2.Arthroscopic Meniscectomy in Bucket Handle Tear of the Meniscus

Recently, the frequency of meniscal injury of knee has been increasing due to increase of sports activities. It has been known that bucket handle tear is the most common type of the entire injuries of the meniscus. We found 127 cases of meniscal tears and experienced 42 cases of bucket handle tears in the arthroscopic findings from Jan. 1991 to April 1994 in our hospital. We analysed 31 cases which were followed up for at least 1 year. The purpose of this study is to analyse the locking history and frequency, to discuss the several types of bucket handle tear in arthroscopic view, to introduce three portal techniques using posteormedial or posterolateral portal to resect the meniscus, and to analyse our clinical results. So we concluded as follows. 1. Among the 31 cases, 23 cases(74%) had an episode of locking. 2. We found many different types of bucket handle tears in arthroscopic view and the most common type was complete tear in a classic type(11 cases). 3. We were able to resect the meniscal fragment very easily using posteromedial or posterolateral portal. 4. We obtained satisfactory results in 24 cases(77%).

5.Three Cases of Dermatofibrosarcoma Protuberans.

Dermatofibrosarcoma protuberans (DFSP) is a rare, distinctive cutaneous tumor, which consists of spindle shaped ceils arranged in densely packed interlacing bundles with the storiform or cartwheel pattern. Histologically, it resembles deep growing dermatofibroma, nodular fasciitis, neurofibroma and neural sheath tumors. DFSP is one of t.he connective tissue tumors which is difficult. to diagnose histologically as well as clinically. Recently, the immunochemical staining with a monoclonal antibody to CD34 is reported to give assistance in the clear differential diagnosis of DFSP from other fibrous or neural tumors. Herein, three cases of DFSP were stained by immunohistochemical staining with S-100 protein, vimentin, factor VIII and anti-CD34 antibody in order to assess the use of anti-CD34 in the differential diagnosis of DFSP.

6.Avascular Necrosis of Femoral Head: Findings of Contrast-Enhanced MR Imaging.

PURPOSE: To evaluate the findings and the role of contrast enhanced magnetic resonance imaging in avascular necrosis of femoral head. MATERIALS AND METHODS: Sixteen patients with avascular necrosis of femoral head were examined with MRI. Tl-weighted and T2-weighted images and contrast-enhanced Tl-weighted images were obtained. Enhancing characteristics of the necrotic area and synovium were determined. Also a change of the disease extent after enhancement was assessed. RESULTS: Twenty seven avascular necrosis of the femoral head including 11 cases of bilateral lesion were detected. Fifteen cases revealed collapse of the femoral head. The portions of the lesion with low signal intensity on Tl-weighted images and high signal intensity on T2-weighted images showed contrast enhancement in 15 cases. However, the portions with low signal intensities both on T1 and T2-weighted images showed enhancement in one case. There was no significant change of the disease extent after enhancement. Synovium showed enhancement in 18 cases, and joint effusion was detected in 23 cases. CONCLUSION: Contrast enhanced MR images may be helpful in predicting histopathologic findings of avascular necrosis of the femoral head, but not useful for evaluating the extent of disease.

8.Transconjunctival Lower Eyelid Widening using Tucking Method between Lower Eyelid Retractor and Lower Tarsal Plate.

Double eyelidplasty, ptosis correction with epicanthoplasty are widely-performed surgeries in Asians. However, patients with a narrow palpebral fissure or an up-slanting appearance tend to have a slightly disappointing surgical outcome. In those patients, cosmetic lateral canthoplasty may be an effective adjunctive procedure for lengthening the palpebral fissure in selected patient, but may have limiting results on patients with a short palpebral fissure or severe up-slanting appearance. Lower eyelid descent was previously recognized as a symptom of aging process or a complication of lower blepharoplasty. In the present, many Korean women want large, round, and oval-shaped eyes. The authors have introduced a lowering the lower eyelid via a transconjunctival approach to enlarge the palpebral fissure in selected patients. A total of 441 Korean patients underwent lowering of the lower eyelid via a tranconjunctival approach with or without lateral canthoplasty from 2005 to 2011. There were 12 males and 429 female with age ranging from 19 to 44 years old. The procedure was composed of transconjunctival incison and tucking procedure between lower eyelid retractor and lower tarsal plate. The up-slanting lower eyelid shape was easily modified by the procedure in all cases. Most patients were satisfied with the results. Mean follow-up period from 2 weeks to 4 years. Postoperative conjunctival swelling and chemosis were spontaneously resolved within 2 or 3 weeks. The lowering of the lower eyelid is a simple, effective and reliable method for selective patients.

9.Detection of Genital Human Papilloma Viruses Using PCR.

Fifteen cases of genital warts were examined for the presence of human papillomavirus (HPV) using polymerise chain reaction (PCR). HPV6/11 DNA were found in all cases of genital warts. The PCR based methods described here provide a sensitive, accurate means of detecting genital HPVs.
