1.The treatment of complex anterior urethral stricture (report of 78 cases)

Objective To evaluate the selection of different procedures for the treatment of complex anterior urethral stricture and the key to successful operation. Methods 78 patients with complex anterior urethral strictures underwent different procedures of urethroplasty.Of them various mucosa urethral reconstruction were adopted in 40 cases,one-stage pedicle flaps urethroplasty in 26 and two-stage urethroplasty of Johanson procedure in 12. Results The patients were followed up for 6 to 36 months (mean 16.5 months).67 patients voided well and complications developed in 11. Of the 11 cases urethrocutaneous fistula occurred in 1,meatal stenosis in 2 and penile chordee in 1 for mucosal urethroplasty;urethrocutaneous fistula occurred in 2 and 1 of them was accompanied by hair bearing neourethra,urethral re-stricture in 3 for pedicle flaps urethroplasty;penile chordee developed in 2 and 1 of them was accompanied by hair bearing neourethra for two-stage urethroplasty of Johanson procedure. Conclusions Selection of different procedures for treatment of complex anterior urethral strictures depends on stricture length,location and severity.Penile pedicle flap urethroplasty is an effective method for those with urethral stricture 10 cm,especially in those whose bladder mucosa is not available.
3.Evaluation of Intervention on AIDS High-risk Sexual Behavior among Young Students in Jilin MSM Population

Objective To evaluate effect of intervention on acquired immune deficiency syndrome(AIDS) high-risk sexual behavior of males who have sex with males(MSM)population in Jilin,and to provide a basis for establishing apropriate intervention strategy for this population.Methods A cross-sectional survey was carried out on the MSM population of young students with the method of classification.Comprehensive intervention strategy included health education via new media,behavioral intervention,voluntary conseling and testing,peer education,condom promotion and standardized services for clinic of sexually transmitted diseases(STDs)and other comprehensive interventions.Before and after intervention,300 MSM were enrolled,and their demographic characteristics,knowledge of AIDS prevention and control,high risk behaviors and intervention measures were investigated and analyzed.Results Before and after the intervention,the awareness rate of AIDS related knowledge was 67.67% and 94.33%,respectively,the condom usage rate was 23.57% and 11.79% in the most recent and the last 6 months before the intervention,and it was 76.21% and 62.10 % after the intervention,respectively.The proportion for those who used condom every time in heterosexual sex behavior was increased from 23.53% (before intervention) to 69.60 % (after intervention),and the condom usage rate in the last 6 months was increased from 15.44% to 66.40%.Proportion of the MSM who had STDs related symptoms was decreased from 21.33% to 7.67% after the intervention.Conclusion After the comprehensive intervention,the awareness rate of AIDS related knowledge among MSM in Jilin is significantly increased,the incidence of highrisk sexual behavior is decreased and the condom usage rate is improved.Intervention by use of new media is low-cost,easy to operate,and can be used as a good supplementary strategy for traditional behavioral intervention.
4.Nonspecific granulomatous prostatitis (report of 26 cases)

Objective To elucidate the clinical features of nonspecific granulomatous prostatitis(NSGP). Methods From February 1994 to February 2002,26 cases of NSGP confirmed histologically were retrospectively analysed.Of them 20 cases were treated with anti-inflammatory therapy alone;4 cases with anti-inflammatory therapy plus ?-receptor blocker,and 2 cases who concomitantly had retention of urine underwent TURP. Results All the 26 patients were followed up for 6 to 42 months.The therapeutic results were satisfactory.The Qmax increased from 4.6~12.8 ml/s to 16~28 ml/s,and the hard prostate mass became soft,with reduction from 2.5 cm?3.5 cm to 0.5 cm?1.0 cm.The serum PSA decreased from 15.5~60.8 ng/ml to 1.5~10.6 ng/ml. Conclusions In clinical practice NSGP is often mistaken for prostatic carcinoma,so special attention should be paid to the differential diagnosis.The definite diagnosis depends on histopathological examination.
5.Evaluation of the surgical approach in the operation management of complex posterior urethral stricture

Objective To present the preliminary experience on the operative approach in the surgical management of complex posterior urethral stricture. Methods The operative approach of 72 cases of complex posterior urethral stricture was reviewed. Results The total success rate was 89%(64/72). 34 cases were operated with simlpe transperineal approach and the success was rate 91%, transperineal inferior pubic approach was adopted in 14 and the success rate was 93%;simple transpubic approach was conducted in 6 with a success rate of 67% and transpubic perineal approach in 18 with a success rate of 89%. 41 cases with long posterior urethral stricture were operated with a success rate of 85% and 18 cases with complication with a success rate of 94%. Conclusions Each operative approach has it own advantages and disadvantages.Transperineal approach is preferred because the procedure is simpler,less traumatic with less occurrence of complication.
6.A clinical study of enhanced continent mechanism of tapered ileum by extramural support from pouch-abdominal wall

Objective To construct a reliable continent tube, which is easy to catheterize and is surgically simple. Methods 10 patients with bladder cancer underwent a procedure in which ileal segment was tapered as an efferent tube,a part of which was placed between the back surface of the rectus muscle and wall of the ileal pouch. Internal orifice of the tapered ileum was anastomosed to the ileal pouch and external orifice of the tapered ileum was anastomosed to umbilicus. Urodynamic and radiological studies were carried out postoperatively on 9 patients. Results The stoma could be easily catheterized with a 16 F catheter in all the patients. One patient died of heart disease 55 days postoperatively. In the other 9 patients, 8 were completely continent day and night. The urodynamic study of the efferent tubes showed a maximum close pressure with a full pouch was significantly higher than those with an empty pouch (t=-7.14,P=0.000098 ). Retrograde radiogram of the efferent tubes showed perfect canalization without stenosis. Conclusion This study suggested that the continent mechanism of tapered ileum can be greatly enhanced by fixing it between the abdominal and the pouch wall,allowing also easy catheterization and surgical simplicity.
7.Enhanced continent mechanism of tapered ileum by suspension technique

Objective To construct a reliable and continent urinary reservoir,which is surgically simple. Methods (8 adult mongrel female dogs underwent a procedure in which an ileal segment was tapered as an efferent tube and a 1.5 cm in width of Dilurn polyester with a tension of 400g was used to suspend the efferent tube to enhanced the continent mechanism of the tapered ileum.) Urodynamic and radiological studies were carried out postoperatively in all. Results All the stomas could be easily catheterized with a 14 F catheter. The urodynamic study of the efferent tubes showed the maximum close pressure ranged from 117 cmH 2O to 157 cmH 2O,with a mean of 137 cmH 2O.Retrograde radiogram showed perfect canalization of the efferent tube without stenosis and pouchgram revealed no contrast medium in the efferent tube. Conclusions This study suggested that the continent mechanism of tapered ileum can be greatly enhanced by the suspension technique.
8.An experimental and clinical study of colic mucosa graft for urethral reconstruction

Objective To investigate the possibility of urethroplasty with a free graft of colic mucosa for the treatment of complex anterior urethral stricture of long length. Methods Six dogs underwent a procedure in which the urethral mucosa was totally removed and replaced with a free graft of colic mucosa. All the dogs were sacrificed 12 weeks after the procedure for histological examination of urethra.Besides, a patient with complex anterior urethral stricture was treated by urethroplasty with the use of a colic mucosal graft.Retrograde urethrography,urethroscopy and uroflowmetry were carried out 3 months after operation. Results Histological examination revealed that all the grafted colic mucosa have survived in the urethra of the experimental dogs and some of the glandular epithe lium has been transformed into urethelium. Clinically,the patient voided well after removal of the urethral stent.Neither necrosis of the neourethral mucosa nor stenosis at the anastomosis sites has been observed on urethroscopy. Retrograde urethrogram demonstrated a wide urethral caliber with no significant graft sacculation. Uroflowmetry examination showed the maximum flow rate was 30 ml/s. Conclusions Colic mucosa graft urethroplasty is an effective one stage procedure for the treatment of complex anterior urethral stricture. The technique is useful for urethral reconstruction when local or preputial skin and buccal or bladder mucosa are not available.
9.Hospital Infection among Inpatients:Situation Analysis 2005-2007

OBJECTIVE To sum up the cases of hospital infection,in order to further improve the experience of the work of hospital infection.METHODS The investigation of hospital infection situation among inpatients 2005-2007 was reviewed.RESULTS The hospital infection rates in 2005-2007 were 1.81%,1.65% and 1.52%,respectively,mainly in the lower respiratory tract infection and surgical incision.CONCLUSIONS Hospital infection is closely related with the age,the underlying diseases,the antibiotic misuse and the environment.
10.The protective effect of bone marrow mesenchymal stem cells by anoxic preconditioning on diabetic cardiomyopathy

Objective To explore the protective effect of bone marrow mesenchymal stem cells (MSC) on diabetic myocardium and anoxic pre-conditioning (AP).Methods Eight-week-- old male Sprague-Dawley rats were given with a single intraperitoneal injection of streptozotocin (STZ) (60 mg/kg)to induce diabetes mellitus (DM).Donor rats were 8-week-old male Sprague - Dawley rats.Before transplantation,MSC were incubated in CM-DiI at a concentration of 2 μg/mL for 20 min.AP-MSC were exposed to 3 hours of anoxia.At 4 months after STZ injection,diabetic rats were randomly given with an intramyocardial injection of one of the followings:150 μL of Dulbecco's modified Eagle's medium ( DMEM),5 ×106 MSC/150 μL,or 5 × 106 AP - MSC/150 μL (n =10 for each group).Three months after STZ injection and 2 weeks after transplantation,we evaluated the cardiac function by echocardiography,and also evaluated the cardiac conditions by alkaline phosphatase staining,western blot analysis for apoptosis related proteins and signal pathways.Results MSC,especially AP- MSC increased fractional shortening (FS) of diabetic heart (P <0.01 vs DMEM respectively).AP-MSC greatly increased the capillary density of diabetic myocardium (P <0.01 vs DMEM and MSC group respectively).AP-MSC are anti-apoptotic in the rat DCM model,possibly mediated through cardiac upregulation of Bcl-2/Bax ratio ( P < 0.05 ) and inhibiting the expression and activation of caspase - 3 ( P < 0.01 ).Conclusions Intramyocardial transplantation of APMSC has a protective effect on diabetic cardiomyopathy.
