3.Studies on Electrocardiogram of 18,000 Koreans.

Statistical analysis of 18,211 cases of electrocardiogram which were recorded in Chung-Ang University Hospital from January 1969 to December 162% of total cases(61% of male and 63% of female) and the incidence of normal electrocardiogram was decrease with aging. 2) Abnormal Q-wave was seen in 0.4% of total cases(0.5% of male and 0.3% of female) and the incidence of abnormal Q-wave was more common over 50 years of age. 3) Left axis deviation was seen in 1.1% of total cases(1.4% of male and 0.9% of female) and the incidence of left axis deviation was more common over 50 years of age. Right axis deviation was seen in 0.3% of total cases(0.3% of male and 0.4% of female) and the incidence of right axis deviation was more common under 30 years of age. 4) Left ventricular hypertrophy was seen in 3.6% of total cases(4.8% of male and 2.5% of female) and the incidence of right ventricular hypertrophy was more common under 30 years of age. 5) ST-segment depression was seen in 2.0% of total cases(2.2% of male and 1.9% of female) and the incidence of ST-segment depression was increased with aging. 6) T-wave inversion was seen in 1.6% of total cases (1.5% of male and 1.7% of female) and the incidence of T-wave inversion was increased with aging. 7) Atrioventricular block was seen in 1.4% of total cases(1.8% of male and 1.0% of female) and the incidence of atrioventricular block was more common over 50 years of age. 8) Incidence of complete left bundle branch block was 0.2% and was more common overs 60 years of age. Incidence of complete right bundle branch block was 1.1% and the incidence was more common over 50 years of age. Incidence of incomplete right bundle branch block was 3.0% and the incidence was more common under 40 years of age. 9) The incidence of premature beat was 2.7% of total cases, atrial fibrillation 1.5%, supraventricular tachycardia 0.2%, sinus tachycardia 7.5%, sinus bradycardia 2.2% and sinus arrhythmia 2.3%. 10) Low voltage was ssen in 3.8% of total cases and the incidence of low voltage was more common over 50 years of age.

4.Is the Renal Uptake of 99mTc-DMSA Decreased in Microalbuminuric Diabetic Patient?.

PURPOSE: Diabetic nephropathy is the most common cause of end stage renal disease and the incidence is progressively increasing. The aim of this study was to investigate the differences of 99mTc-DMSA renal uptake among diabetic patients with normoalbuminuria, microalbuminuria and overt proteinuria, and then to determine the clinical usefulness of 99mTc-DMSA in predicting early diabetic nephropathy. MATERIALS AND METHODS: 99mTc-DMSA scan was performed and a total renal uptake of 99mTc-DMSA was measured in 145 diabetic patients. Patients were divided into 3 groups according to the amount of 24 hour urinary albumin excretion as Group I (normoalbuminuria, 74 cases), Group II (microalbuminuria, 39 cases), and Group III (overt proteinuria, 32 cases). The differences of 99mTc-DMSA renal uptake among the 3 groups and the correlation between the renal uptake of 99mTc-DMSA and other clinical parameters were analyzed. RESULTS: The total renal uptake of 99mTc-DMSA of Group II (40.8+/-11.0%) was significantly lower than that of Group I (54.4+/-6.3%, p<0.001). The uptake of Group III (27.7+/-12.0%) was significantly lower than those of both GroupI and Group II (p<0.001). 99mTc-DMSA total renal uptakes correlated negatively with serum creatinine level (r=-0.629, p<0.001) and positively correlated with creatinine clearance rate (r=0.702, p<0.001). CONCLUSION: 99mTc-DMSA total renal uptake of diabetic patients with microalbuminuria was significantly decreased compared with that of patients of normoalbuminuria. Therefore, 99mTc-DMSA scan can be used as a diagnostic study for early detection of the diabetic nephropathy.

5.Contents of Lead, Cadmium and Zinc in Bone, Muscle and Fin of Carassius carassius from Middle Stream of Nakdong River, Korea.

In order to determine the contents of lead, cadmium and zinc, the tissues of the crussian carp, Carassius carassius, collected from the middle streams of the Nakdong river were examined. During the dry season from 8 to 15, March, 1987, six loci were selected to sample the fish and river water; five of them were the midstreams of the Nakdong river i.e., the vicinities of the Andong dam, the Nakdong bridge, the Waegwan bridge, Gangjung and Gaepori, the other one was the Kumho river around the Paldal bridge(see Fig.1). The microanalyses of lead and cadmium contents were undertaken by the atomic absorption spectrophotometer(Model IL-551) connected with CTF-IL 655, while that of zinc by the flame method with IL-551 only. The contents of lead and cadmium in water sampled from Gaepori distant from about 30km downward from the junction with the Kumho river were about 1.5 times higher than those from Gangjung distant about 0.5km upward from the junction, and the content of zinc from Gaepori was slightly higher than that from Gangjung. However, the contents of lead and cadmium in water sampled from Gaepori were three fifth of those from the vicinity of Paldal bridge far about 2km upperward from the junction. In the other words, the contents of lead and cadmium in the samples from the vicinity of the Paldal bridge were about 2.5 to 3 times higher than those from the Gangjung and 5 times higher than those from the Andong dam. The contents of the heavy metals in the tissues of Carassius carassius were relatively consistent to those of the aquatic environments were the fish were collected, with higher contents in the tissues of the crussian carp collected from the vicinity of Gaepori and the Paldal bridge which were so much polluted than in those from other loci in upper stream from the junction. And the contents of lead and cadmium in tissues were positively correlated with the age of the fish, however, that zinc was not. The contents of lead in bone of crussian carp from the vicinity of the Paldal bridge in the group of 0-1 year old fish were similar to those of 4-5 years old fish from Gaepori, and higher than those of 3-4 years old fish collected from the upper stream of the junction. It is likely that fishing from Gaepori and the paldal bridge is not recommended, and all the industrial producers have to pay intensive attention to the water pollution due to the sewage disposal.

6.The Electrocardiographic Changes in the Anemia.

The electrocardiography was performed to the 95 patients(58 male, 37 female) with chronic anemia in Department of Internal Medicine, Busan City Hospital, Busan University Hospital, Busan paik Hospital In-Je Medical College, Busan korea between April 1978 and March 1981. The hemoglobin levels were 4 Gm%-10Gm%, and the symptoms such as dizziness, palpitation, weakness, and appetite loss were continued for 6 months or more. The results were as following: 1. The abnormal electrocardiographic changes were observed in 49 of the 95 chronic anemic patients. 2. Prolonged QTc interval was observed in 12(12.6%) of 95 cases, depressed ST segment in 48(50.5%), inverted T wave in 32(33.7%), LVH in 31(32.6%), low voltage in 5(5.3%), and Rt. B.B.B. in 5(5.3%). 3. In the Hb level of 4.0~5.9Gm% 20% of the total 95 patients revealed prolonged QTc interval, 86.7% depressed ST segment, 40% inverted T wave, 20% LVH, 6.7% Rt. B.B.B., in the Hb level of 6.0~7.9Gm 10.6% prolonged QTc interval, 39.3% depressed St segment, 42.9% inverted T wave, 39.3% LVH, 3.6% low voltage and in the Hb level of 8.0-9.9Gm% 11.5% prolonged QTc interval, 46.1% depressed ST segment, 27% inverted T wave, 32.7% LVH and 7.7% low voltage.

7.Insulin-Like Growth Factors and Their Bindign Proteins in Uterine Leiomyoma Pretreated with Gonadtropin Releasing Hormone Agonist.

BACKGROUND: Uterine leiomyoma is the most common pelvic tumor, occurring in 20-25% of women in reproductive age. Gonadotropin releasing hormone agonist (GnRHa) has been reognized as a temporary medical management for this disorder. The etiology of these tumors is unknown but it has been shown that the insulin-like growth factors (IGF-I, IGF-II) are promoters of growth in nongynecologic tumors. Several recent studies have suggested the possible role of IGFs in human leiomyoma growth. The IGF binding proteins (IGFBPs) are believed to modulate actions of IGF and to have IGF-independent actions. The purpose of this study was to evaluate the type of IGF and IGFBP which may be involved in leiomyoma growth and to investigate a possible IGF related mechanism of action of GnRHa. METHOD: The IGFs and IGFBPs were measured by double antibody radioimmunoassay, western ligand blot and immunoprecipitation in the tissue cytosols of normal uterine myometria (n=15), nontumorous myometria adjacent to a leiomyoma and leiomyoma from patients nontreated (n=15) and treated (n=10) with GnRHa. RESULTS: The mean IGF-I and IGF-II level were significantly higher in leiomyoma from untreated patients than in the adjacent myometrium and normal myometrium but no significant differences in these IGF levels between normal myometrium and adjacent myometrium were noted. The IGFBP-2, IGFBP-3 and 26kDa IGFBP were detected variably but IGFBP-4 was consistently present in all tissues. There were no significant differences in the relative intensity for IGFBP-4 and the frequency of IGFBPs between leiomyoma, adjacent myometrium and normal myometrium from untreated patients. The IGF-I, IGF-II levels and the relative intensity of IGFBP-4 in leiomyoma from GnRHa-treated patients were significantly lower than those in untreated patients, but these levels in the adjacent myometrium were comparable. The frequency of each IGFBP in leiomyoma and the adjacent myornetrium from GnRHa-treated patients did not significantly differ from untreated patients. CONCLUSION: Both IGF-I and IGF-II are involved in the growth of leiomyoma and GnRHa may in part act to decrease size of leiomyoma by regulating the local levels of IGF-I, IGF-II and IGFBP-4.

8.Treatment of the Unstable Thoracolumbar Spine Fractures Using Posterior Approach

Continued improvements in treatments of the thoracolumber spine have occured in the last several years. The operative treatment of choice currently is short segment fusion(instrumentation one level above to one level below the injury). The development of pedicle screw systems has brought short segment fusion into clinical reality from the posterior approach. We analysed the clinical results of 20 patients with unstable fracture and fracture-dislocation of thoracolumbar spine who were treated with the posterior transpedicular screw system. The mean fol- low-up was 18 month. Ll was the most common injury level and the bursting fracture was the most common fracure type by Denis classification. The mean preoperative sagital index was 21.5。 and improved 8.1。 postoperatively. The mean preoperative anterior and posterior height of the vertebral height were 56.9% and 88.8% and improved postoperatively 94.7 and 88.8 respectively. The mean preoperative neural canal impingement in neural defici: patient was 47.2% and was improved 30.65% postoperatively. The mean prooperative ASIA motor was 34.1 and improved 41.7 postoperatively. There was no metal failure. These data suggested that the posterior transpedicular screw fixation, transpedicle bone graft, decompression and posterolateral bone graft was able to porvide sufficient stability and to achieve the neurologic improvement.
