Leiomyoma is a benign tumor, which usually occur at the smooth muscle of the gastrointestinal tract and uterus but rarely occurs on the corpus cavernosum of the penis. We experienced a patient with leiomyomas on the bilateral corpus cavernosa of the penis accompanied with impotence in a 60-year-old man. The patient was treated with complete local excision and has remained well without recurrence for l year, So we report this case with review of literatures.

3.The Efficacy of Recombinant Human Follicle Stimulating Hormone (rhFSH) in Human IVF-ET Program.

OBJECTIVES: Recently, recombinant FSH (rFSH) has been manufactured using a Chinese hamster ovary cell line transfected with the gene encoding human FSH. Both rFSH and urinary gonadotropin (uFSH) could be used for controlled ovarian hyperstimulation (COH). However, uFSH implies a number of disadvantages, such as batch-to-batch inconsistency, no absolute source control, dependence on large amounts of urine, low specific activity, and low purity. The purpose of this study was to evaluate the efficacy of rFSH in human IVF-ET program. MATERIALS AND METHODS: A total of 508 infertile women was enrolled in this study. They are classified into rFSH group (n=177) or uFSH group (n=331), and all of them were matched by age and cause of infertility in same period. The Puregon(R) (Organon, Holland) was used as rFSH, and the Metrodin-HP(R) (Serono, Switzeland) and Humegon(R) (Organon, Holland) was used as uFSH. We subdivided the patients into three age groups. The outcomes of IVF-ET program were analyzed using the statistical package for social sciences (SPSS). RESULTS: There was no significant differences in the level of estradiol on hCG injection day, the numbers of retrieved oocytes, matured oocytes, fertilized oocytes, transferred embryos, frozen embryos between the two groups. The total dose (IU) of gonadotropin for COH was significantly lower in the rFSH group compared to uFSH group (1339+/-5491.1 vs 2527.8+/-1075.2 IU, p<0.001). Clinical pregnancy rate per embryo transfer in the rFSH group showed increasing tendency, compared to the uFSH group, but there was no statistical significance (35.2% vs 29.3%). Our results demonstrated that the relative efficiency of rFSH compared with uFSH is higher in older patients. CONCLUSIONS: The ovarian stimulatory effect and clinical outcome of recombinant FSH was similar to that of the urinary gonadotropin. The IVF-ET cycles with significantly lower dose of gonadotropin in rFSH group showed comparable results. Therefore, we suggest that recombinant FSH is more potent and effective than urinary gonadotropin.

4.Quantitative Analysis of GABA and/or Glycine Like Immunoreactive Synaptic Terminals on Masseter gamma-motoneurons for Jaw Closing Muscle of the Rat.

The distribution of GABA and/or glycine like immunoreactive nerve terminals on the soma of the masseteric gamma motoneurons were investigated using retrograde tracing of WGA-HRP (wheat germ agglutinin conjugated horseradish peroxidase) and postembedding immunogold labeling methods in serial ultrathin sections. Quantitative analysis of 140 nerve terminals apposing on somata of gamma motoneuron size less than 21 mm in average diameter was performed. The results obtained were as follows. 1. Synaptic covering % of apposing nerve terminals was 21.45+/-11.48% and packing density was 12.85+/-6.17. 2. Nerve terminals immunoreactive (IR) to GABA or glycine were F type containing pleomorphic vesicles with round shape predominant. Majority of nerve terminals immunonegative to GABA or glycine were S type containing spherical vesicles and few of them were F type. 3. 11.42+/-10.00% of examined nerve terminals were IR to GABA only, and 12.71+/-9.85% were IR to GABA and glycine, and 15.21+/-9.58% were IR to glycine only. 4. Synaptic covering % of nerve terminals IR to glycine only was highest (4.58+/-4.50%), followed in order by GABA and glycine (3.18+/-2.77%), and GABA only (2.38+/-2.06%). 5. Among all terminals, immunonegative nerve terminals (60.66+/-14.65%) were much more than nerve terminals immunoreactive to GABA and/or glycine (39.34+/-14.65%) These results show that inhibitory synaptic input and synaptic organization of the masseteric gamma motoneurons reveal characteristic features in contrast to that of alpha motoneurons and which may correlated to the electrophysi-ological characteristics of masseteric gamma motoneurons.

5.A cephalometric study on correlation between mandibular symphysis and craniofacial skeleton.

The configuration of mandibular symphysis is likely to be dependent upon the genetic predeterminants and/or compensatory adjustments onto functional demands. The interrelation of morphological distinctives of symphysis in conjunction to the craniofacial skeleton had been scarcely anecdotal, therefore, the objective was to determine the correlation of morphological modifications between the mandibular symphysis and craniofacial complex. Lateral cephalometric headfilms of 212 subjects were employed for the conventional measurements. The proportion of chin height against chin depth length was referred as chin ratio, then, Low Symphysis (IS) and High Symphysis (HS) groups were turned out by means of the chin ratio. These samples yielded 35 in LS and 35 in HS groups. The data per capita were statistically analyzed and the following results were drawn ; 1. Overall characteristics of the craniofacial skeleton in HS group manifested hyperdivergence and LS group showed hypodivergence. 2. Gonial angle increased as chin ratio increased and was highly correlated to the chin ratio. 3. The chin ratio presented high correlation to the vertical face height, especially in terms of the chin height to anterior face height and the chin depth to posterior face height. 4. The morphological configuration of chin was hardly correlated with hyoid bone position.

6.Early Colorectal Cancer.

Purpose : Early colorectal cancer is defined as the depth of tumor invasion limited to mucosa or submucosa regardless of the presence or absence of lymph node metastasis. We performed a retrospective study to determine the chronological changes in frequencies of early colorectal cancer and clinicopathologic differences between early colorectal cancer (ECC) and advanced colorectal cancer (ACC). Methods : We reviewed hospital records of the patients with colorectal cancer operated between January 1990 and December 1995. We classified the patients into two groups, ECC and ACC, according to the depth of tumor invasion and compared the clinicopathologic characteristics. Results : Fifty eight patients (5.2%) were diagnosed with early colorectal cancer among 1113 colorectal cancer patients operated at the same period. The frequency of ECC has increased from 1.9% in 1970~1989 to 5.2% in 1990~1995. The average age of patients with ECC at the time of surgery was 55.8 compared to 56.5 for patients with ACC group (p>0.05). Most patients (72.4%) with ECC had bleeding symptoms and majority of the ECCs were located in the rectum (72.4%). The mean size of tumors was 2.6 cm in its greatest diameter and was significantly smaller than that of ACC (5.7 cm). Compared to ACC, ECC had better histologic differentiation and fewer lymph node metastases (p<0.05). Thirty six of the ECC patients underwent bowel resection and remaining 22 underwent local excision. After a mean follow up period of 39.1 months (range 2~81months), recurrence was detected in one case. There was no death during the follow up period. Conclusion : The frequency of ECC has increased recently. Compared to patients with ACC, patients with ECC had more favorable clinicopathologic characteristics and better outcome. In selected patients, minimal operation can be applied without compromising the clinical outcome.

7.Synaptic connectivity and ultrastructrue of the masseteric muscle spindle afferent trigeminals according to geometric location in the trigeminal motor nucleus of the cat.

The purpose of this study was to investigate any relationship between the geometric factors of synaptic contacts of muscle spindle afferent terminals and masseteric motor neurons in the trigeminal motor nucleus. Terminals from the masseteric muscle spindle afferents were stained with intra-axonal injection of HRP and were examined electronmi-croscopically with serial sections at the central and peripheral regions of trigeminal motor nucleus of the cat. The number of terminals examined were 76 in peripheral and 105 in central region. The results obtained were as follows. 1. Most of the labeled terminals showed simple synaptic connectivity. Each terminals in peripheral and central region made synaptic contact with 1 to 5 neuronal profiles. Two or three labeled terminals were occasionally seen to make synaptic contact with the same dendrite. 2. The average number of postsynaptic proximal dendrite per labeled terminal was higher in the central region than in the peripheral region. In contrast, that of postsynaptic distal dendrite per labeled terminal was higher in the peripheral region than in the central region. 3. The average diameter of postsynaptic dendrites in the central region was larger than that in the peripheral region. This imply terminals in the peripheral region contacted with further distal part of the distal dendrite than that in the central region. These results indicate that synaptic connectivity associated with the spindle afferents from masseteric muscle is different according to their geometric location within the trigeminal motor nucleus and suggest that there will be precise interrelationship between the morphology, pattern of synaptic connectivity and functions of muscle spindle afferents.

9.Fracture strength of the IPS empress crown : the effects of incisal reduction and axial inclination on upper canine.

PURPOSE: The purpose of this study was to compare the fracture strength of the IPS Empress ceramic crown according to the incisal reduction (2.0mm, 2.5mm, 3.0mm) and axial inclination (4degrees., 8.degrees, 12degrees.) of the upper canine. MATERIALS AND METHODS: After 10 metal dies were made for each group, the IPS Empress ceramic crowns were fabricated and each crown was cemented on each metal die with resin cement. The cemented crowns mounted on the testing jig were inclined 30 degrees and the universal testing machine was used to measure the fracture strength. RESULTS: 1. The fracture strength of the ceramic crown with 3.0mm depth and 12degrees inclination was the highest (839N). Crowns of 2.0mm depth and 12degrees inclination had the lowest strength (559N). 2. There was no significant difference in the fracture strength by axial inclination in the same incisal reduction group. 3. The fracture mode of the crowns was similar. Most of fracture lines began at the loading area and extended through proximal surface perpendicular to the margin irrespective of incisal reduction.
