1.The Result of the Dacryocystorhinostomy on the Chronic Dacryocystitis.

We performed 75 cases of daryocystorhinostomy from Jan. 1988 to Dec. 1990. A routine dacryocystography was performed prior to operation in all cases. A modified T oti's method was applied as a surgical technique "H" shaped incision was done. Both anterior and posterior flaps were sutured adequately and vaseline guaze was used as a plugging agent for 7 to 10 days Success was achieved in 72(96.0%) of 75 cases. The causes of 3-failures were organized granulation tissue and trauma.
2.Histological Study in Choroidal Vascular Occlusion Induced by Laser Irradiation Combined with Fluorescein Injection.

The purpose of this study is to observe the choroidal vascular occlusion by photocoagulation under intravenous administration of fluorescein sodium. Argon laser photocoagulation(intensity: 50 mW, duration: 0.05 sec., spot size: 200 micrometer) was applied to the retina of the left eye(control group) and the same method was repeated to the right eye combined with intravenous infusion of 2 ml of fluorescein sodium in the pigmented rabbit(experimental group). The photocoagulated rabbit eyes were enucleated after 2 hrs, 1, 3 and 5 days and the retina and choroid were observed under electron microscopy. The result were as follows. 1. In the control group, loss of melanin pigments in the retinal pigment epithelium and destruction of outer segments of the photoreceptor were observed but endothelial cells of choriocapillaris and Bruch's membrane were intact. 2. In the experimental group, two hours after irradiation, endothelial cells of choriocapillaries were destroyed and capillary lumen was occupied with thrombus components, the retinal pigment epithelium and outer segment of the photoreceptor cells were also destroyed. One day after irradiation, occupied materials in lumen were absorbed but residual debris of thrombus was demonstrated. Three days after irradiation, newly formed endothelial cel1 processes were extended in the area of destroyed choriocapilJaries. Five days after irradiation, long processes of newly fonned endothelial cells extended in the area of choriocapillaries. The outer part of retina was repaired with newly formed pale pigment epithelium and proliferated Miil1er cells. 3. In conclusion, argon laser-provoked vascular change was more easily recognized by intraluminal fluorescein sodium which probably acted as an energy absorbent.

3.Extraocular Sebaceous Carcinoma.

Sebaceous carcinoma is an uncommon neoplasm usually associated with ocular adnexa. This malignancy may occur at any anatomic site that contains sebaceous glands. Despite the widespread anatomic distribution of sebaceous glands, extraocular sebaceous carcinoma occurs with far less frequency. We report a case of an 87-year-old Korean woman with extraocular sebaceous carcinoma treated with total excision.

5.Ultra-Structural Changes of the Chorioretina Following Intra-Vitreal Injection of Ornithine in Rabbit.

The ultrastructural changes of the chorioretina following the intravitreal injection of ornithine in rabbit eyes at 1,2,4,8 week time intervals were investigated employing electron microscopy in this animal experiment. 1. 1 week after injection, swelling of the mitochondria in the cytoplasm of the pigment epithelium was seen. 2. 2 weeks after injection, phagosomes and phagolysosomes were present in the cytoplasm of the pigment epithelium and nerve elements in the inner nuclear layer disappeared and were replaced by Muller cell. 3. 4 weeks after injection, stroma of the choroid was atrophied and abundant melanocytes were aggregated, and papillary formed pigment epithelium with detachment of the retina was seen. 4. 8 weeks after injection, stromal atrophy and attenuated choriocapillaries of the choroid were present, and proliferation of the microvilli of the pigment epithelium after detachment of the retina was seen. 5. In summary intravitreal injection of ornithine in rabbits initially caused marked changes in the pigment epithelium with subsequent degeneration of the choroid and sensory retina.

6.Gallbladder Ejection Fraction Using 99mTc-DISIDA Scan in Diabetic Autonomic Neuropathy.

PURPOSE: We performed this study to evaluate the changes of gallbladder ejection fraction (GBEF) in diabetic patients with or without autonomic neuropathy. MATERIALS AND METHODS: This study included 37 diabetic patients (25 women, 12 men, mean age 51 years) and 24 normal controls (10 women, 14 men, mean age 38 years). After intravenous injection of 185 MBq of 99mTc-DISIDA, serial anterior abdominal images were acquired before and after fatty meal. Regions of interest were applied on gallbladder and right hepatic lobe on 60 and 90 minute images to calculate GBEF. RESULTS: GBEF was significantly reduced in diabetes with autonomic neuropathy (43+/-12.3%) and without autonomic neuropathy (57.5+/-13.2%) compared with normal controls (68+/-11.6%, p <0.05). And also, GBEF was significantly reduced in diabetes with autonomic neuropathy compared with diabetes without autonomic neuropathy (p <0.05). Fasting blood glucose level, age, sex, hemoglobin A1c, body mass index, serum lipid level were not different in these two diabetic patient groups (p>0.05). When 50.2% of GBEF was used as the criteria for diabetic autonomic neuropathy, the sensitivity and specificity were 80%, 76.5%, respectively. The area under receiver operating characteristic curve was 0.846. CONCLUSION: GBEF of diabetic patients with autonomic neuropathy was significantly reduced than that of diabetic patients without autonomic neuropathy.

7.The effect of Large for Gestational Age on Asymmetrical Ventricular Septal Hypertrophy in the Newborn.

PURPOSE: It has been known for a long time that infants of insulin dependent diabetic mothers are prone to develop macrosornia, organomegaly, hyperbilirubinemia, respiratory distress syndrome, hypoglycemia, hypocalcemia, septicemia and congenital anomalies in the neonatal period. And echocardiographic asymmetrical- ventricular septal hyper- trophy(ASH) has been observed in the newborn infants of diabetic mothers. The etiology of the ASH remains unknown, although fetal hyperglycemia and subsequent glycogen deposits have been postulated as contributing factors. Therefore, we have studied whether large for gestational age(LGA) has played an important role of developing ASH. METHODS: We compared echocardiographic findings in neonates of LGA and appro- priate for gestational age(AGA), who were admitted to the Department of Pediatrics, Chung Ang University Hospital from April 1994 to March 1997. RESULTS: 1) Mean LVED in LGA and AGA were 1.96+0.06cm and 1.94+0.04cm, respectively. 2) Mean LVPW in LGA and AGA were 0.37+0.05cm and 0.370.05cm, respectively. 3) Mean IVS in LGA and AGA were 0.400.09cm and 0.380.09cm, respectively. 4) Mean IVS/ LVPW in LGA and AGA were 1.09+0.12 and 1.040.17, respectively. There was no statistical significance between two groups in echocardiographic findings. CONCLUSION: There is no relation between LGA and ASH in the neonate. According- ly, we may not need to perform echocardiography LGA routinely.

10.Occult Hepatocellular Carcinoma Metastasized to Heart: A case report.

We have experienced a case of occult hepatocellular carcinoma metastasized to the heart in a 41 year old female. The hepatocellular carcinoma invaded the hepatic vein, grew in a snakelike fashion up the inferior vena cava and reached the right atrium. Ultrastural examination in this case confirmed the hepatocellular origin of the tumor.
