2.Experimental Induction of Pancreatic Hyperplastic Nodules by Administration of 4-hydroxyaminoquinoline-1-oxide in Rats.

Pancreatic hyperplastic nodules were experimentally produced in male Sprague-Dawley rats by single doses of intravenous injection of 4-hydroxyaminoquinoline-1-oxide(4-HAQO), 5 mg, 10 mg, 20 mg and 30 mg per 1kg of body weight. Every ten rats were sacrificed after 1 day, 1 week and monthly until 6 months. Within the first week, pancreatic acini and islets were severely affected with necrosis and the adjacent acinar cells underwent vacuolization. But, pancreatic ductules remained relatively unaffected. The degrees of acinar cell degeneration were proportionally increased as with the increased doses of 4-HAQO administration. The levels of blood glucose and serum amylase were closely corresponding to the light microscopic features of the pancreas. Two months after administration of 4-HAQO, the nongranular acinar cells started to proliferate and their proportion of total acinar cells was 35%, whereas that of control group was 15%, after 3 months the proportion of nongranular cells was decreased. After 6 months, multiple hyperplastic nodules were discovered within the pancreatic acini of all 12 rats, and the largest one measured 500 micro in diameter. Each hyperplastic nodule was uniformly composed of larger cells in two folds of normal cells with hyperchromatic muclei and prominent nucleoli together with a few zymogen granules in their cytoplasms. Ultrastructurally, the cells contained abundant dilated endoplasmic reticulum and markedly decreased zymogen granules. Nuclear heterochromatin became finely dispersed with loss of marginal clumping. During the regeneration process, admixture of islet cells with membrance-bounded granules was identified among the clusters of regenerating acinar cells. On the basis of above findings, the induction of hyperplastic nodules in rat pancreas by 4-HAQO was confirmed, and its histogenesis may reflect a nodular hyperplasia of non-granular acinar cells with potential progress as a neoplastic lesion. And the evidence of the hypothesis that the pancreatic exocrine cells and endocrine cells are in same origin, was presented.

4.Congenital Dislocation of the Hip

It is well known that early diagnosis and early treatment is very important for the patient with congenital dislocation of the hip joint to have favorable function in the whole life. The goal of treatment is to replace the hip restored in containment with anatomical position. There have been various treatments in both conservative and surgical methods. In the past, conservative treatment has been prevalled, and since Salter has advocated innominate osteotomy, this surgical method could be successively replaced the treatment of congenital dislocation of the hip in the older age group. We have experienced 40 congenital dislocation of the hips out of 35 patients at the Department of Orthopaedic Surgery in National Medical Center. Clinical analysis has been performed, with average 3 years and 9 months follow-up study and the resuits are as follows. 1. Majority of the case was treated primarily at age between 12-24 months. 2. The preponderence of girl to boys was 33:2. 3. The affection ratio of left to right side was nearly equal as 16:14. Five out of 35 cases were involved at both sides. 4. The chief complaints were almost limping and limb shortening. 5. In 40 hips from 35 patients, 12 hips were treated by closed reduction, 3 hips by open reduction and 18 hips by Salters innominate osteotomy in the age group under 7 years old. By Seddon's classification, we obtained 11 excellent and good results out of 12 cases treated by closed reduction, 2 out of 3 hips by open reduction and 15 out of 18 hips by Saiters innominate osteotomy. 6. We performed femoral derotation osteotomy in 2 cases, 9 and 11-year-old patients. Steels operation with femoral shortening in 9-year-old girl and bilateral THR on 2 hips in 44-year-old patient. Result of derotation osteotomy belonged to clinical failure and THR belonged to excellent group by Seddons classification. 7. We experienced one case of avascular necrosis transiently, but finally healed delaying weight bearing.

8.Five Cases of Cutaneous Leishmaniasis.

Cutaneous leishmaniasis is a self-limited infection of the skin caused by Leishmania tropica, a protozoan parasite transmitted by Phlebotomus sandfly. This disease is endemic in tropical and subtropical zone, but recently many cases of cutaneous leishmaniasis were reported in Korean among the peoples who had been worked in the Middle East. We experienced five cases of cutaneous leishmaniasis developed in Korean who had been worked in Saudi Arabia and Jordan. Skin lesions of various sized, central ulcerated and marginal elevated, with serosanguinous discharge and some crust were scattered on exposed area, especially both extremities. Histopathologic findings showed hyperkeratosis, acanthosis, pseudoepitheliomatous hyperplasia, dermal infiltration of histiocytes, lymphocytes, plasma cells and neutrophils, and numerous Leishman-Donovan bodies in and around histiocytes. About 1-4 months treatment with trimethoprim-sulfamethoxazole and metronidazole, two cases were healed with hyperpigmented scar and two cases were improved but one case was not responded.

9.Extraosseous Tuberculosis of the Extremities

Extraosseous tuberculosis of the extremities is relatively rare even in the wide spread incidence of bone and joint tuberculosis. And few reports about extraosseous tuberculosis of the extremities have been published in the literatures. Eight cases of the extraosseous tuberculosis of the extremities were experienced at the Department of Orthopaedic Surgery, National Medical Center, from 1975 to Sep. 1979. Five out of 8 extraosseous tuberculosis were tenosynovitis and the others were bursitis in location. All cases were surgically excised and combined with antituberculous chemotherapy, and primarily cured.

10.Cavernous Hemangioma of Skeletal Muscles of the Extrimities: 5 Cases Report

The firat caae of hemangioma originating in the skeletal muscle was reported by Linton in 1843. Hemangiama involving the skeletal muscle ia less common and the correct preoperative diagnosis is often not made even tbough they attain considerable size. These tumors have remained asymptamatic and undetected by their hosta. We experienced 5 cases of hemangiama in the skeletal muscle for 2 years since 1979 and the diagnosis were confirmed clinicopathologically.
