1.A Case of Extramammary Paget's Disease.

Extramammary paget's disease is uncommon intraepithelial carcinoma of the skin and frequently associated with a subjacent or a regionally proximate carcinoma. We have experienced a case of extramammary Paget's disease affecting 71 year-old man. The patient has been suffered from a well demarcated, and slowly growing erythematous plaque on the left suprapubic area of 3 years. A biopsy specimen reveals infiltration of typical Paget's cells within the epidermis and the adnexa. We review the literature briefly.

2.Gross and microscopic findings of dermatophytes cultured on potato dextross agar.

To ease the convinience of indentification of common dermatophytes in Korea and to check the possibility of diseovering new species not yet reported in Korea, several species of dermatophytes were cultured on potato dextrose agar(PDA) at the room temperature. They were 46 strains isolated from Korea and 6 domestic strains from a foreign country. Gross and microscopic findings of their colonies were examined. The colonial morphology and microscopic appearance were compared with those of the same strains on Sabouraud dextrose agar(SDA) and corn meal agar(CMA). The results were as follows Typical T. rubrum and its varisnts showed characteristic wine red pigmentation which was rapid, diffuse and uniform, typically arrsnged microconidia were better on PDA. T. mentagrophytes showed yellow-brown pigmentation which were more rapid and darker in the center with gradual fading to the periphery. The mscroconidia, grape-like clusters of microconidia and spiral hyphae were seen better on SDA. T. tonsurans showed characteristic mahogsny colored pigmentation, ballooned microconidia and macroconidia which were seen better on PDA, but diffusible pigmentation of agar were present on SDA only. The gross morphology of T. verracosum were similar on PDA and SDA, but macroconidia were seen better on PDA and CMA, and tortuous hyphae with antler-like branching were seen better on SDA. T. rnegninii showed green-brown pigmentation which appeared rapidty and prominently on PDA. T. rubrum-like microconidia were seen better on PDA and CMA, but in SDA only hyphae were seen. The gross morpholgy of M. canis was similar on PDA and SDA, but spindle shaped macroconidia were seen more typically on PDA. The gross morphology and microscopic appearance of M. gypseum were very similar in all three media. M. ferruginenm showed characteristic rust colored pigmentation and bamboo hyphae which were seen more typically on PDA. The gross morphalogy of E floceosum were similar on PDA and SDA, but cluster formation of club shaped macroconidia were seen more prominently on PDA and CMA. These findings suggest that PDA is a mare convincing media t6Cifferentiate among dermatophytes belonging to genus Trichopkyton.

5.A Case of Sporotrichosis misdiagnosed as Lupus Vulgaris.

A case of sporotrichosis misdiagnosed as lupus vulgaris was presented. A 56-year-old woman had been to pea sized, nontender pustules and a hypertrophic-scar-like nodule on the right thigh. She denied any trauma before the lesions developed. Histopathologic findings from the lesion showed tuberculosis-like granulomatous changes. And she was diagnosed as lupus vulgaris. She also had pulmonary tuberculosis and had been treated with antituberculous drugs for 1 year. But skin lesions was not cleared. We suspected the lesions as one of deep mycoses and could confirm sporotrichosis by mycologic studies. They showed characteristic gross colonies and microscopic findings of Sporothrix schenckii..

6.Paget's Disease of the Male Breast.

Paget's disease of the breast occurs almost exclusively in women, and is characterized by a unilateral, sharply defined eczematous skin lesion caused by epidermal metastases from underlying ductal adenocarcinoma of the breast. Paget's disease of the male breast is extremely rare. A 61-year-old patient had a skin lesion on the right nipple for 1 year. The lesion was a well defined, brownish, crusted, and keratotic papule, and was associated with an underlying walnut sized, fixed, hard mass and a palpable ipsilateral axillary lymph node. Histologic findings were consistent with Paget's disease. Results of immunoperoxidase staining technigues for S-100 protein and carcinoembryonic antigen were negative. After a modified radical mastectomy and chemotherapy, the patient has been in remission for 12 months.

7.A case of piroxicam-induced photosensitive dermatitis.

We report a case of piroxicam-induced photosensitive dermatitis in a 54-year-old female. She had taken oral piroxicam and was exposed to the sunlight on her way home for a few minutes. Several hours after the sun-exposure she developed well-defined, confluent, erythematous plaques and numerous vesicobullae with pruritus and prickling sensation on the sun-exposed areas. A phototest was done on her first visit. The minimal erythemogenic dose (2 J/cm²) of ultraviolet (UV) A was markedly decreased whereas that of UVB was within a normal limit. Visible light irradiation for 30 minutes did not cause skin lesions. Six months after the initial skin lesions, a photopatch test with 1% and 10% piroxicam solution followed by UVA (10 J/cm²) irradiation showed positive responses on both concentrations.

9.Efficacy of testicular sperm chromatin condensation assay using aniline blue-eosin staining in the IVF-ET cycle.

OBJECTIVE: This study was performed to evaluate testicular sperm chromatin condensation using aniline blue-eosin (AB-E) staining and its effects on IVF-ET. METHODS: Chromatin condensation was analyzed using AB-E staining in 27 cases of testicular sperm extraction. There were 19 cases of obstructive azoospermia (OA) and 8 cases of non-obstructive azoospermia (NOA) in IVF-ET. Mature sperm heads were stained red-pink whereas immature sperm heads were stained dark blue. The percentage of sperm chromatin condensation was calculated from the ratio of the number of red-pink sperm to the total number of sperm analyzed. RESULTS: The overall percentages of chromatin condensation in OA and NOA were 31.1+/-11.2% and 26.3+/-14.4%, respectively. The fertilization rate was significant higher in OA than NOA (p<0.05); however, the rates of good embryos and clinical pregnancy did not show statistical differences. In OA and NOA, statistical differences were not observed in the rate of chromatin condensation, fertilization, good embryos, and clinical pregnancy between the pregnant group and non-pregnant group. CONCLUSION: Chromatin condensation is less stable than OA and showed a low fertilization rate in NOA. While there were no significant differences in chromatin condensation results between NOA and OA, we propose that a pattern of decreased chromatin condensation in NOA is one of the factors of low fertilization results requiring further study.

10.Non-Relapse Rate in a 24 Week Follow-up Clinical Study with Korean Male Alcohol-Dependent Patients.

OBJECTIVES: In order to provide useful data for the further clinical studies with Korean alcohol-dependent patients, enrollment and non-relapse rates were investigated while alcohol-dependent patients were followed up for 24 weeks. METHODS: The subjects of this study were alcohol-dependent male patients who, for the first time, visited a hospital among 3 groups of hospitals, University Hospital, General Hospital or Mental Hospital, or those who needed to be followed up after discharge from those hospitals. After assigning 12 subjects to each hospital, we investigated enrollment and non-relapse rates while cognitive-behavioral therapy and pharmacotherapy were provided. RESULTS: 1) Forty-eight patients were enrolled and the rate of enrollment was 36.4%. The enrollment rate in the University Hospital group was 52.1%, in the General Hospital group 50.0%, and in the Mental Hospital group 10.4%. 2) In 48 patients, non-relapse rate was 27.1% at 12 week and 18.8% at 24 weeks after starting follow-up. It was 24.4% and 20.0% at 12 and 24 weeks, repectively, in the University Hospital group while the General Hospital group showed 33.3% and 16.7%, respectively, and the Mental Hospital Group showed 20.0% at both 12 and 24 weeks. CONCLUSION: These results provide important data for further clinical studies of Korean alcohol-dependent patients.
