6.A Study of the pH Values of Discharges in Otolaryngology.

The authors examined 312 outpatients, including 100 with otorrhea, 144 with rhinorrhea and 68 with pharyngeal discharge. The pH of otorrhea was generally acid, except in 2 cases of diffuse otitis externa where the discharge was strongly alkalihe. Otorrhea in purulent otitis media was mucopurulent or purulent and was strongly acid. The pH of rhinorrhea ranged from 6.6 to 7.4, being generally weakly alkaline. In acute rhinitis, it was alkaline in half the cases; a tendency to greater alkalinity showed in cases of marked swelling of the inferior nasal conchae. In chronic paranasal sinusitis, the pH generally shifted to the acid side; in acute paranasal sinusitis and in nasal polyps, it was weakly alkaline in all cases; and in allergic rhinitis it was neutral with in a narrow range. the usual pH of oral and pharyngeal discharges ranged from 6.4 to 7.0, being generally weakly alkaline. The character of inflammatory discharges was serous or mucous in all cases, except for a case of peritonsillar abscess with mucopurulent discharge; the pH was weakly alkaline in 5 cases (7.4 per cent), neutral in 26 (38.2 per cent), and acid in 37 (54.4 per cent). When these discharges changed in character from seromucous to purulent, their pH was lowered.

7.Testicular Biopsy: Comparison of Open Surgical and Percutaneous Needle Biopsy Techniques.

Testicular biopsy has been known as very useful diagnostic procedure for the male infertility. For the past 9 months percutaneous testicular biopsy procedures were attempted on 64 patients with azoospermia or atrophic testis using a new Tru-Cut biopsy needle. In addition surgical testicular open biopsy procedure on one side and percutaneous testicular needle biopsy procedure on the other side were performed in comparison with two techniques. Biopsy specimens were fixed in Bouin solution. They were stained and histologically examined. Results were summarized as follows: Numbers of seminiferous tubules obtained by needle biopsy technique were found to be fewer than that of the tubules obtained by open biopsy technique. But diagnostic accuracy was not compromised by the needle biopsy technique. There was no bleeding either from the testis or scrotum, no extusion of testicular tissue from the puncuate site. In conclusion percutaneous testicular biopsy using Tru-Cut biopsy needle has been proved to be simple, time saving, less expensive and accurate method for testicular biopsy.

10.Surgical Principles and Tactics for Successful Clipping of Intracranial Aneurysms.

Microsurgical clipping is a standard treatment modality of cerebral aneurysms. Despite the far advances in surgical technique and craft, aneurysm surgery is still challenge to neurosurgeons. It carries considerable risk of mortality and morbidity to the patient unless the surgeon keeps the basic principles and tactics for accurate placement of aneurysm clip. To attain the goal of complete isolation of aneurysm from the cerebral circulation with concurrent saving parent artery and critical perforators, safe exposure of aneurysm, proper clip selection and accurate placement of the clip should be necessary. From exposure of target aneurysm to the final clip application, the authors will describe notable considerations for the successful clipping of aneurysm based on their own experience and review of literature.
