1.Percutaneous Locking Compression Plate(LCP) Internal Fixation for Distal Tibial Comminuted Fracture

ObjectiveTo explore the effect of percutaneous locking compression plate(LCP) internal fixation for distal tibial comminuted fracture.Methods35 patients of distal tibial Comminuted fracture were treated with percutaneous locking compression plate(LCP) internal fixation.ResultsAll patients were followed up for average 1.8 years(ranging from 1 to 2.5 years).All of cases have healed well,and the average bone healing was 14.6 weeks (ranging from 8 to 28 weeks ).All of them have no infection and no loosening or breakage of internal fixation.According to Mazur criterion,excellent effect 17 cases,good effect 16 cases,fair effect 1 case poor effect 1 case,94.3% was excellent or good.ConclusionThe LCP internal fixation for distal tibial comminuted fracture have the advantage of less invasive,good internal fixation and bone union fastly and little complications.

3.Effects of endophytic fungi on growth and two kinds of terpenoids for Euphorbia pekinensis

Objective To study the producing method for Euphorbia pekinensis promoted by endophytic fungi.Methods The tissue culture plantlets were innoculated with endophytic fungi,then it was transplanted to the pods and cultured for one year.The biomass and content of terpenoids(isoeuphpekinensin and euphol)were determined.Results The growth of the E.pekinensis was promoted by endophytic fungi E4(Fusarium sp.Ⅰ)and E5(Fusarium sp.Ⅱ).The fresh weight of the root increased 49.45% and 59.74% for E4 and E5 treated,respectively.The plants dry weight coefficient increased 2.99% and 6.48% for E4 and E5 treated,respectively.The contents of isoeuphpekinensin and euphol in the plants were increased for tissue cultured in both bottles and pods.For the plants cultured in pods for one year,the isoeuphpekinensin and euphol increased 92.79% and 40%,respectively treated by E4.The isoeuphpekinensin and euphol increassed 105.32% and 241.38%,respectively treated by E5.Conclusion The endophytic fungi E4 and E5 could promote the growth of E.pekinensis and accumulated terpenoids.
4.Analysis of risk factors for spontaneous rupture of primary hepatic carcinoma

Objective To study the risk factors for spontaneous rupture of primary hepatic carcinoma.Methods The clinical data of 31 patients with spontaneous rupture of primary hepatic cancer admitted to our hospital from Jan 1999 to Dec.2008 were retrospectively analyzed,and compared with 31 randomly selected concurrent patients without rapture of HCC.Results Three factors were shown to be the risk factors for spontaneous rupture of primary hepatic carcinoma.Those were APTT,HBeAg and the greatest height of tumor protrusion above the surface of liver.Conclusions APTT,HBeAg and the greatest height of tumor protrusion above the surface of liver are risk factors for spontaneous rupture of primary hepatic carcinoma.
5.A Case of Kasabach-Merritt syndrome with generalized hemorrhagic diathesis.

Kasabach-Merritt syndrome is the association of thrombocytopenia, disorder of coagulation, spontaneous bleeding and enlargement of a hemangima or extensive hemangiomatosis, which can be often life threatening. We experienced a three years-old girl with suddenly enlarging hemangioma of right cheek, complicated by severe thrombocytopenia, consumptive coagulopathy and hemorrhagic cystitis. Generalized petechia and ecchymosis were noted on the overlying skin of hemangioma, trunk and extremities. But the study of MRI and angiography, hypevascular was located within the right masseter muscle extending to temporalis muscle and anterior to parotid gland. So selective embolization with PVA (polyvinyl alcohol) was performed through the distal branch of internal maxillary artery and facial artery. She was medicated with oral corticosterioid for 3 weeks. After embolizationn, blood flow to hemangioma and facial swelling were markedly decreased. Patient was followed up for 7 years and she showed normal contour of face and no bleeding diathesis.

7.A Case of Favre-Racouchot Syndrome.

Favre-Racouchot syndrome(nodular elastosis with cysts and comedones) is a peculiar complication of solar elastosis in which comedones, follicular cysts, and nodules of altered elastic tissue occur mainly sround the orbits, on the cheeks, on snd around the nose, and oceasionally in other exposed areas, but sornetimes on the neck or behind the ears. We present a typical case of the Favre-Racouchot syndrome in a 66 year old man with maxillary cancer. The patient had numerous yellowish cysts and black topped comedones located on the forehead, around the orbits and the nose, on the neck, and also behind the ears of 5 years duration. The histopathology of the lesion showed dilated pilosebaceous openings and epitheliaI lined cysts filled with layered keratin in addition to pronounced solar elastosis.

8.High Lumbar Disc Herniations.

STUDY DESIGN: This study is a retrospective evaluation of the high lumbar disc herniations. OBJECTIVE: The purposes of this study is to observe the clinical and radiological findings and out-comes of the high lumbar disc herniations, thus providing a guideline for making diagnosis and for proper treatment modality. SUMMARY OF LITERATURE REVIEW: High lumbar disc herniations at the L1 -2, L2-3 and L3-4 levels represent less than 5% of all disc herniations. The location of pain was highly complected. The represented symptoms and signs such as motor, sensory and reflex changes were variable and potentially misleading in suggesting a level of the disc herniation. MATERIAL AND METHODS: Thirty-one patients with high lumbar disc herniations were retrospectively evaluated. Seventeen patients were treated conservatively and fourteen patients underwent surgery. Review of results was undertaken by an independent observer using a proven outcome assessment measure. RESULTS: The incidence of high lumbar disc herniation is 4.7%(L1-2: 13%, L2-3: 29%, L3-4: 58%) with declining frequency as the level ascends. The peak age incidence is 6th decade in male and 5th decade in female. The positive rate of femoral nerve stretching test(74%) is higher than that of straight leg raising test(51%). In fourteen operative cases, the positive rate of straight leg raising test is 86%. The accuracy of MRI findings in predicting the type of herniated disc is 93%. In results of operation thirteen cases showed more than fair grade(93%). CONCLUSION: The age incidence of the high lumbar disc herniation is older than that of the lower lumbar disc herniation. The femoral nerve stretching test is more useful than straight leg raising test in making diagnosis of high lumbar disc herniations. The prognosis after treatment is comparable with high lumbar disc herniations.

Solid pseudopapillary tumor of the pancreas is a rare tumor that affects young females with low malignant potential and good prognosis with more than 90% survival at 5 years. Metastasis is very rare. We report the case of a 74-year-old female who had pancreatic solid-pseudopapillary tumor and synchronous hepatic metastasis.

10.Impact of hepatitis B virus infection on efficacy of combined antiretroviral therapy in patients with acquired immunodeficiency syndrome

Objective To investigate the influence of hepatitis B virus (HBV) infection on efficacy of combined antiretroviral therapy (cART) in patients with acquired immunodeficiency syndrome (AIDS).Methods Seventy-eight subjects with human immunodeficiency virus (HIV)/HBV co-infection and 156 subjects with HIV mono-infection were included.CD4+ T cell count,HIV viral load,HBV-markers and liver functions were routinely tested.The differences in survival rate,as well as immunological and virological responses between the two groups (HIV/HBV co-infection group and HIV mono-infection group) during cART were compared.Categorical data were compared by Chisquare test,measurement data were compared by t test,and measurement data with abnormal distribution were compared by Mann-Whitney test.Results At month 42 of cART,HIV RNA levels and CD4+ T cell counts of the two groups were comparable.However,at month 48,54 and 60 of cART,the immunological and virological responses of HIV/HBV co-infection group were less favorable than those of HIV mono-infection group.At each time point of month 12,24,36,48 and 60 of cART,3 out of 13 subjects with HIV/HBV co-infection maintained hepatitis B e antigen (HBeAg)loss; the HBeAg seroconversion rates were 32.1% (9/28),50.0% (14/28),53.6% (15/28),64.3% (18/28) and 71.4% (20/28),respectively (x2 =10.189,P=0.037) ; HBV DNA negative rates were 95.1% (39/41),82.9% (34/41),68.3% (28/41),43.9% (18/41) and 43.9% (18/41),respectively (x2 =29.982,P=0.000); liver dysfunction rate was 32.1 % (25/78),51.4% (38/74),33.8% (22/65),47.9% (23/48) and 6.7% (3/45),respectively (x2 =28.053,P=0.000).Mortalities in HIV/HBV co-infected and HIV mono-infected individuals were 24.4% (19/78) and 5.1 % (8/156),respectively (x2 =18.841,P<0.01).Sixteen out of the 19 deaths (84.2 %) in HIV/ HBV co-infected subjects died of end stage liver diseases.Conclusions HBV co-infection diminishes the long term efficacy of cART.End stage liver diseases are the primary cause of death in HIV/HBV co-infected subjects during cART.
