yielded better delineation of the lower extension. Even though, differentiation of tumor non-tumor thrombi by the echogenecity and density of the thrombus was not possible, the finkdings of adjacent tumor mass, complete obstructive thrombus within dilated lumen with bulging wall, and nontapered acute margin of thrombus made the possibility of tumor thrombus more likely.

10.A Case of CHAP Syndrome.

CHAP syndorme consists of choreoathetosis, orofacial dyskinesia, affective change, hypotonia, and pseudobulbar signs. CHAP syndrome was firstly des- cribed with onset 1 to 7 days after induced profound hypothermia and complete circulatory arrest for con- genital heart lesion repair. Since then, only a few cases have been reported until now. Case We report a patient who demonstrated all of the elements of the CHAP syndrome in severe cachexic state. Brain SPECT(99mTC-ECD) shows decreased perfusion in left fronto-parieto-temporal lobe. Comments The pathogenesis of CHAP syndrome is obscure. However our case led us to suggest that more essential component pre- disposing to the CHAP syndrome is hypoxia rather than hypothermia. Furthermore, it seems to be related with circulatory insufficiency in extrapyramidal nervous systems.
