2.Histiocytosis X Case Report.

Eosinophilic granuloma, Hand-Schuller-Christian and Abt-Letterer-Siewe syndrome are known to be the same disease entity involving the reticulohistiocytic system of the body, although their clinical manifestations are different. So they were included under the term of Histiocytosis X and this concept has been generally accepted. The authors have experienced one case of Hand-Schuller-Christian disease and that of eosinophilic granuloma. The former was a 23 years old man with complaints of exophthalmos, polyuria and a tender palpable mass on the right parietal area and in the latter a soft pulastile mase located on the left parietal area in a 3 year old boy. There was no neurological deficits on their admission. We managed them with radiation and steroid therapy following surgical curettage.

4.Two Types of Retinal Capillary Path in Humans using Fluorescein Leukocyte Angiography.

PURPOSE: To explain hypothesis that there are two types of human retinal capillary pathways. One is simple type and the other is complex type. The former plays a role in blood flow regulation and the latter does capillary function itself. METHODS: We measured the length, tortousity, and flow velocity of human retinal capillaries using Fluorescein Leukocyte Angiography (FLA) with a Scanning Laser Ophthalmoscope (SLO) on two healthy adult male volunteers. We studied distribution and pathways of human retinal capillaries on the posterior pole. RESULTS: The lengths of capillaries were distributed with two peak incidence. One was 70 microgram and the other 240 microgram. There was no relationship between capillary length and tortousity. The correlation between capillary length and leukocyte velocity showed that the shorter capillary group has a wide range of velocity but the longer group has a narrow range of velocity. CONCLUSIONS: Two types of retinal capillary pathways exist on human retina. Compared with two types of capillaries on the cat retina described by Ben-nun et al, we found that the shorter capillary type plays a role in blood flow regulation and the longer type in capillary function itself.

We report on a 28 years old female with recurred cardiac myxomas who presented with dizziness, headache, and blurred vision. She had an excision of biatrial atrial myxomas 10 years age. Varying sized multiple cerebral aneuysms and myxomas in left atrium and left ventricle were found on a cerebral angiogram and echocardiogram, respectively. After wide excision including interatrial septum and part of left ventricular septum, her symptoms were much improved.

9.Prevalence and Characteristics of Chemotherapy-related Cognitive Impairment in Patients with Breast Cancer.

PURPOSE: Evidence suggests that some patients with breast cancer experience cognitive difficulties following chemotherapy. This longitudinal study was done to examine the prevalence of cognitive impairment and trajectory of cognitive function over time in women with breast cancer, who received adjuvant chemotherapy. METHODS: Participants were 137 patients with breast cancer. They completed neuropsychological tests and the Functional Assessment of Cancer Therapy-Cognitive Function before adjuvant therapy (pretest), toward the end of adjuvant therapy (posttest), and 6 months after the completion of adjuvant therapy (follow-up test). Of the patients, 91 were treated with adjuvant chemotherapy and 46 patients who did not receive chemotherapy made up the comparison group. A reliable-change index and repeated-measure ANOVA were used for statistical analyses. RESULTS: At the posttest point, over 30% of patients showed complex cognitive impairment and reported greater difficulty in subjective cognitive function. At the follow-up test point, 22.0% of patients exhibited complex cognitive impairment and 30.8% of patients complained of subjective cognitive impairment. Repeated-measure ANOVA showed significant decreases after receiving chemotherapy followed by small improvements 6 months after the completion of chemotherapy in cognitive domains of change for attention and concentration, memory, executive function, and subjective cognitive function. CONCLUSION: These results suggest that chemotherapy in patients with breast cancer may be associated with objective and subjective cognitive impairments. Further studies are needed to explore the potential risk factors and predictor of chemotherapy-related cognitive changes. Also nursing interventions for prevention and intervention of cognitive impairments should be developed and tested.
