1.Mortality and Activity after Hip Fracture: A Prospective Study.

PURPOSE: This prospective study conducted to estimate mortality and activity of hip fracture among persons over 50 years of age, in Jeju island, Korea during the year 2002. MATERIALS AND METHODS: One hundred forty-nine patients among 150 patients over 50 years of age who lived in Jeju island and sustained a femoral neck or intertrochanteric fracture during the year 2002 were followed-up for average 2 years. Standardized annual mortality ratio was calculated and comparison was made between hip fracture patients and general population in Jeju island. RESULTS: One hundred forty-nine patient were followed-up (35 in men, 114 in women). The mean age of patients was 77.4 years (range 50-98 years). Mortality at 2 years (range, 1.6-2.6 years) was 28.2% (42 patients), mortality at 1 year was 16.8% (25 patients) and mortality at 6 months was 12.1% (18 patients). Patients who were classified as poor (Halpin grade III and IV) increased in number from preoperative 11 patients to postoperative 37 patients. CONCLUSION: Considering 28.2% of high mortality and 25.2% of poor activity who can not do outside activity at average 2 year follow up, the hip fracture is life threatening risk factor of elderly. Prevention falling and hip fracture risk is needed.

2.The Study of the Dry Cough Incidence Rate about Imidapril, the Angiotensin Converting Enzyme Inhibitor.

BACKGROUND AND OBJECTIVES: Imidapril, one of the ACE inhibitor, has been used to treate hypertension, congestive heart failure, diabetes mellitus and renal disease. ACE inhibitor, however often causes dry cough and this adverse effect affects the compliance rate negatively. This report aimed to examine the incidence rate of coughing caused by Imidapril treat-ment. And we compared the results with the incidence and tendency of dry cough caused by the other ACE inhibitors. MATERIALS AND METHOD: This study enrolled 38 patients who followed up at internal medicine and took Imidapril from the period of May 9, 1997 to December 17, 1997. We tested its effectiveness, safety and tolerance. The initial dosage of it was 5mg/day and the maximum dosage according to blood pressure was 10mg/day for 8 weeks. The tendency and the incidence of dry cough have been analyzed and compared with recently published Korean reports. RESULTS: The 5 mg dosage of Imidapril showed 68.1% in lowering blood pressure rate and it went up to 81.8% during the 8 weeks treatment perriod. The blood pressure regularity rate was 36.3% after the 8 weeks treatment. The overall rate of adverse effect occurrence was 26.1% (6/23) and 8.7% of them were likely to cough. One of the patients who showed nausea stopped medication. The recent articles reported that the incidence of dry cough had been reduced: Enalapril (33.8%), Fosinopril (16%), Ramipril (15%), Imidapril (7.8 - 8.7%), Losartan (4.3%). CONCLUSION: Imidapril is as safe and effective as other ACE inhibitors. Various ACE inhibitors that were newly developed show their effectiveness in lessening incidence rate of dry cough, and Imidapril is thought to be an excellent drug for this matter. Even though Losartan shows the lowest incidence rate of dry cough, it needs to more regular usages since it is still on the stage of its medical adaptation.

4.Wide-Spread Benign Schwannoma Involving Multiple Cervical Vertebral Bodies.

Benign Schwannoma rarely involves the vertebral bodies extensively. Two cases of widespread involvement of cervical vertebral bodies by Schwannoma are reported. Despite of huge amount of neoplastic mass and severe destructive bony changes, the neurologic deficits were minimal and postoperative result was not discouraging. Finding of plain X-rays, cervical CT scan and tumor pathology were discussed as well as surgical approach.

5.A Clinical Analysis of the Ventriculoatrial Shunts in Hydrocephalus.

The ventriculoatial shunt consists of diverting the cerebrospinal fluid from one lateral ventricle to the right atrium of the heart by means of a subcutaneous valve regulated drainage system. Of the numerous drainage systems so far devised, two, Holter and Pudenz, can be cited as having received widespread acceptance. This study is a clinical investigation of the 18 cases of ventriculoatrial shunts performed at Hanyang University Hospital between May 1972 and December 1978. Of them 17 cases and the Holter System, and one the Pudenz System. Ventricular catheter tip was placed in the frontal horn and cardiac catheter tip in the upper margin of the 6th thoracic vertebra. Postoperatively immediate neurological improvement usually took place in most instances. However, four complications developed. One of them was ventricular catheter obstruction with small brain tissue particles which was removed by revision. The other ventricular catheter obstruction was the result of thickening of the cerebral mantle and reduction in size of the ventricle. This shunt was made patient again byshortening the ventricular catheter to place inside the shrunk ventricle. The third was shunt infection. In this case, the whole shunting system was removed and reapplied into a peritoneal route, after infection subsided. The last was postoperative epidural hematoma developed due to incomplete hemostasis which required emergency surgery. Shunt obstruction was said as an inevitable complication in many instances, but it may easily restored by revision. Shunt infection was also surgically amenable. Although the necessity of revision was inevitable in some instances, ventriculoatrial shunt still seemed to be the most effective method so far devised.

6.A Case of Transitional Medulloblastoma in the Posterior Fossa of a Child.

The case is reported of a 2 1/2-year-old femal with headache, vomiting and paraparesis. Clinical feature and operative findings are quite different from those of classic medulloblastoma in its lateral location, well-circumscribed tendency, non-invasiveness, no CSF metastasis, and favourable prognosis after surgical removal. Histological features are the combination of those of classic, and desmoplastic medulloblastoma. Review of literature was done.

Verified bacteriologically and histologically was this tuberculous cerebellar abscess. 8.Corneal Melting and Descemetocele Resulting From Noninfectious Keratitis Related to the Cosmetic Contact Lenses.

PURPOSE:To report 2 cases of corneal melting and corneal melting with descemetocele that occurred in users of cosmetic contact lenses. CASE SUMMARY: A-12-year-old and a 13-year-old female who used cosmetic contact lenses were referred to our clinic under the preliminary diagnosis of keratitis and corneal melting. The patients had purchased the lenses from an optician and had worn the lenses for approximately 1 month without being educated on their proper use. The signs and symptoms improved after 2 weeks of treatment with oral steroid and 1% topical prednisolone acetate. However, descemetocele occurred in the 12-year-old patient. Reepithelization of the cornea had been completed within the treatment period. However, corneal thinning with mild opacity remained in the lesions, and the best corrected visual acuities on the Snellen chart were 20/30 in both patients.

9.Aberrant Expression of p53, MCM2 Protein and Proliferative Activity in Glioma.

In gliomas, the most frequently studied tumor suppressor gene is the p53 gene which has been found to be mutated in a considerable fraction of astrocytomas and glioblastomas. Mutations that alter the function of the p53 gene product are thought to play a critical role in glial tumorigenesis. The murine double minute 2(MDM2) gene has been shown to code for a cellular protein that forms a complex with the p53 tumor suppressor gene product and inhibits its function. The fact that MDM2 can negatively regulate p53 suggests MDM2 could function as an oncogene when overexpressed. We investigated the expression of p53, MDM2 and proliferative activity of the tumor cells in 71 cases of gliomas(17 cases of differentiated astrocytomas. 25 cases of anaplastic astrocytomas and 29 cases of glioblastomas). Their paraffin-embedded tissue sections were immunostained with monoclonal antibody(p53 and PCNA, proliferating cell nuclear antigen) and polyclonal antibody(MDM2) for detection of p53, MDM2 and PCNA respectively. The results were as follows: The p53 staining was positive in 28 cases(39.4%) and MDM2 staining in 5 cases(7.0%) of 71 gliomas. The p53 positive-staining was detected in 2 cases(11.8%) of 17 differentiated astrocytomas, 9 cases(36.0%) of 25 anaplastic astrocytomas and 17 cases(58.6%) of 29 glioblastomas. The p53 expression was associated with malignancy grade(p<0.005) and proliferative activity was strongly associated with malignancy grade(p=0.0001). The p53 expression was closely associated with proliferative activity : p53-positive tumors had significantly higher median PCNA-labeling index than p53-negative tumors(40.6+/-10.1% versus 19.6+/-15.0%)(p=0.0001). But, MDM2 expression was not associated with proliferative activity(p=0.4575). The proportion of p53 immunoreactivity had significant association with proliferaive activity: the more the proportion of p53 immunoreactivity increased, the higher PCNA-labeling index elevated(p=0.0001). None of the tumors with MDM2 expression showed immunoreactivity for p53. These results suggest that the mutation of p53 gene plays a critical role in malignant transformation in glioma and it could be the prognostic factor for histologically same grade gliomas and that a subset of human gliomas escapes from p53-regulated growth control by amplification and overexpression of MDM2. Therefore gene study targeting these genes may be useful for the management of human glioma as a diagnostic modality.

10.A Case of pasteurella multocida pleural empyema.

Pasteurella multocida, a Gram-negative coccobacillus, is part of the normal oral flora of many types of animals, including domestic dogs and cats. It is the etiologic agent of a variety of infectious diseases, such as hemorrhagic septicemia in cattle or fowl cholera in chiken. Although this is a primary pathogen in the animal world, infection due to Pasteurella multocida in man has been described with increasing frequency recently. The majority of individuals with pasteurella multocida pulmonary infection possess some underlying pulmonary diseases, most commonly bronchiectasis or COPD. With review of literature, We report a young man who developed the empyema caused by Pasteurella multocida.
