3.Effect of Synovectomy on Prognsis of Rheumatoid Arthritis

This is a long term follow-up study to clarify whether arthroscopic synovectomy would be able to prevent further destruction of the articular cartilage in rheumatoid joints. Three hundred and eighteen rheumatoid joints of 94 patients who had surgery in the multiple joints(average 3.4 joints per person) and was followed for an average 39.9 months (range, 24-126 months) were selected. Diagnosis of rheumatoid arthritis was made according to the 1987 revised criteria of the American College of Rheumatology. The preoperative roentgenographic narrowing of cartilage space (modified classification of ARA) and the arthroscopic findings (grading of Outerbridge) were recorded. The points of investigation were 1) further destruction of articular cartilage on X-ray, postoperatively, and 2) reaccumulation of effusion. The results were as follows; 1) Knee Joint: Among 103 knee joints there were 92 (89.3%) Stage I, G-0 knees, 8 Stage II (G-2: 1, G-3: 3, G-4: 4 joints), and 3 Stage IIIB, G-4, preoperatively. The 3 Stage IIIB, G-4 knees showed progression of narrowing of joint space and were replaced by artificial joints two years after the synovectomy. 2) Wrist joint: Fifteen joints (28.3%) out of 53 joints were Stage II, G-4, and 2 joints joints (3.8%) were Stage I, G-3, preoperatively. They were progressed to Stage IIIB at final follow-up. 3) Other Joints: There was no further narrowing of the joint space following surgery in 22 fingers, 2 toes, 34 elbows, 34 shoulders and 70 ankles. 4) Prognosis seemed to depend upon postoperative medication. From these results it would be suggested that the rate and severity of recurrence of synovitis and further destruction of articular cartilage can be decreased with early synovectomy so long as proper anti-rheumatic medication is followed, although the surgery can not halt the disease process.

Takotsubo cardiomyopathy is considered a neurogenic heart disease resulting from impaired autonomic
function and is known to be associated with various neurological and medical conditions. To our
knowledge, although tetanus is frequently accompanied by autonomic instability, there has been no
report of Takotsubo cardiomyopathy in patients with tetanus. Here we describe a patient with Takotsubo
cardiomyopathy as a manifestation of autonomic instability in severe tetanus.

7.A Study on the Emission and Dispersion of Particulate Mather from a Cement Plant.

To investigate the an air pollution by particulate matter and its dispersion, a cement plant producing portland cement 600,000ton/year and its vicinity were surveyed from October, 1980 to April, 1983. The survey was mainly focused on main stack emission rate of the cement plant and particle size distribution in the dust, dustfall and total suspended particulate concentration in the area by month and distance from the stack. The results of the study were as follows; 1. The main stack emission rate was surveyed before and after the spray tower was additionally installed to the original E.P. and bag filter. Before the spray tower installed, the main stack emission rate was higher(0.64g/Nm3) than the emission standard of Korean Environmental Preservation Law's(0.5g/Nm3, amended to 0.4g/Nm3 on April 1983), but after the spray tower was installed, its main stack emission rate was markedly decreased to the standard (0.43g/Nm3). 2. 2~3 micrometer of the particle size was the largest portion(20.8%) of the dust particulate from the main stack and 50% of the frequency distribution was 1.5 micrometer of the size. Most particle size was below 10 micrometer. 3. The spray tower reduced the dustfall to 37.81~9.76ton/km2/month while dustfall appeared at 45.29-15.45ton/km2/month, in the vicinity of plant before spray tower installed. 4. Mean concentrations of total suspended particulate for 24 hours of the various stations were determined in 20.6-200.0 microgram/m3, 3 stations of them were higher than the value of Harry and William's arithmetic average standard 130 microgram/m3. 5. Linear regression between dustfall[X] and total suspended particulate[X] concentration was an equation, Y=4.024X+11.479.[r=0.91]. 6. During the whole seasons in the opposite area 100m apart from the emission source the prevailing wind direction was with estimated more than 30ton/km2/month, and the concentration of total suspended particulate for 24 hours averaging time was more than 140 microgram/m3 in the same area and direction. 7. Assuming the wind direction were constant through the day dustfalls for a day were estimated at 13.40ton/km2/day, 10.79ton/km2/day and 4.55ton/km2/day at various distances of 100m, 500m and 1,500m from the emission source respectively. 8. In the simutalion of dustfall and suspended dust by area, Gaussian dispersion model modified by size distribution of particulate matter was not applicated since the emission of dust were from multi sources other them stack. From the above results, it could be applied that the dispersion of dust from the cement plant is estimated and regulated for the purpose of environmental protection.

8.Clinical Study of Contralateral Inguinal Hernia Development in Children with Unilateral Inguinal Hernia.

Contralateral exploration in children with unilateral inguinal hernia is controversial. This study was done to identify risk factors for the development of contralateral inguinal hernia in patients with unilateral inguinal hernia. The clinical experience of 4,206 inguinal hernias repaired by one pediatric surgeon on 3,358 children at HanYang University Hospital from September 1979 to December 2002 was analyzed. 1,868 (55.6%) hernias occurred on right side, 1,190 (35.4%) on left side, and 300 (8.9%) were bilateral. 2,702 children were boys and 656 were girls (M:F=4.1:1). 170 children of 3,058 children with unilateral hernias (5.6%) developed contralateral inguinal hernia at 1 day to 95 months after herniotomy. 146 children were boys and 24 were girls (M:F=6.1:1). The patients who had had herniotomy before 1 year of age developed contralateral hernia in 17.4%, compared with 5.6% overall average. The earlier the first herniotomy was performed, the more frequently contralateral hernia developed. The occurrence of contralateral inguinal hernia was more frequent in boys (146 of 2,460, 5.9%) than girls (24 of 598, 4.0%) and more frequent after left herniotomy (80 of 1,190, 6.7%) than after right herniotomy (90 of 1,868, 4.8%), but statistically not significant. 52.9% of contralateral inguinal hernia developed within 1 year after hernia repair, and 87.6% developed within 3 years. Routine exploration on contralateral side in children is not necessary.

9.Report of a Case of Menetrier's Disease.

Menetriers disease is a rare disease, characterized by a marked hypertrophy of the mucosa of the fundus and corpus of the stomach and gastrointestinal loss af protein often causing transient edema. The 16-Year-old female patient was admitted to the Seoul Adventist Hospital and she complained epigastric pain & discomfort for 20 days. Under the UGI & endoscoyy, pathologic examination was done. The diagnosis was estabilished. She wes treated medically.
