Objective To investigate anterior longitudinal ligaments (ALL) ossified surrounding osteotomy vertebra impact the lordosing effect of pedicle subtraction osteotomy (PSO) in patients with thoracolumbar kyphosis secondary to ankylosing spondylitis (AS).Methods We retrospectively reviewed 102 AS patients with thoracolumbar kyphosis treated with single-level PSO at our institution from September 2007 to August 2015.There were 92 male and 10 female.The average age was (35.6±11.8)years old (range from 17 to 65 years old).Patients were stratified into ossified group (54 cases) and non-ossified group (48 cases)based on the presence of ALL ossification adjacent to osteotomy vertebra.Compared the contribution of adjacent disc wedging to total correction of each PSO segment between the ossified and non-ossified groups.The long-term correction loss of spine and pelvic sagittal morphology were also evaluated and compared between the 2 groups.Results Patients in the ossified group accomplished significantly lower amount of correction in single level segment of PSO (36.3°±6.9° vs.41.5°±6.9°),and there was significant difference between the two groups.The contribution of adjacent disc wedging to total correction of PSO was significantly larger in the non-ossified group (22.9％ vs.7.8％,P＜0.001).For subgroups with a minimum 2 year follow-up,loss of corrections concerned sagittal vertical axis (SVA),which was (1.7±4.5) cm vs.(-0.2±4.0) cm in ossified group and non-ossified group,and there was significant difference between the two groups.Pelvic tilt (PT) was 3.5°±8.2° vs.2.0°± 10.4°,lumbar lordosis (LL) was-7.9°±11.9° vs.-0.1°± 11.9° and sacral slope (SS) was 4.5°±9.3° vs.1.6°±7.9°,and there were all significant differences between the two groups.The change of adjacent disc wedging angle was marginally higher in the unossified group (-2.1°±6.2° vs.-0.1°±3.7°,P=0.09),but there was no significant difference between the two groups.No significant correction loss of osteotomy angle was observed in both groups.Conclusion Osteotomy vertebrae accompanied by unossified adjacent ALL in PSO of AS were prone to create more disc-originated lordosing effect immediately after surgery.However,a correction loss might occur more commonly during a long term follow-up.

Objective To observe the protective effect of silybin-phosphatidylcholine compound(SPC) on acute liver damage induced by alpha-naphthylisothiocyanate(ANIT) in mice.Methods Forty mice were randomly divided into 4 groups:normal group,model group,SPCⅠgroup,SPCⅡgroup.Intragastric administration of 0.5% carboxymethylcelluloes-Na(CMC-Na) suspension were given to the normal and model group,and 2 quantities of SPC suspension(150 and 300 mg/kg) were respectively given to SPCⅠ and SPC Ⅱ groups for 1 week.At 12 h after last administration of SPC,all of groups besides normal were given ANIT(dissolved in peanut oil,(60 mg/kg)) by lavage and the normal group just only were given peanute oil by the same volume and same way.After 16 h of ANIT administration,the levels of serum alanine transferase(ALT),aspartate aminotransferase(AST),serum total bilirubin(TB),malondialdehyde(MDA) and superoxide dismutase(SOD) were detected by biochemical method.Results Serum levels of ALT,AST,TB and MDA markedly increased after ANIT was administered via intragastric tube,and the level of serum SOD also decrease.SPC could obviously inhibit the alteration of the activities of serum ALT,AST,TB,MDA and SOD(P

OBJECTIVE: To analyze the utilization of traditional Chinese medicine (TCM) injections in our hospital in order to provide reference for clinical rational use of TCM injection. METHODS: The utilization of TCM injection in our hospital from 2006 to 2008 was statistically analyzed in terms of consumption sum, DDDs and its order. RESULTS: The consumption sum and DDDs of TCM injection in our hospital increased year by year, especially stasis-removing agents and antitumor drug. Over the 3 years the proportion of consumption sum of TCM injections in the total consumption sum were 57.3%, 59.4% and 55.9%, respectively. There was poor synchrony between consumption sum order and DDDs order. CONCLUSION: TCM injections are used rationally in our hospital. In addition, safety reevaluation of TCM injection should be improved.

OBJECTIVETo evaluate the effect of lateral supramalleolar artery descending branch antidromic flap for the repair of soft tissue defects in the foot and ankle.

METHODSFrom May 2009 to October 2013,12 patients with soft tissue defects combined with tendon and bone exposure in the foot and ankle were treated by lateral supramalleolar artery descending branch antidromic flap for the repair of soft tissue defects in the foot and ankle, including 9 males and 3 females with an average age of 37.5 years old ranging from 19 to 58 years. Ten cases had the soft tissue defects in the dorsum of foot and 2 in the ankle. The defect area of soft tissue was from 11 cm x 9 cm to 8 cm x 5 cm.

RESULTSTwelve patients were follow-up for 3 to 12 months (averaged 7.3 months). The flaps of 9 cases were survived,the flaps edges of the other 3 cases were necrosis,and healed after dressing change. The flaps were slightly swelling without ulcer occurrence.

CONCLUSIONLateral supramalleolar artery descending branch antidromic flap can repairing the damage by one-stage operation with advantage of dissection easy,rich blood supply without sacrifice of major artery.

Objective:To evaluate the radiographic and clinical outcomes of pre-operative Halo-gravity traction (HGT) and posterior correction surgery in treating patients with neurological deficits secondary to severe focal angular kyphosis of the upper thoracic spine.Methods:A total of 16 patients (11 males, 5 females) with neurologic deficits secondary to severe focal angular kyphosis of the upper thoracic spine undergoing preoperative HGT and posterior correction operation from January 2010 to December 2019 were retrospectively analyzed. The average age was 12.9±5.6 years (range 6-27 years). The standing X-ray of whole spine was taken at pre-, post-traction and post-operation. The Cobb angles of main curve at both sagittal and coronal planes were measured accordingly. The neurologic function at pre-traction, post-traction and post-operation was assessed according to the American Spinal Injury Association (ASIA) grading. The complications during HGT, operation and post-operative follow-up were recorded for each patient.Results:The average values of focal kyphosis and scoliosis were 96.1°±16.0° (71°-128°) and 75.5°±20.5° (40°-107°) at pre-traction respectively. The spinal cord function graded by ASIA criteria at pre-traction was B in 1 patient, C in 6 and D in 9, respectively. The correction rates of focal kyphosis and scoliosis were 32.8%±15.0% (18.0%-65.9%) and 22.9%±8.0% (14.1%-38.6%) after traction, which were further improved to 45.4%±14.9% (29.0%-69.0%) and 33.6%±8.6% (23.3%-49.3%) at post-operation without significant correction loss during 35.6±14.2 (24-72) months follow-up. After traction, the spinal cord function improved to grade D in 4 patients and grade E in 12 patients. At the last follow-up, the spinal cord functions were grade E in 15 patients and grade C in 1 patient. No neurologic monitor events occurred during operation. One patient suffered from transient left brachial plexus after operation. Further, proximal hook loosening was observed in 2 patients during follow-up. The spinal cord function was ASIA grade C pre-operatively in one patient, who recovered to ASIA grade E after operation and significantly deteriorated to ASIA C at 4 years follow-up.Conclusion:The correction of spinal kyphoscoliosis was satisfactory in this cohort. Preoperative HGT followed by posterior spinal correction surgery is an effective and safe procedure in treating neurological deficits secondary to focal angular kyphosis in the upper thoracic spine.

Objective To analyze the outcomes of bracing treatment for girls with adolescent idiopathic scoliosis (AIS), and to investigate the predictive factors of the protocol. Methods This study included 142 girls with AIS who finished standardized bracing treatment from July 2003 to July 2009. These patients had a mean age of 13.1±1.5 years, a mean main curve of 29.6°±5.4°, and a mean Risser grade of 2.0±1.5 before bracing treatment. Curve progression was defined that Cobb angle was greater than 6° compared to bracing initiation or was aggravated to more than 45° (indicative for surgery). The outcomes of bracing treatment were assessed based on the ratio of curves of progression or indicative for surgery. Chi-square and Logistic regression Analyses were performed to investigate the predictive factors of bracing treatment. Results The duration of bracing treatment averaged 2.5±1.0 years. Twenty-seven girls with curve progression (19%)and 115 girls (81%) with non-progression were found. Final curve which was greater than 45° was found in 18 girls (13%) who need a correction surgery, the remaining 124 girls (87%) had completed bracing treatment and avoided surgery. Chi-square analyses revealed that curve progression were more common in younger girls with lower Risser grade, with initial larger Cobb angle and with a main thoracic curve pattern.Logistic regression analyses found that premenarchal status and a main thoracic curve pattern were the independent risk factors of curve progression despite bracing. While initial Cobb angle which was greater than 30° was the additional independent risk factor of progression requiring surgery. Conclusion Bracing treatment could effectively prevent curve progression in most girls with AIS. The degree of growth maturity, the pattern and grade of curve are the influencing factor for bracing treatment.

Cisplatin (CDDP), homoharringtonine (HHT), mitoxantrone (MIT) and hydroxycamptothecin (HCPT) are highly effective anti-tumor drugs. To evaluate their effects in the therapy of leukemia and establish a valuable method to estimate anti-tumor drugs, Annexin V/PI double parameter flow cytometry was used to detect the effects of these drug inducing apoptosis and death in Jurkat cell line. The results showed that MIT and HCTP-induced apoptosis effects on Jurkat cell line were obvious at 4 hours in early phase after adding drug (P < 0.05) and at 8 hours in late phase after adding drug (P < 0.05). HHT had obvious effect on inducing apoptosis of Jurkat cells, but no significant difference from low to high doses. The effect of CDDP on inducing apoptosis of Jurkat cell line was obviously weaker than that of HHT, MIT and HCPT, its weak effect on apoptosis of Jurkat cell line was found only at high concentration of drug for long time. Death effects on Jurkat cell line can not be observed in every experimental group. It is concluded that low dose of MIT can effectively induced apoptosis of Jurkat cell line. Annexin V/PI double parameter flow cytometry can be used as a reliable method for clinical screening anti-tumor drugs.

Objective To compare the simple cases of hand-foot-mouth disease(HFMD) with HFMD patients complicated with encephalitis and HFMD cases complicated with pulmonary edema (PE). To explore predictor factors of disease progression and unfavorable prognosis. Methods Forty-one EV71-infected children admitted to the Fuyang First People's Hospital in Anhui Province from March to May in 2008 were investigated in the research, who were classified as encephalitis-complicated cases ( encephalitis group, n = 15 ), PE-complicated cases ( PE group, n = 15 ) and simple cases (simple group, n= 11 ). Their clinical manifestation, laboratory findings, and immunophenotypes of peripheral blood lymphocyte were analyzed to find predictors associated with disease progression and unfavorable outcomes. Results The mortality rate in PE group was 66.7%, which was significantly higher than that in encephalitis group. Ninty-three point three percent cases in PE group and encephalitis group were younger than 3 years old, with statistic difference compared to simple group. Patients in PE group had higher total blood white cell (WBC) counts and higher absolute neutrophil counts and tended to have higher breathing rate, heart i'ate and glucose level than encephalitis group. The percentages of T cells and natural killer (NK) cells were significantly lower among patients complicated with encephalitis than simple HFMD patients.Conclusions PE is one predictor for poor prognosis. Factors correlated with unfavorable outcome include high WBC, high absolute neutrophil counts; elevated breathing rate, heart rate and glucose level. The immunophenotypes of peripheral blood lymphocytes can also predict the disease progression.

Objective To evaluate the therapeutic effect of hBNP on rats with chronic heart failure (CHF). Methods Thirty CHF rats defined by echocardiography at 12 weeks post abdominal aortic constriction were randomly divided into Ad-hBNP group (2. 5 × 1010 VP/ml NS Ad-hBNP 1 ml/week ×4,n = 14), Ad-Track group ( n = 8 ), placebo group ( NS, n = 8 ), 10 sham-operated rats served as control group. After 4 weeks treatment, cardiac function was evaluated by echocardiography and hemodynamic measurements. Heart weight (HW) and HW/body weight (BW) ratio were determined. Results IVS,LVPW, LVEDD and LVESD were significantly reduced in the Ad-hBNP group [(2. 34 ±0. 29)mm, (2. 28 ± 0. 18)mm, (6. 50 ±0. 42)mm, (3.54 ±0. 59) mm] than those in the Ad-Track group[(2. 71 ±0. 35) mm,(3.02 ±0.85)mm, (7.71 ±0.83)mm, (4.72 ±0.80)mm] and in the NS group [(2.78 ±0.23)mm,(2. 83 ± 0. 53 ) mm, (7. 34 ± 0. 97 ) mm, (4. 55 ± 0. 77 ) mm, all P ＜ 0. 05]. The LVEF and LVFS of the Ad-hBNP group [(79. 27 ±7.01 )%, (43.38 ±6. 73)%] were significantly higher than in the Ad-Track group[(70.85±4.81)%, (35.72 ±3.68)%] and in the NS group[(69.67 ±6.90)%, (34.91 ±5.10)%, all P ＜0. 01]. HW and HW/BW were also decreased in Ad-hBNP group than in the Ad-Track group and the NS group. Conclusion Exogenous hBNP improved the cardiac function and attenuated remodeling in CHF rats.
