1.A Study the Procurement of HLA Class I Typing Trays Using Gushed Out Blood During Placental Delivery.

BACKGROUND: Microlymphocytotoxicity test is most widely used for HLA Class l typing but almost all laboratories depend on imported HLA Class 1 typing trays. Matching criteria for the selection of HLA- matched platelets to treat platelet refractoriness is not as strict as for bone marrow transplantation. Therefore, with the acquisition of various antisera against high frequency HLA antigens, self-made HLA typing trays can be used for HLA typing of HLA-matched platelet donors. METHODS: 140 samples obtained during placental delivery were tested for the presence of HLA antibodies against a well-characterized panel of 90 cells. Specificity of HLA antisera were determined by evaluating the correlation coefficient r of the 2 x 2 table, x2 test. Antisera strength was evaluated by the strength index. RESULTS: HLA antibodies were detected in 25 samples by primary screening and 23 samples also showed a positive reaction in secondary screening(16%). Among 23 samples, 1 1 antisera were of reagent grade quality and 7 were monospecific antisera. DISCUSSION: Imported HLA typing trays can be replaced by harvesting HLA antisera against HLA antigens which are relatively common in Koreans through continuous HLA antibody screening using gushed out blood during placental delivery. 2.A Case of Lymphoepitheliomalike Carcinoma of the Uterine Cervix.

The lymphoepitheliomalike carcinoma of the cervix is histologically similar to lymphoepithelioma of the nasopharyngeal region or medullary carcinoma of the breast which has grossly circumscribed, histologically poorly differentiated nonkeratinized squamous cell carcinoma with prominent stromal inflammation. Although the entity has been rarely reported in the united states, it has been reported relatively common in Japan. The Japanese literature suggests that patient with this varient may have a more favorable prognosis when compared with the usual squamous cell carcinoma of the cervix A patient with a squamous cell carcinoma of the uterine cervix with marked lymphocyte and plasma cell infiltration is reported and the literature reviewed.

3.Dosimetric Consideration of the Lung Block in the Mantle Field.

PURPOSE: To evaluate the dose under lung block as a function of depth and the effectiveness of a block as a function of block width. MATERIALS AND METHODS: Field size of mantle field was 22.8 x 32.4 cm2 . Dose distribution of the mantle field was measured with two dimensional water phantom system. To analyze the effectiveness of the lung block, central axis plane, 5cm off-axis plane, and 10cm off-axis plane were studied. RESULTS: The dose under the lung block was recorded with maximum at the depth between , 5cm and 10cm . In the central axis plane, dosimetric block width was 10-15% les than physical block width. In the 5cm off-axis plane. Dosimetric block width was 4-9% less than physical block width. In the 10cm off-axis plane, dosimetric block width was 2% less than physical block width. CONCLUSION: Depth dependence of the dose under the lung block was founded. Also, block width dependence of the lung block was founded. To induce the accurate relation between the physical block width and the "effective" block width, it needs more detailed understanding of the variables involved.

Diverticular disease of the colon is frequently involved in Western countries, which in korea, it is regarding as rare disease. In Western the diverticulosis is mainly affected on left side and increasing in according to age and false type is more frequent than ture type. While in oriental countries, the diverticulosis of the colon is mainly affected on right side and it is more frequent in younger patients. Cecal diverticulitis is known to a very rare disease and very difficult to be differenciated from acute scopy has purnished another useful tool in the differential diagnosis of diverticular disease. We experienced a case of cecal diverticultitis which had been diagnosed by colonofiberscopy and reviewed literatures briefly.

5.A case of Ebstein's anomaly prenatally diagnosed by fetal sonography.

We experienced a case of Ebstein's anomaly prenatally by fetal sonography. Ebstein's anomaly is a rare congenital heart defect characterized by abnormally downward displacement of the septal and posterior leaflets of the tricuspid valve. In the absence of tricuspid regurgitation, this condition may be completely asymptomatic. On the other hand, symptomatic newborns often develop life-threatening congestive heart failure. A 30-year-old multiparous woman was referred for the evaluation of fetal cardiac defect at 26th gestational week from a private clinic. Fetal ultrasonography showed markedly enlarged right atrium and downward displacement of tricuspid valve into the right ventricle. After discussion with the patient, the pregnancy was terminated and Ebstein's anomaly was confirmed by autopsy.

6.Avulsion Fracture of the Lesser Tuberosity of the Humerus in Adolescent:A Case Report and Literature Review

During adolescence, traumas in the subscapularis muscle lead more often to osseous avulsion of the lesser tuberosity or subscapularis tendon, as opposed to often to intrasubstance tendinous injury in adults. This injury is commonly diagnosed as persistent shoulder pain, and it is difficult to diagnose immediately. This article is a case report of teenage patient who underwent open reduction and internal fixation for a displaced lesser tuberosity avulsion fracture. After experiencing a sports trauma 8 months ago, a 14-year-old male came to our clinic complaining of right shoulder pain and weakness. We used a cannulated screw to humeral lesser tuberosity and repaired infraspinatus additionally. We applied a progressive physical therapy regimen postoperatively. On postoperative 6 months, full range of motion of shoulder joint and strength was restored. We report our successful surgical treatment outcome of a fracture of the lesser tuberosity in an adolescent.

10.Mercury inhalation poisoning and acute lung injury.

Acute mercury inhalation poisoning is a rare cause of acute lung injury. It is usually fatal because of progressive pulmonary failure. We experienced a patient with acute respiratory distress syndrome (ARDS) after illicit use of mercury vapor for hemorrhoid treatment; he developed acute chemical pneumonitis following exposure to mercury vapor. Prompt treatment with corticosteroids and penicillamine for acute chemical pneumonitis was instituted; radiologic pulmonary infiltrates disappeared within a week, but late phase neurologic sequelae and pulmonary interstitial fibrosis progressed.
