PURPOSE: Renal transplantation which allows children normal growth and development and a return to normal life. is now proven to be the best modality for children with ESRD Up to Recently, the number of renal transplantations in Asia has rapidly increased and the outcome has also improved. This investigation was planned to estimate the current status of pediatric renal transplantation in Asia and to find the keys for better improvement of outcome in pediatric renal allograft in Asian countries. MATERIAL AND METHODS: The participating countries and institutions for this investigation were China, Hong Kong, India, Indonesia, Japan, Malaysia, Pakistan, Philippines, Singapore, Thailand, Korea, KSPN (Korean Society of Pediatric Nephrology), KONOS (Korean Network for Organ Sharing). RESULTS: Many countries in Asia still do not have a well organized nation wide renal transplantation registration system independently in the pediatric field . So it's very difficult to evaluate the real state of pediatric transplantation among Asian countries. According to the estimation with fragmented data from each countries, in the front running countries of pediatric renal transplantation in Asia, about 40 or more transplants were performed in each country per year and the five year actuarial renal allograft survival was around 80% which is similar to that of western countries. But there were large gaps among the behind groups. CONCLUSION: Vigorous attempts to perform renal transplantation for children especially younger than 5 years old would be encouraged as well as organ donation from brain dead donor and non heart beating cadaveric donor also should be activated to cope effectively with the shortage of living donor supply. Large number of recent reports shows the favorable outcome of pre-emptive renal transplantation, we should make more efforts toward pre-emptive renal transplantation. First of all, in order to improve the outcome and to narrow the gap between Asian countries in pediatric renal transplantation, effective and continuous efforts to establish nationwide pediatric renal transplantation registration program as well as official, nation-to-nation data sharing program should be needed.

Sacral agenesis is a rare condition. This syndrome was first described by Hohl in 1852 and is usually associated with other anomalies such as vertebral deformities, deformed hypoplastic legs, urologic and gastrointestinal problems. We have experienced two cases of this deformity; one with partial sacral agenesis, and the other with partial lumbar agenesis and complete absence of sacrum.

The laser is a new surgical modality that utilizes high intensity beams radiant energy to vaporize of coagulate tissue. Radiant energy is generated by stimulated emission of photons resulting in a unidirectional monochromatic beam that is temporary coherent small diameter beams of great power density are achievable by this means. The author reviewed 24 months experience with a CO2 laser involving neurosurgical operations to 45 neurosurgical lesion, i, e. brain tumor 25, intracerebral hemorrhage 15, cerebral paragonimiasis 1, cysticercosis 1, brain abscess 1, and arteriovenous malformation of brain 1 and spine 1 were removed with the use of Shaplan CO2 laser since 1981. We have conclude that there had been a significant reduction in blood loss and neurological morbidity though it was readily conceded that the number of operations were relatively small. The particular advantages of the combination of CO2 laser and microscope were felt to be precision reduction of mechanical trauma good hemostasis and low cost. Especially the blood loss and replacement during operation have been reduced.

OBJECTIVE: In accordance with the increasing number of elderly people in our society, the incidence of pedicle screw fixation which is a technically demanding procedure and requires a lengthy operation time has increased in the patients with lumbars instability over 60 years old. To ascertain the outcome and efficacy of the pedicle screw fixation in patients over 60 years old, we analyzed the surgical results including complications. PATIENTS AND METHOD: From May, 1994 to September, 1997, we assessed 30 patients over 60 years of age and 159 patients under the age of 60 with lumbar spinal instability who underwent surgery of pedicle screw fixation and compared with the results of surgery between two groups. RESULTS: For the elderly, the average operation time was 244 minutes and the transfusion amount was 3.3 pints. For younger patient the average operation time was 246 minutes and the transfusion amount was 3.2 pints. Four surgical complications occurred; two patients with voiding difficulty, one patient with heart problem, and a patient with DIC(Disseminated Intravascular Coagulation). However, all complications were controlled without further problems. The success rate in the elderly was 60%, which was lower than the success rate of 76% among younger patients. CONCLUSION: Considering the condition of the diseases in the elderly and the fact that the operations were performed without any major problems, these results are acceptable. Based on our analysis, it is our view that advanced age per se should not be regarded as a contraindication for pedicle screw fixation.

OBJECTIVES: Cases of psychiatric disorders are reported with a review of literature on mental health problems in North Korean defectors in South Korea. METHODS: Four patients who were admitted to a psychiatric hospital with major depression, schizophrenia, bipolar affective disorder, or mixed disorder of conduct and emotions of adolescent were studied. RESULTS: The most common precipitating factor was the separation from close persons they had been dependent on. Defector-specific stress included unfamiliar culture of South Korea, lack of ability to adapt to South Korean society, uselessness of past education and professional skills, stress with regard to being identified or discriminated as a North Korean defector, and guilt feeling for leaving their family behind in North Korea. North Korean defector-specific clinical findings were noted; these included reluctance to expose themselves as North Koreans, negative emotional reaction to discrimination of South Korean neighbors, submissive and passive attitude, loneliness, anxiety for the future related to refugee status, and strong stigma related to psychiatry. CONCLUSION: This case report suggests that an individualized or tailored treatment that considers their unique psychosocial situation, which especially focuses on discrimination and social support, is needed for North Korean defectors with mental disorders.

Gastric diverticula have been considered to be rave when compored with diverticula in other sites of the gastrointestinal tract. It's prevalence is 0.043% of routine gastrointestinal radiologic examination by Palmer's extensive review. Gastric diverticula almost always occur as a single lesion and approximately 75% of gastric diverticula occur in the juxtacardiac region, high on the posterior wall of the stomach, about 2 cm below the esophagogastric junction and 3 cm from the lesser curvature. We report a case of 75-year-old male patient with multiple gastric diverlicula which was first encountered by endoscopy and confirmed by upper gastrointestinal radiologic examination.

Systolic time intervals were studied in a total of 83 patients with pure or predominant isolated valvular heart disease. They consisted of three groups of patients : namely, 38 cases of mitral stenosis, 25 cases of mitral regurgitation and 20 cases of aortic regurgitation. The mean of the electromechanical systole was within normal ranges in all three groups, and threre was no significant difference between the groups. The mean of the left ventriclar ejection time was also within normal limits in all groups, as was that of the electromechanical systole, but it was significantly shorter in patients with mitral regurgitation than in the others, and was significantly longer in patients with aortic regurgitation. The mean of the pre-ejection period and the ratio of the pre-ejection period to the left ventricular ejection time were within normal ranges in patients with aortic regurgitation, whereas both parameters were significantly increased in patients with mitral stenosis or mitral regurgitation, particularly in the latter.
