2.The Treatment of Unicameral Bone Cysts by Topical Injection of Methylprednisolone Acetate: Reaport of 4 Cases

Unicameral bone cysts have been treated by variable surgical methods, but reported with high recurrent rates. Recently, the treatment of unicameral bone cyst by topical injection of methylprednisolone acetate has been reported with excellent results. The authors treated 4 cases of the unicameral bone cyst by topical injection of methylprednisolone acetate and followed up for 3 years and 5 months to 4 years. The results were as follows: l. 3 cases were healed completely with obliteration of cyst cavity and 1 case was in residual healing process. 2. If the cavity persists or recurrs, it seems possible to cure the cyst by repeated local injection.

3.An Experimental Study of the Effect of Histamine on the Rabbit Corpus Cavernosum Tissue.

Penile erection is controlled by adrenergic and nonadrenergic-noncholinergic (NANC) neurotransmission. There are many other substances involved in the corpus cavernosum smooth muscle contraction and relaxation. Among them. histamine is supposed to play an important role to the penile erection mechanisms in both human and animals, but the exact effect and mechanisms are not established. To investigate the effects of histamine on the corpus cavernosum tissue and the intracellular signal transduction mechanisms, we have studied on the rabbit corpus cavernosum using organ bath and the following results were obtained. 1. Corpus cavernosum tissue showed contraction in response to histamine in a dose-dependent manner( ED50 = 2.59 x 0.00001mol). 2. Pheniramine maleate ( 0.00001mol), a H1-receptor antagonist, abolished histamine-induced contraction, but cimetidine(0.00001mol), a H2-receptor antagonist, had no effect on histamine- induced contraction. 3. Histamine-induced contraction was abolished by W-7 (0.0001mol), a calmodulin antagonist, but not affected by staurosporine ( 0.0000001mol), a protein kinase C inhibitor. 4. In the precontracted tissue by phenylephrine histamine produced no relaxation, but pheniramine maleate produced significant relaxation in precontracted tissue It is concluded that in rabbit, histamine induced a dose-dependent contraction mediated by H1- receptors and calcium-calmodulin complex as an intracellular second messenger, but there were few H2-receptors in rabbit corpus cavernosum tissue. By the results of this study, it is supposed that histamine or pheniramine maleate could be used as a drug of intracavernosal injection therapy, but human tissue study, in vivo study and/or in vitro study under electrical stimulation must be preceded.

Leiomyomas most frequently occur in the genitourinary and gastrointestinal system. This report describes the clinical and imaging findings of a leiomyoma originating in the prevesical space, a rare location. The mass abutted the fundus of the uterus, but after the correct identification of feeding vessels from the right iliac artery, prevesical leiomyoma was diagnosed radiologically.

5.The Normal Vasoseminal Vesiculography.

Vasoseminal vesiculography was performed on 40 asymptomatic men. Considerable variability in the appearance of the normal adult seminal tract was seen. The average lengths of the left and right ampullae were 4.23+/-0.77cm and 4.09+/-0.78cm respectively. Most ampullaes (75%) began at the vesicular fundus; the remainder projected either slightly more lateral (18%) or medial (7%) to the fundus. The average lengths of the left and right vesicle were 3.21+/-0.61cm and 3.24+/-0.49cm respectively ; the average widths were 1.36+/-0.30cm and 1.36+/-0.27cm. Eighty-seven per cent of subjects had symmetric-appearing vesicles. The average lengths or the left and right ejaculatory duct were 12.7+/-0.27mm and 13.0+/-0.59mm respectively; the average widths were 1.9+/-0.59mm and 1.8+/-0.58mm. Two normal configurations were noted; straight (70% ) and curved (30%). All ducts appeared symmetric. Criteria for normality are presented and the radiographic techniques were reviewed.

6.A Descriptive Study of the New Injury Severity Score (NISS) Based on the Traffic Accident Injury Pyramid.

PURPOSE: This study was conducted to construct traffic-accident-related injury pyramid based on the New Injury Severity Score (NISS). METHODS: We constructed injury episodes to use the Traffic Accident Insurance database (1997~2001) and calculated the NISS by Abbreviated Injury Scale (AIS). All episodes were classified into three groups by using the NISS: minor group (1~10), moderate group (11~25), and severe group (25~75). Seventy-five points were assigned to all death episodes. We constructed a traffic-accident-related injury pyramid by using an NISS criterion. We compared frequencies and percentiles of episodes in the NISS-based injury pyramid. RESULTS: The number of episodes was 2,012,135. Of these, 1,331,599 episodes involved males, and the other 680,536 episodes involved females. The number of death episodes was 2,091 in 1997, 1,971 in 1998, 2,430 in 1999, 2,546 in 2000 and 141 in 2002. The mean NISS was 6.89+/-6.31, and the percents for the minor group, the moderate group, and the severe group were 85.58%, 13.39%, and 1.03%, respectively. The percent of males in the severe group was higher than that of females by 2.43 times. The percent of older persons (>46 yrs) in the severe and moderate groups was higher than that of young adults (16-45 yrs) The percents of belt-unattached episodes, pedestrian episodes, and separated-car accident episodes in the severe group were higher than those of bet-attached episodes, passenger or driver episodes, and car-to-car accident episodes. The mean out-patient-department follow-up period and the mean loss rate of labor ability increased from the minor group through the moderate group to the severe group. CONCLUSION: We confirmed the characteristics of the traffic-accident-related injury pyramid.

7.Cell Surface Antigen in Bladder Tumors: An Approach Using Morning Urine of Bladder Washing Specimen.

The major blood group antigens A, B or O (H) are present on normal bladder, epithelium. Several recent studies have suggested that the loss of cell surface antigens may be a precursor of subsequent invasion of the bladder by tumor. Because the specific and cell adherence test demonstrates the presence or absence of these antigens the test may have an important prognostic and screening value. We have examined cells in the morning urines or bladder washing specimens for 7 cages of bladder tumors and 5 cases of normal controls for specific red cell adherence. Even if we have studied insufficient cases, our results suggest that a correlation exists between absence of the cell surface antigen and histopathologic dedifferentiation. And seen is a progressive logs of cell surface antigen as malignant potential increase as measured by clinical staging.

Histamine is supposed to play an important role to the penile erection mechanisms. We reported that histamine induced a dose-dependent contraction in rabbit corpus cavernosum tissue but in human the exact effect and mechanisms are not established. To investigate the effects of histamine on the corpus cavernosum tissue and the intracellular signal transduction mechanisms, we have studied on the human corpus cavernosum using organ bath and the following results were obtained. 1. Histamine produced no contraction in human corpus cavernosum tissue. 2. In the precontracted tissue by phenylephrine histamine produced dose-dependent relaxation (ED50=2.70x0.00001mol). 3. Histamine-induced relaxation was not dependent on endothelium 4. Histamine-induced relaxation was not affected by administration of pheniramine maleate(0.00001mol), but abolished by cimetidine(0.00001 mol). 5. In the precontracted tissue by phenylephrine histamine produced no relaxation, but pheniramine maleate produced significant relaxation in precontracted tissue. 6. Histamine-induced relaxation was not affected by methylene blue(0.000 01mol), a guanylate cyclase inhibitor, but in a precontracted strip by phenylephrine.forskolin, an adenylate cyclase activator, produced dose-dependent relaxation (ED50=9.34x0.0000001mol). It is concluded that in human, histamine induced dose-dependent relaxation, independent from endothelium, mediated by H2-receptors and cAMP as an intracellular second messenger. The H1-receptors were supposed to be associated with contraction, but distributed in scanty density. By the results of this study, it is supposed that histamine or pheniramine maleate could be used as a drug of intracavernosal injection therapy, but in vivo study and/or in vitro study under electrical stimulation must be preceded.

Hamartoma is a very rare congenital tumor. A few cases of hamartomatous pol yps in the stomach were reported. Most of harrurtomatous polyps in the stomach occur in patients of polyposis coli. We have experienced four cases of gastric hamartomatous polyps without polyposis coli. There are some differences between previously reported cases and ours. In our casee, the distribution of age is from fourth to ninth decade, men and women ratio is 1: 1. The number of polyps are one in three cases and two in one case. The size of polyps is from 5mm to 12 mm. The location of polyps are various, but most common in antrum. We have experienced four cases which were different from previously reported ones and report them with a brief review of literatures.
